1/234. Repair of an aneurysm of the pulmonary trunk in a 65-year-old patient.

    The case of a 65-year-old patient with asymptomaticaneurysm of the pulmonary trunk associated with severe insufficiency of the pulmonary valve and symptomatic coronary artery disease is presented. The surgical procedure included coronary artery bypass grafting, aneurysmectomy, and pulmonary artery replacement with implantation of a stentless bioprosthesis and lengthening of the root of the bioprosthesis with a reversed vascular Y prosthesis, which was anastomosed to the left and right pulmonary artery.

2/234. Atherosclerotic aneurysms of the superficial femoral artery: report of two ruptured cases and review of the literature.

    Isolated arteriosclerotic aneurysms of the superficial femoral artery are rare. In citing the literature a total of 30 cases in 28 patients in the last 25 years were found. In addition to the above cases, two aged patients with ruptured aneurysms of the superficial femoral artery are reported; these were managed successfully with partial aneurysmectomy and restoration of the circulation of the extremity with a synthetic graft. The prognosis for this type of aneurysm following surgical therapy is good, despite the advanced age of the patients, and amputation is relatively rare, occurring in only two out of the 30 aneurysms (6.6%) reported. The risk of rupture is 46.6% (14/30) and is greater than that found in peripheral aneurysms. This, in association with the possibility of the creation of thrombosis (5/30; 16.6%) or embolization (1/30; 3.3%), threatens the extremity itself as well as the life of the patient, increasing the risk of complications and even death at a rate of 66.6% (20/30). Timely diagnosis, immediate surgical reconstruction and prompt mobilization, however, can guarantee a good prognosis for these aged patients.

3/234. aneurysm of saphenous vein coronary bypass graft: diagnosis by computed tomography.

    A patient with a history of coronary artery bypass graft surgery underwent computed tomography scanning for evaluation of a lung mass. A heterogeneous mediastinal mass discovered incidentally on computed tomography scanning was shown to be a saphenous vein bypass graft aneurysm.

4/234. Late development of an aneurysm of a saphenous vein used as an aortocoronary conduit.

    A case of a large saphenous vein aortocoronary aneurysm that developed late after coronary artery bypass grafting is presented. This is the first case of a large saphenous vein aortocoronary aneurysm identified by serial angiography and 3-dimensional computed tomographic scanning.

5/234. Left main coronary artery compression by aneurysmal pulmonary artery in a patient with tetralogy of fallot with absent pulmonary valve.

    We describe an 11-year-old girl with tetralogy of fallot and absent pulmonary valve, who on selective coronary angiography was found to have extrinsic compression of the left main coronary artery by the aneurysmally dilated pulmonary artery. This abnormality has not been reported previously.

6/234. subclavian steal syndrome and flow-related aneurysm. Another reason to treat.

    A 48-year-old woman presented with a symptomatic right subclavian steal syndrome due to proximal subclavian artery stenosis. Anatomically the innominate artery was absent. collateral circulation followed the vertebro-vertebral pathway with reversal of blood flow in the ipsilateral vertebral artery. There was also multiple dilated intervertebral collaterals and an associated saccular aneurysm on one of them. Surgical carotid-subclavian transposition permitted relief of clinical symptoms, disappearance of collateral circulation and subtotal regression of the aneurysm. This spontaneous evolution confirmed the role of high-flow in the pathogenesis of some aneurysms and the habitually good prognosis of flow-related aneurysms with correction of the cause. Arteriography still appears essential in diagnosis, pretherapeutic assessment and sometimes post-treatment evaluation in subclavian steal syndrome.

7/234. Calcified splenic artery aneurysm discovered during coronary arteriography.

    We present a case of calcified splenic artery aneurysm discovered during routine coronary angiography. Early awareness of these aneurysms is important since their rupture may cause serious complications. This case emphasizes the importance of attention to cardiac and extra-cardiac structures during coronary angiography.

8/234. The use of an external-internal shunt in the treatment of extracranial internal carotid artery saccular aneurysms: technical case report.

    BACKGROUND: Extracranial internal carotid artery aneurysms (EICAA) are rare lesions. Resection and grafting is the preferred method of management. However, the details of shunt use in surgery for this type of aneurysm has been described in few articles. We describe an external-internal shunt with intra-aneurysmal trans-orifice insertion. CASE REPORT: A 55-year-old woman presented with a 5-year history of a progressively enlarging pulsatile neck mass. An examination revealed no neurological deficit. Right carotid angiogram showed a saccular EICAA involving the ICA distal to the bifurcation, with kinking of the internal carotid artery (ICA). The dome of the EICAA extended from the upper border of C4 to the midportion of C2 and the maximum diameter was 4 cm. RESULTS: Using the shunt technique, we successfully removed the aneurysm and reconstructed the ICA. The end-to-end anastomosis was easy because the shunt was involved only in the distal free end of the ICA, but not in the proximal free end of the ICA. CONCLUSION: This technique could be an option for the treatment of EICCA when a shunt is needed to maintain the cerebral circulation.

9/234. A case of extra-hepatic portal vein aneurysm: evaluation by 3-dimensional computerized tomography angiogram.

    portal vein aneurysms (PVAs) are rare lesions associated with congenital vascular anomalies or chronic portal hypertension. Although usually benign, they occasionally lead to complications such as aneurysmal rupture, porto-systemic shunts, mural thrombosis in the portal vein, and compression of the biliary tract. So far, the diagnosis of these lesions has been dependent on 2-dimensional imaging modalities such as computed tomography (CT) or magnetic resonance imaging (MRI), or invasive procedures such as percutaneous transhepatic portography. Here we present the first documented case of an extra-hepatic portal vein aneurysm evaluated by 3-dimensional CT angiography. This easily performed and accurate imaging technique may obviate the need for invasive angiographic procedures in the future for the 3-dimensional characterization of deep vascular malformations in the portal circulation.

10/234. pulmonary artery compression by a giant aortocoronary vein graft aneurysm.

    Late failure of saphenous vein aortocoronary bypass grafts is predominantly due to vein graft atherosclerotic disease. Rarely, saphenous vein aortocoronary bypass grafts undergo aneurysmal degeneration. We report a case of a giant true aneurysm of a saphenous vein aortocoronary bypass graft producing right heart failure from main pulmonary artery compression.
