1/16. Human cyclosporiasis diagnosis: report of a case in Sao Paulo, SP, brazil.

    diagnosis of the human cyclosporiasis is reported in Sao Paulo, SP, Brasil. cyclospora cayetanensis has been identified in the feces of a patient by a modified Kinyoun staining method, with later sporulation in a solution of 2.5% potassium dichromate. The probability that this parasite is the eventual cause of gastrointestinal disturbances in the country was stimulated by this finding, which was arrived at by a simple technique. It had been kept in mind that the disease was expressing itself mainly among immunocompromised patients, whose number is increasing; especially in those with acquired immunodeficiency syndrome (AIDS), which is caused by the human immunodeficiency virus (hiv).

2/16. Two simultaneous cases of cyclospora cayatensis enteritis returning from the dominican republic.

    According to the "International Passenger Survey," published in 1996 by the Office of Trading Standards, 534,000 British people traveled to the Caribbean area (personal communication, ABTA, 1998). The dominican republic, the eastern end of a large Caribbean island, has become in recent years one of the most popular destinations for UK holidaymakers as well as for travelers from many other countries. cyclospora cayatensis has been firmly identified as a cause of gastroenteritis among international travelers,1 including human immunodeficiency virus (hiv)-positive individuals,2 but it has not been previously reported in the literature in British individuals returning from this increasingly popular vacation destination.

3/16. isospora belli infection. A report of two cases in patients with AIDS.

    OBJECTIVE: To present the findings of isospora belli infection in two patients with the acquired immunodeficiency syndrome (AIDS). CLINICAL FEATURES: One patient was part Aboriginal, the other an immigrant recently arrived from peru. Both men were infected with the human immunodeficiency virus (hiv). They presented with watery diarrhoea, one with severe weight loss. isospora belli oocysts were detected in multiple faecal specimens from both patients. One patient had a concomitant infection with cryptosporidium sp. INTERVENTION AND OUTCOME: There was a rapid response to treatment with cotrimoxazole (960 mg, four times a day for 10 days). A complete response was obtained in the patient with concurrent cryptosporidiosis. The diarrhoea recurred after cessation of treatment, necessitating maintenance therapy. CONCLUSION: Although isospora belli is an uncommon cause of gastroenteritis in australia, it is increasingly recognised as a cause of diarrhoea in travellers, immigrants from endemic areas and immunocompromised people. It has also been reported in the Aboriginal community; this article reports the first case of AIDS-related isosporiasis in an Aborigine. It is likely that both patients acquired isospora belli before they become hiv seropositive.

4/16. Case report--the acquired immunodeficiency syndrome and isospora belli infection.

    A patient with the acquired immunodeficiency syndrome (AIDS) presenting with isospora belli infection is reported. The difficulties encountered in the diagnosis and therapy of isosporiasis are discussed. The diarrhoea responded to treatment with intravenous fluid replacement and oral furazolidone.

5/16. thrombocytopenia and vomiting due to difluoromethylornithine.

    A 32-year-old Haitian male with acquired immunodeficiency syndrome presented with complications of isospora belli enteritis. Therapy with the investigational drug difluoromethylornithine was initiated. Severe thrombocytopenia, nausea, and vomiting developed during intravenous drug therapy and recurred upon rechallenge with low-dose oral difluoromethylornithine. Therapy was discontinued because of these severe adverse effects.

6/16. isospora belli infection: treatment with pyrimethamine.

    isosporiasis is an uncommon but important diarrheal disease of humans that, like cryptosporidiosis, is life-threatening in patients with the acquired immunodeficiency syndrome (AIDS). isospora belli infection responds rapidly to therapy with trimethoprim-sulfamethoxazole, but patients with AIDS have a high rate of adverse reactions to this therapy. The cases of two patients with AIDS, sulfonamide allergy, and I. belli infection are reported. They were treated successfully with pyrimethamine alone, 75 mg/d, and recurrence was prevented with daily pyrimethamine therapy, 25 mg/d. In patients with AIDS with sulfonamide allergy or intolerance, pyrimethamine alone seems to be a reasonable alternative therapy for I. belli infection.

7/16. isospora belli and cryptosporidium sp. from a patient not suspected of having acquired immunodeficiency syndrome.

    Several protozoa including cryptosporidium sp. and isospora belli were found in a stool specimen from a patient not suspected of having acquired immunodeficiency syndrome (AIDS). As a result of the parasitologic findings, serologic tests were ordered that verified the diagnosis of AIDS. All stool specimens are routinely tested for presence of acid fast organisms with a modified acid fast technique.

8/16. Acquired immunodeficiency with intestinal cryptosporidiosis: possible transmission by Haitian whole blood.

    A 31-year-old Frenchman had an acquired immunodeficiency syndrome (AIDS) with profound depression of cellular immunity and relative sparing of humoral immunity. The clinical picture included intractable secretory diarrhoea, vomiting, abdominal pain, and weight loss. Gastrointestinal cryptosporidiosis was present and a perfusion technique showed profuse secretion of fluid in the proximal small bowel. The patient also had recurrent salmonella typhimurium septicaemia, cytomegalovirus infection, and cerebral toxoplasmosis and he died within 13 months. This patient did not belong to any of the groups known to be affected by this type of acquired immunodeficiency (homosexuals, drug addicts, haemophiliacs, Haitians) but had been transfused with Haitian blood 4 years before onset of symptoms. This case supports the notion that some forms of AIDS may be transmitted by blood, with a long incubation period.

    isospora belli is a cause of protracted diarrhea in immunocompromised patients. acquired immunodeficiency syndrome (AIDS), seen mostly in homosexual men and narcotic addicts, predisposes affected persons to a number of opportunistic infections. As isospora belli has been reported only once in this group, we report isospora belli in an AIDS patient with chronic diarrhea.

10/16. isospora belli diarrheal infection in homosexual men.

    Unsporulated isospora belli oocysts were detected in the stool specimens of three homosexual men. The oocysts were ellipsoidal measuring 23-33 X 12-15 micron. It is acid fast with modified cold kinyoun stain (MCK) and reveals orange fluorescence with the Truant's stain. Sheather's sucrose flotation method is effective in concentrating isospora oocyst as with other coccidial oocysts. One specimen showed many Charcot-Leyden crystals. All three patients responded to treatment. Isosporiosis is rare in this country and is endemic in the tropics and subtropics. Although one case acquired the infection after returning from an endemic area, the other two cases had no travel history. This raised the suspicion that this, like other parasitic infections (amebiasis, giardiasis, cryptosporidiosis) may be sexually transmitted. Such exotic parasitic infections in homosexual men during the outbreak of the acquired immunodeficiency syndrome adds another unusual infectious agent to the differential diagnosis of diarrheal disease in this high risk group of population.
