1/20. lichen planus with involvement of all twenty nails and the oral mucous membrane.

    A 57-year-old man had had deformities of all ten fingernails for one and a half years before presentation and deformities of all ten toenails for the previous six months. The surfaces of the nails were rough, with excessive longitudinal striations. The bases of the nails were slightly hypertrophic, and the tips were atrophic and itchy. A longitudinal nail biopsy including the nail matrix revealed the typical histology of lichen planus. Reticulated pigmentation, maceration, and erosion on the buccal mucous membrane were also discovered. Histological analysis of the buccal mucous membrane revealed lichen planus intermingled with eosinophils. Immunological blood analysis revealed elevated CD4 T cells and CD4/CD8 ratio. He worked as a tinsmith and had dental metal. The metal series patch test revealed positive reactions to chromate and tin. Treatment with systemic steroids was quite effective in treating the nail lesions.

2/20. Report of a new case with four skin diseases.

    A 70 year old man is presented with four distinct skin diseases that were diagnosed concomitantly. lichen planus, vitiligo, lichen sclerosis, and disseminated actinic porokeratosis were all diagnosed clinically and histologically in this man, who was otherwise healthy.

3/20. Squamous cell carcinoma and lichen sclerosus et atrophicus of the prepuce.

    Two cases of squamous cell carcinoma of the prepuce arising on balanitis xerotica obliterans are described. This event is unusual and not well known. The surgical treatment was a wide circumcision in which the prepuce and part of the shaft skin were removed, performing as well a decortication of the glans base. This technique seemed to be satisfactory in removing the carcinoma and obtaining a definite improvement in the clinical picture of balanitis xerotica obliterans as well.

4/20. lichen nitidus: a histologic and electron microscopic study.

    A lesion of lichen nitidus in a 53-year-old man was examined by histology and electron microscopy. The histology showed typical features. The electron microscopy revealed primary changes in hydropic degeneration of basal cells which possibly indicated severe edema between the basal keratinocytes and in the subepidermal area. The basal keratinocytes themselves were not particularly altered, except for clustered micro-vacuolar structures in a small portion of the peripheral cytoplasm. In one of the eight blocks observed, the cytoplasmic process of a mast cell was found between the basal keratinocytes, indicating that mast cells may possibly infiltrate the epidermis in lichen nitidus. Scattered mast cells in the dermis were also noted. Thus primary pathologic changes of lichen nitidus may be induced by severe edema, which was accompanied by inflammatory cells including mast cells, in the region of the dermal-epidermal junction.

5/20. Annular atrophic lichen planus.

    We describe a case of generalized, asymptomatic, chronic annular lichen planus. The centers of the large annular lesions were atrophic and had lost their elasticity. Histologic examination revealed an absence of elastic fiber in the center. Electron microscopy revealed fragmented elastic fibers. Examination of early lesions showed that elastolysis had already begun in the heavy cellular infiltration. We conclude that the annular configuration resulted from the elastolytic activity of the inflammatory cells with the formation of foci of localized acquired cutis laxa.

6/20. Morphea coexisting with lichen sclerosus et atrophicus.

    A 65-year-old woman presented with four autoimmune diseases. These include morphea, lichen sclerosus et atrophicus, insulin-dependent diabetes mellitus, and autoimmune thyroid disease. The relationship between morphea and lichen sclerosus et A is discussed, as is the link between these conditions and autoimmunity.

7/20. Verrucous carcinoma in penile lichen sclerosus et atrophicus.

    A patient presented with a verrucous carcinoma within a lesion of penile lichen sclerosus et atrophicus. We report a case of lichen sclerosus and the specific squamous cell carcinoma variant, verrucous carcinoma, treated by mohs surgery.

8/20. Diffuse fasciitis with eosinophilia associated with morphea and lichen sclerosus et atrophicus.

    Diffuse fasciitis with eosinophilia (DFE) is a well described syndrome, among the connective tissue disorders. However, DFE is not commonly accepted as an own entity, because of its rare evolvement in scleroderma and the often indistinguishable histological pattern of both diseases. Here we report on the association of DFE and morphea in two patients and an additional lichen sclerosus in one of them. This points to a close relationship of DFE and other connective tissue diseases.

9/20. Is childhood idiopathic atrophy of the nails due to lichen planus?

    Four children with symptomless or 'idiopathic' atrophy of the nails were studied. There were no other cutaneous signs. A longitudinal nail biopsy was performed on each patient and demonstrated active lichen planus in three cases and changes consistent with scarring lichen planus in the fourth.

10/20. lichen sclerosus et atrophicus following radiation therapy.

    Several years after postmastectomy radiation therapy, lichen sclerosus et atrophicus (LSA) developed in two patients within the radiation fields. This sequence, to our knowledge, has not previously been described. The etiology of LSA is largely unknown, but trauma has been implicated as a provoking factor in some cases, and LSA might be an isomorphic response to the trauma of radiation therapy.
