1/13. Mucinous tumor of the gallbladder with a separate nodule of anaplastic carcinoma.

    A case of mucinous tumor of the gallbladder with a separate nodule of anaplastic carcinoma is reported. The patient was an 83-year-old Japanese man who underwent cholecystectomy under the preoperative diagnosis of a mucus-producing gallbladder tumor. A mucinous tumor was found in the neck and distal body of the gallbladder, associated with a separate nodule in the fundus. The latter nodule was initially diagnosed as a benign xanthogranulomatous lesion. However, the immunohistochemical study revealed that the atypical cells in the superficial part of the nodule were positive for cytokeratin and epithelial membrane antigen, confirming the diagnosis of anaplastic carcinoma. Although the occurrence of mural nodules in mucinous cystic tumors of the ovary and pancreas is well reported, to our knowledge, this is the first report on the occurrence of a mucinous tumor with a nodule of anaplastic carcinoma in the gallbladder.

2/13. Usefulness of MR imaging in the postsurgical monitoring of gallbladder cancer in a patient with bile duct cancer that developed 7 years after resection of mucinous adenocarcinoma of the gallbladder.

    We encountered a case of left hepatic duct cancer that developed 7 years after surgical resection of early-stage adenocarcinoma of the gallbladder. A 65-year-old woman was hospitalized with high fever and general fatigue. She also had elevated serum levels of alkaline phosphatase, gamma-glutamyltranspeptidase, and carbohydrate antigen 19-9. Seven years earlier, she had undergone extended cholecystectomy and resection of the extrahepatic bile duct for early-stage mucinous adenocarcinoma of the gallbladder. Conventional examinations did not reveal any responsible lesions. Magnetic resonance (MR) cholangiography, however, showed a tumor obstructing the left hepatic duct, and dynamic MR images revealed multiple foci of bacterial abscess in the liver. Surgically resected tissue again revealed mucinous adenocarcinoma. The present case is rare in that metachronous mucinous adenocarcinoma of the biliary system occurred after a long interval. This case suggests the usefulness of MR imaging in the postsurgical monitoring of patients with gallbladder carcinoma.

3/13. Mucin-producing carcinoma of the gallbladder: imaging demonstration in four cases.

    We report four cases of mucin-producing carcinoma of the gallbladder (three papillary adenocarcinomas and one mucinous carcinoma), with an emphasis on imaging features. Our findings suggest that when a papillary protrusion or thickened wall, including cystic areas and/or calcifications, is seen radiologically in the enlarged gallbladder, mucin-producing carcinoma of the gallbladder should be included in the differential diagnosis.

4/13. Imaging findings in mucin-producing carcinoma of the gallbladder.

    Mucin-producing carcinoma of the gallbladder is rare, and its imaging features have not been well documented. Its symptoms and signs are nonspecific and the prognosis of the disease is poor. We present a 73-year-old male with mucin-producing carcinoma of the gallbladder, emphasizing its imaging features on ultrasound, computed tomography and magnetic resonance imaging. The tumor usually presents as a cauliflower-like soft tissue mass with small calcified spots in the gallbladder wall and some laminated high viscosity fluid inside the gallbladder cavity. Recognition of these features can provide clues to the diagnosis of mucin-producing carcinoma of the gallbladder.

5/13. Composite endocrine cell, typical adenocarcinoma and signet ring carcinoma of the gallbladder.

    The authors report a case of composite tumor of the gallbladder which exhibited features of both adenocarcinoma (with typical areas, signet ring cells and clear cells) and endocrine cell carcinoma. The histological picture suggests the origin of both components in a common primitive cell with the capacity to differentiate to different types of metaplastic epithelium and supports the hypothesis of a sequence metaplasia--dysplasia--carcinoma.

6/13. An autopsy case of triple cancers including signet-ring cell carcinoma of the breast--report of a rare case with reference to a review of the literature.

    A 67 year old Japanese woman who had been suffering from general fatigue was diagnosed as having metastasis of signet-ring cell carcinoma of the bone marrow from a biopsied specimen. A clinical effortive search to the systemic organs revealed a tumor of 4 x 3 cm in size in the lower part of her left breast, which was subsequently diagnosed as the primary site histopathologically. The patient was immediately treated surgically, however, her general condition generally deteriorated and she expired 5 months after the operation. autopsy revealed dissemination of tumor cells in the bone marrow, bilateral pleura, and soft tissue around the operated site. The autopsy additionally revealed squamous cell carcinoma of keratinizing type in the uterine cervix and well differentiated tubular adenocarcinoma in the gallbladder. A review of the literature revealed this case to be the first reported case of triple cancers including signet ring-cell carcinoma of the breast, proven by autopsy in japan.

7/13. Signet-ring cell carcinoid of the gallbladder.

    A case of signet-ring cell carcinoid of the gallbladder is reported. The tumour diffusely infiltrated the gallbladder wall and extensively ulcerated the mucosa. Neoplastic nests were composed of numerous signet-ring cells mixed with clear endocrine cells. The latter expressed chromogranin a, gastrin and somatostatin and contained neurosecretory granules. The diagnostic problem of differentiating between signet-ring cell carcinomas and composite adenocarcinoma-carcinoid tumours is discussed.

8/13. meningeal carcinomatosis from a clinically undiagnosed signet-ring cell primary in the gallbladder.

    A patient with meningeal carcinomatosis is reported in whom the primary--which was not discovered clinically despite extensive investigation and cytological examination--proved to be the rare signet-ring cell tumor of the gallbladder. Determination of the nature and site of origin of the neoplastic cells in cytospin preparations of cerebrospinal fluid was complicated by the failure of conventional methods to stain cytoplasmic mucin. It is hoped that the application of monoclonal antibodies may resolve similar diagnostic problems in the future.

9/13. Case report: an unusual case of gallbladder cancer in an automotive worker.

    Mucinous adenocarcinoma of the gallbladder was found at autopsy in a 65-year-old male. The tumor was composed predominantly of signet ring cells, an uncommon histologic form. In addition, there was an atypical pattern of metastasis, with cancer involving not only the walls of the cystic duct, large and small bowels, peritoneum, abdominal nodes, and omentum, but also the kidneys, ureters, lungs, and heart. Although the patient lacked the common predisposing factors for gallbladder cancer, he did work in an industry with a significant relative risk for the development of this cancer.

10/13. Primary carcinoma of the gallbladder associated with chronic inflammatory bowel disease.

    patients with chronic inflammatory bowel disease have an increased risk of developing carcinoma of the bile ducts. This risk is highest in those with a long history of colitis and total involvement of the colon. The majority of these biliary tract cancers involve the extrahepatic or intrahepatic bile ducts. Occasionally, however, the gallbladder is the site of origin of the tumour. We report three cases of primary carcinoma of the gallbladder complicating chronic inflammatory bowel disease. In two, there was universal chronic ulcerative colitis and in one, Crohn's ileocolitis.
