1/535. Solitary keratoacanthoma of the nipple in a male. Case report.

    A heretofore undescribed localization for solitary keratoacanthoma, namely the skin of the nipple, is reported. The tumor developed in a 69-year-old man who had suffered a trauma in the thorax near the left nipple 5 months before. A possible origin in mammary ducts, considering the breast as a modified sweat gland, is discussed.

2/535. calcium oxalate crystals in benign cyst fluid from the breast. A case report.

    BACKGROUND: Two types of calcification have been observed in breast lesions. The more common is composed mostly of calcium phosphate and is detected in routine histologic tissue sections of frequently malignant lesions. The rare type is calcium oxalate and is found exclusively in benign cysts. CASE: In a 47-year-old female, strongly birefringent polyhedral crystals of calcium oxalate were detected in benign breast cyst fluid. CONCLUSION: calcium oxalate is not clearly visible on routine histologic sections, and examination of the cytologic specimens under polarized light reveals them. awareness of this potential pitfall might lead to conservative management.

3/535. Pneumocystography of benign and malignant intracystic growths of the female breast.

    Pneumocystography established the preoperative diagnosis of 20 intracystic breast tumours including 9 benign intracystic papillomas and 11 cases of intracystic carcinoma generally considered to be a rarity. The method is described and details about the clinical and radiologic appearances of both benign and malignant intracystic tumours are given. The importance and advantages of the method are emphasized.

4/535. Systemic multifocal fibrosclerosis.

    We describe a case of hydronephrosis as a result of retroperitoneal fibrosis in a patient who had previous sclerosing lobulitis of the breast. To the best of our knowledge this is the first reported association between these two conditions in the english literature. We presume these conditions are linked and unify them under the general heading of systemic multifocal fibrosclerosis.

5/535. Acute inflammatory (non-purulent) arthritis concomitant with a developing breast abscess.

    A 34 year old female presented 7 weeks post-partum with acute diffuse arthritic manifestations and pyrexia. Extensive investigations (grossly raised erythrocyte sedimentation rate [ESR], c-reactive protein [CRP], normal serology and others) were performed to pursue a diagnosis. Subsequently a breast abscess was diagnosed. Surgical treatment of this led to almost immediate resolution of the joint complaints and return of ESR/CRP to normal levels. This was considered a hitherto unreported case of acute non-purulent inflammatory arthritis concomitant to an existing infection elsewhere in the body. The pathogenic mechanism is unclear but speculatively toxin-mediated.

6/535. radiation-induced localized scleroderma in breast cancer patients.

    radiation-induced scleroderma in breast cancer patients appears to occur in approximately one out of every 500 patients. We report four cases that developed within 3 months of conservative breast surgery and postoperative radiation treatment. The reaction was contained entirely within the treatment field and demonstrated the typical features of this condition where the breast becomes erythematous, violaceous, indurated, retracted, and progressively pigmented. The breast tends to soften and become more comfortable over 1-4 years; however, significant induration, retraction and pigmentary changes remain. There appears to be no predictive factors. radiation-induced scleroderma must be differentiated from cellulitis and recurrent breast cancer.

7/535. False-positive radiographic diagnosis of breast implant rupture because of breast abscess.

    A case of a periprosthetic abscess simulating breast implant rupture is presented. Both clinical findings and film-screen mammography suggested extravasation of a radiodense material adjacent to an implant. ultrasonography was thought to confirm the extraluminal silicone. However, at surgery the mass was found to be a breast abscess that had herniated through the capsule. The double-lumen implant outer saline-filled chamber had deflated, but the silicone-containing inner chamber was intact. magnetic resonance imaging would have distinguished between abscess and silicone.

8/535. The clinical, histologic, and ultrastructural presentation of polyvinyl sponge (Ivalon) breast prostheses removed for massive fluid accumulation.

    The current study describes what we believe is the first report of bilateral massive seromas associated with open-cell Ivalon sponges. Additionally, the gross, histologic, and ultrastructural features consistent with previous reports of polyvinyl alcohol prostheses are presented. Despite the reported chemical inertness of polyvinyl alcohol, this material may incite a biologic response in some patients, leading to dense fibrosis and occasional foreign-body giant-cell reaction. It is postulated that the molecular breakdown products of the polyvinyl alcohol polymer may create an osmotic gradient across the periprosthetic capsule, which may lead to intracapsular fluid accumulation, as presented in this case.

9/535. calcium oxalate crystals (Weddellite) within the secretions of ductal carcinoma in situ--a rare phenomenon.

    A case is described in which calcium oxalate (Weddellite) crystals were identified in an area of ductal carcinoma in situ of the breast. Seventy other cases were examined but no evidence of Weddellite was detected. This is evidently a rare phenomenon in carcinoma in situ.

10/535. myiasis secondary to Sermatobia hominis (human botfly) presenting as a long-standing breast mass.

    A case of a 54-year-old woman who presented with a breast mass is reported. Histologically, a chronic granulomatous inflammatory response was observed. The response was associated with an organism diagnosed as a fly larva, Dermatobia hominis (human botfly). The incidence of myiasis, infestation by fly larvae, presenting as a long-standing breast mass and mimicking a neoplasm is extremely rare, especially in the united states.
