1/18. Acrokeratosis paraneoplastica (Bazex syndrome) with adenocarcinoma of the colon: report of a case and review of the literature.

    Acrokeratosis paraneoplastica is a rare disease and is uncommon even in patients with upper aerodigestive tract cancer. We report a 63-year-old man with a 1-month history of numerous pruritic lesions and vesicles on both feet. Although he had received local therapy, progressive dense scale formation involving both palms and both soles was found. colonoscopy was performed because of hematochezia, and it revealed an early colon cancer. After the resection of the cancer, the skin lesions began to fall off dramatically. To the best of our knowledge, there is no report of acrokeratosis paraneoplastica associated with colon cancer in the literature. This is the first case report of acrokeratosis paraneoplastica associated with early colon cancer.

2/18. A case of alpha-fetoprotein-producing gastric cancer.

    INTRODUCTION: A case of alpha-fetoprotein (AFP)-producing gastric cancer is described in a 57-year-old Chinese woman. CLINICAL PICTURE: She presented with bleeding tendency and bone pain, and was found to have haematological evidence of disseminated intravascular coagulation and spinal metastasis. Her tumour markers, including AFP, Ca 19-9 and carcinoembryonic antigen (CEA) were elevated. In view of the elevated tumour markers, there was an exhaustive search for a primary lesion in the gastrointestinal tract, liver and ovaries. There was no radiological evidence to suggest any lesion in the chest, liver or pelvis. Lectin affinity electrophoresis of the AFP showed AFP-L2 and AFP-L3 bands, which are suggestive of a non-hepatoma malignancy. MANAGEMENT: gastroscopy showed a gastric ulcer and she developed bleeding after the gastric biopsy which required urgent surgery. Intraoperatively she was found to have carcinomatous peritone and a malignant ulcer in the greater curve of the stomach. histology confirmed a linitis plastica like adenocarcinoma which stains for AFP. OUTCOME: She died from multi-organ failure 3 days after surgery. CONCLUSION: AFP-producing adenocarcinoma of the stomach is not uncommon. Lectin affinity electrophoresis of AFP is helpful in the differentiation between hepatoma and non-hepatoma malignancies.

3/18. Difficulties with the diagnosis of linitis plastica carcinoma of the rectum and anal canal.

    The early and precise diagnosis of linitis plastica-type tumours of the rectum and anal canal is difficult because of their insidious presentation and anaplastic nature. A case report is documented of a patient with a linitis plastica tumour of the lower rectum and anal canal which arose as a secondary tumour from the prostate. The presentation, clinical features, treatment and pathological findings in this case are contrasted with those from other reported cases of prostatic carcinoma invading the rectum and of linitis plastica carcinoma of the large bowel. The unique features of the case are summarized.

4/18. Acrokeratosis paraneoplastica (Bazex syndrome): an atypical presentation.

    A 62-year-old male presented with a 2-year history of hyperkeratotic lesions of the hands and feet. Previous treatment with topical steroids was unsuccessful. A complete physical examination revealed the presence of blood in the stool, and sigmoidoscopy showed an ulcerative growth at the rectosigmoid junction. The histopathology showed adenocarcinoma.

5/18. Metastatic carcinoma of the colon similar to Crohn's disease: a case report.

    A 68-year-old Japanese man with a history of linitis plastica carcinoma of the stomach and subsequent gastrectomy 8 years previously presented with lower abdominal pain. Radiological and endoscopic examinations showed multiple submucosal nodular lesions similar to Crohn's disease in the ileocecal area. A firm diagnosis could not be made after initial multiple biopsies. Finally, a submucosal biopsy revealed adenocarcinoma. The ileocecal lesion was diagnosed as a recurrence because of the histological findings, which included mucosal preservation, a similarity with the histologic type of stomach carcinoma, and atypical immunoreactivity for primary colon carcinoma; the lesion was negative for both cytokeratin 7 and cytokeratin 20. In cases where metastatic carcinoma of the colon is suspected, we recommend early consideration of a submucosal biopsy.

6/18. Flexural Bazex syndrome associated with tonsillar adenocarcinoma.

    Bazex syndrome (acrokeratosis paraneoplastica) is characterized by a psoriasiform skin eruption that favors acral sites and has been associated with underlying malignancy in all reported cases. Squamous cell carcinoma of the upper third of the respiratory system and upper third of the gastrointestinal tract and squamous cell carcinoma of unknown primary with cervical lymph node metastasis are the most commonly associated neoplasms. Here we report a case of flexural Bazex syndrome with tonsillar adenocarcinoma.

7/18. Urogenital linitis plastica metastatic from stomach.

    We herein describe an example of gastric linitis plastica metastatic to the urinary bladder. A comprehensive appraisal of the literature yields only one similar reported case. Our patient experienced an objective partial response to neoadjuvant chemotherapy.

8/18. Gastric linitis plastica metastatic to the uvea.

    A 59-year-old man underwent total gastrectomy for diffuse, poorly differentiated gastric adenocarcinoma diagnosed as linitis plastica. Loss of vision in the right eye 5 months later due to extensive choroidal tumours was the first indication of metastatic disease. Radiologic studies showed multiple bony metastases. The blind, painful eye was enucleated. Pathological examination of the globe showed massive metastatic mucus-secreting adenocarcinoma of the choroid, with positive immunohistochemical staining for carcinoembryonic antigen (CEA) of the foci of the more highly differentiated neoplastic cells. The plasma CEA level had been normal. The patient died 3 months after enucleation from metastatic disease.

9/18. CT or US-guided fine needle aspiration biopsy in gastric neoplasms.

    Gastric carcinoma of the linitis plastica type is occasionally difficult to diagnose endoscopically because of the large inflammatory response and the sparsity of tumor cells. Five patients who presented with signs and symptoms of gastric carcinoma underwent upper gastrointestinal endoscopy to confirm the diagnosis of carcinoma. In each case the gross appearance of the stomach was felt to represent gastric carcinoma but the biopsy and/or brushing specimens were unable to make the diagnosis. Ultrasound or CT in each of these five patients demonstrated thickening of the gastric wall and, in one instance, evidence of extensive metastatic disease. Fine needle aspiration biopsy was performed and a diagnosis of adenocarcinoma was made cytologically. Four were primary gastric adenocarcinoma of the linitis plastica type and one was metastatic adenocarcinoma from the breast. It is suggested that guided aspiration biopsy be performed when the diagnosis of gastric carcinoma cannot be confirmed endoscopically.

10/18. Crohn's disease-associated carcinoma. A poorly recognized complication of inflammatory bowel disease.

    We have reviewed eight cases of Crohn's disease associated carcinoma (CDAC) of the bowel treated at Saint Barnabas Medical Center since 1977. Five patients had colorectal carcinoma in areas of dysplasia within histologically recognizable Crohn's disease. One of the large bowel carcinomas was a diffusely infiltrating signet ring adenocarcinoma (linitis plastica), three were mucinous carcinomas, and one contained both cell types. survival ranged from 4 to 55 months. Three patients developed ileal carcinomas in areas of dysplasia within histologically recognizable Crohn's disease. One of the ileal cancers was a moderately differentiated adenocarcinoma; two were poorly differentiated adenocarcinomas. survival ranged from 8 to 44 months. The dysplastic changes seen in the bowel adjacent to the tumors in these patients were identical to the characteristic pre-cancerous (dysplastic) changes well described in ulcerative colitis. The histopathologic changes seen in this high-risk group of patients are also similar to those of previously reported CDAC. Those patients with the more diffuse dysplastic changes might have been detected before the development of invasive cancer had they undergone periodic colonoscopic surveillance. One patient in the series with an asymptomatic lesion was, in fact, identified at surveillance colonoscopy. It would appear that Crohn's disease patients have a similar risk for carcinoma previously recognized in ulcerative colitis patients and that surveillance protocols should be developed for this group of patients.
