1/4. An unusual case of an extra row of eyelashes in the superior eyelid skin crease.

    A diabetic patient was referred to the office for assessment of anterior blepharitis and associated tear film complications. On examination, he was noted to have bilateral, symmetrical rows of extra cilia within both upper eyelid skin creases. Differential diagnoses of aberrant cilia are discussed.

2/4. A case report of demodicosis.

    We report on a patient who had an excessive number of mites (Demodex folliculorum) on his eye lashes, i.e. 10 mites on eight lashes examined. A review of three previous reports suggested that one mite for every two lashes represents an over population of this nearly ubiquitous mite. Greasy scales on the lashes and foamy tears suggested that seborrhea was present as well as demodicosis. The primary symptoms of itching was relieved after one week of treatment with mercuric oxide ointment and lid scrubs, although the number of mites was not reduced. Treatment for an additional two weeks did reduce the mite number. We therefore recommend continuing treatment for demodicosis for at least three weeks even if symptoms abate sooner.

3/4. Topical vancomycin formulation for methicillin-resistant staphylococcus epidermidis blepharoconjunctivitis.

    We successfully treated two patients with severe staphylococcus epidermidis blepharoconjunctivitis by means of a topical vancomycin hydrochloride solution (50 mg/ml) prepared with sterile water. Aqueous vancomycin preparations, however, cause significant ocular irritation probably because of low pH and osmolality values. solutions prepared with normal saline (5 mg/ml) and phosphate-buffered artificial tears (5 mg/ml and 50 mg/ml) were significantly less irritating and possessed equivalent in vitro antimicrobial activity. Topical vancomycin should be used only when commercially available antibiotics are inadequate.

4/4. Eyelid dermatitis. A common, often confounding rash.

    Five common conditions and several less common ones cause eyelid dermatitis. diagnosis and treatment depend on recognizing their characteristics, obtaining a good history, performing a careful skin examination, administering and interpreting patch tests, prescribing the proper medications at the right time, and teaching the patient how to avoid trouble in the future. A case report illustrates the importance of integration and progression of diagnostic and therapeutic steps in resolving the management challenges of eyelid dermatitis.
