1/9. Nodular perianal herpes simplex with prominent plasma cell infiltration.

    BACKGROUND: Nodules are exceptional manifestations of herpes simplex virus (HSV) infection in immunocompromised patients. Only two cases of nodular HSV-2 infection of the perianal region have been reported previously. GOAL: The case of a 46-year-old homosexual man with AIDS presenting with painful perianal nodules resembling squamous cell carcinoma is described. STUDY DESIGN: This case report presents details of the histologic findings and treatment regimen. RESULTS: Histologic examination showed the presence of rare multinucleated giant epithelial cells and a dense inflammatory infiltrate composed mostly of plasma cells. polymerase chain reaction analysis was positive for HSV-2 and negative for HSV-1, cytomegalovirus, Epstein-Barr virus, and human herpesvirus types 6 and 7. After being treated ineffectively with oral acyclovir (4 g/d) for 15 days, the patient was treated with oral valacyclovir (6 g/d), resulting in marked improvement in 10 days and complete resolution after 2 months. CONCLUSIONS: In immunocompromised patients, HSV-2 infection may present with atypical clinical and histopathological features.

2/9. Unusual presentation of early lymphogranuloma venereum in an hiv-1 infected patient: effective treatment with 1 g azithromycin.

    The incidence of lymphogranuloma venereum (LGV) is low in the western world. Early LGV is characterised by bubonic disease following a painless papule or small ulcer. We report a white bisexual male who presented with a painful perianal ulcer, inguinal lymphadenitis, and concomitant infection with human immunodeficiency virus 1 (hiv-1). chlamydia trachomatis serovar L2 was identified as the cause after polymerase chain reaction and genotyping the major outer membrane protein by restricted fragment length polymorphism. Treatment with a single dose of 1 g azithromycin was effective. This case illustrates that early LGV may mimic other genital ulcer diseases, such as genital herpes or chancroid, especially in HIV infected patients. In the western world, LGV must still be included in the differential diagnosis of bubonic disease with or without sexually acquired ulcers.

3/9. herpes simplex virus type-2 ulcers resistant to acyclovir in an AIDS patient--successful treatment with foscarnet.

    The case of a 25-year-old homosexual man with large inguinal and perianal ulcers is reported. He had been pretreated extensively with acyclovir. herpes simplex virus (HSV) was identified in the ulcer tissue by in-situ hybridization. No clinical improvement with acyclovir but a prompt response to foscarnet was noted. In a relapse after 11 weeks an acyclovir-resistant HSV type 2 was isolated. Again, a prompt response to foscarnet was noted. The case is presented in detail, and the clinical impact of resistance of HSV to acyclovir is discussed.

4/9. Chronic perianal herpes simplex in immunocompromised hosts.

    Four immunosuppressed patients are described with chronic ulcerative herpes simplex virus infection in the sacral and perianal area. Three of these patients were evaluated for decubitus ulcers. Prompt diagnosis was possible when the characteristic morphologic features were recognized and when viral culture and Tzanck smear specimens were obtained. Previously reported cases are reviewed as well. Chronic mucocutaneous herpes simplex infections are complications of immunocompromised hosts and should be recognized early if appropriate therapy is to be initiated.

5/9. Acquired immune deficiency syndrome (AIDS) manifesting as anogenital herpes zoster eruption: demonstration of virus-like particles in lymphocytes.

    A case of AIDS manifesting as a necrotic, haemorrhagic anogenital herpes zoster and lymphadenopathy, is described. Investigations of t-lymphocyte subsets showed decreased T4 and slight increase of T8. Electronmicroscopy revealed tubuloreticular virus-like structures in lymphocytes.

6/9. Severe acquired immunodeficiency in male homosexuals, manifested by chronic perianal ulcerative herpes simplex lesions.

    Four homosexual men presented with gradually enlarging perianal ulcers, from which herpes simplex virus was cultured. Each patient had a prolonged course characterized by eight loss, fever, and evidence of infection by other opportunistic microorganisms including cytomegalovirus, pneumocystis carinii, and candida albicans. Three patients died; Kaposi's sarcoma developed in the fourth. All were found to have depressed cell-mediated immunity, as evidenced by skin anergy, lymphopenia, and poor or absent responses to plant lectins and antigens in vitro. Natural-killer-cell activity directed against target cells infected with herpes simplex virus was depressed in all patients. The absence of a history of recurrent infections or of histologic evidence of lymphoproliferative or other neoplastic diseases suggests that the immune defects were acquired.

7/9. Anorectal herpes: radiographic findings.

    radiography revealed aphthous ulcers, plaquelike erosions, and deep collar button ulcerations in a patient with herpes involving the rectum and the sigmoid colon. It is concluded that herpes simplex virus should be added to the list of disease entities that produce aphthous and collar button ulcerations.

8/9. Acute motor paralytic bladder in renal transplant patients with anogenital herpes infection.

    We report on 2 renal transplant patients in whom acute urinary retention developed after anogenital herpes infections. In 1 case a reversible bladder motor and sensory neuropathy occurred secondary to herpes simplex virus infections. In the other case a motor paralytic bladder developed secondary to an anogenital varicella-zoster infection. documentation was by carbon dioxide cystometrography and denervation hypersensitivity testing. Both cases were reversible without alteration of the immunosuppressive regimens.

9/9. Perianal cytomegalovirus ulcer in an HIV-infected patient.

    A 51-year-old man with acquired immunodeficiency syndrome presented with a perianal ulcer. Histologically there were numerous cytomegalic inclusions within endothelial cells and stomal fibroblasts. cytomegalovirus within infected cells was demonstrated by immunohistochemistry and in situ hybridization. No cytopathic typical of herpes simplex were detected. The ulcer resolved completely after a treatment with ganciclovir.
