21/62. medulloblastoma in the nevoid basal-cell carcinoma syndrome: case reports and review of the literature.

    The nevoid basal-cell carcinoma syndrome (NBCCS) is a rare autosomal-dominant inherited disorder. Its clinical manifestations are multiple basal-cell nevi and cysts of the jaw along with skeletal anomalies and various combinations of numerous other defects. NBCCS is characterized by a marked propensity for developing cancers. One of the most frequently reported tumour is brain medulloblastoma. We are reporting two cases of NBCCS and medulloblastoma. A review of the case reports demonstrates certain prominent features of medulloblastoma associated with NBCCS. The patients generally are males, presenting at an unusually young age, under 5 or 2 years and show a longer survival rate. Its lay down to search for NBCCS in early medulloblastoma's, especially under 2 years.

22/62. Bilateral unicameral bone cysts in the hamate bones.

    This is a case report of bilateral unicameral bone cysts located in the hamate bones of a 22-year-old man. Unicameral bone cysts are rarely seen in the bones of the hand or bilaterally. In this case the diagnosis was made clinically using both radiographic and magnetic resonance imaging studies, and verified by microscopy of the biopsied specimen. Treatment consisted of curettage, autogenous bone grafting, and immobilization. To our knowledge there has not been any record in the literature of symmetrical unicameral bone cysts in the carpal bones.

23/62. Aneurysmal bone cyst of the fifth metacarpal.

    An aneurysmal bone cyst of the hand occurred in a 10-year-old girl. The hand is an extremely rare location for this lesion. A case report is presented along with a review of the medical literature.

24/62. A unilateral mesenchymal disorder of the head.

    A hitherto undescribed group of lesions consisting of cystic bony lesions, exostosis, fibromatous lesion, unilateral tonsillar hypertrophy, epidermoid cyst (cholesteatoma) and hyperplasia of the mandible confined to the left side of the face is reported. The case may represent a variant of the proteus syndrome.

25/62. Spontaneous healing of aneurysmal bone cysts. A report of three cases.

    We report three cases of spontaneous healing of aneurysmal bone cysts (ABC). In one case histological material was obtained after resection of the already ossified expansile mass discovered as a lytic lesion seven months previously. In the two other patients, spontaneous ossification of a radiologically presumed ABC in the lytic and expansile phase was observed after nine and seven months respectively. The healed lesions have remained stable at 12, 32, and 36 months respectively. These findings suggest that when the diagnosis can be made with confidence, and the lesion is in a location and at a stage that does not entail any risk of fracture or compression, expectant management should be considered. Our three patients were aged 22, 19 and 18 years, older than usual for developing ABC. This is also true for many of the few other reported cases of spontaneous or almost spontaneous healing and suggests that ABC has a greater tendency to stabilise in older patients.

26/62. Aneurysmal bone cyst of the thoracic spine: treatment by excision and segmental stabilization with Luque rods.

    Preoperative radiological evaluation with magnetic resonance imaging and computed tomography was valuable in planning the surgical management of a destructive lesion of the posterior elements of the thoracic spine that was causing spinal cord compression in an 18-year-old woman. Preoperative recognition of bilateral involvement of the pedicles in addition to the laminae and spinous process led to use of prophylactic segmental stabilization of the spine with Luque rods after successful excision of an aneurysmal bone cyst. This case provides an example of the usefulness of computed tomographic scanning and magnetic resonance imaging in assessing the distribution and location of vertebral tumor and its potential effect on spinal stability. The efficacy of combining radical excision with stabilization for treatment of aneurysmal bone cysts of the spine is emphasized.

27/62. Regression of aneurysmal bone cyst following open biopsy.

    Aneurysmal bone cysts are well described but relatively rare bone lesions. Histological diagnosis is usually obtained before definitive treatment. We report a patient with aneurysmal bone cyst regression after excisional biopsy and prior to preoperative embolization. This was also a patient with the William syndrome. This has not previously been reported, nor is there any known association between aneurysmal bone cyst and the William syndrome.

28/62. Rare benign bone tumors and dystrophy in the hand. review of literature and report of four cases.

    In 1986, the Groupe d'Etude des Tumeurs osseuses (Bone Tumors Studying Group), organised a multicentric study of benign bone tumors and dystrophies localised in the hand. The result was very poor, because only five cases were found. Adding these cases to those already published in the literature, we report: 58 aneurysmal cyst, 12 chondromyxoid fibroma, 9 chondroblastoma, 6 bone cyst, 5 monostotic fibrous dysplasia. The total of 90 cases seems to confirm that the hand is a very unusual localisation for these tumors. These concerned equally metacarpal and phalanx for 37 each, but the carpus was concerned in only 7. diagnosis was regularly missed before operation, and chondroma was usually suggested; the reason is probably that these tumors lost their usual characters in this particular localisation, except in two cases of fibrous dysplasia. In most cases curettage and cancellous graft seems sufficient to ascertain histology and cure the lesion.

29/62. Aneurysmal bone cyst: clinicopathologic features and treatment of ten cases involving the hand.

    Ten cases of aneurysmal bone cyst in the hand were identified among 208 such lesions in our institutional files. The patients (six women and four men) had a mean age of 27.3 years. Two patients had open physes. Seven lesions were in the metacarpals. Radiographic examination showed that in all cases the lesion was both expansile and completely lucent. There were no recurrences in the three patients treated by complete excision and bone grafting. curettage and bone grafting in seven cases was associated with four recurrences. Three of these four local recurrences were treated successfully with curettage procedures. Treatment of aneurysmal bone cysts of the small bones of the hand requires either thorough exteriorization, curettage, and bone grafting or excision and bone grafting.

30/62. Aneurysmal bone cyst in the metacarpal of a child: a case report.

    An unusual case of aneurysmal bone cyst in the metacarpal of a five-year-old girl is described. The lesion was initially diagnosed and treated as a simple bone cyst. Subsequently the affected metacarpal was excised and replaced by an autogenous fibular graft, with good results. A discussion of the pathogenesis of the lesion is given, with a review of the literature.
