1/6. Conjunctival rhinosporidiosis associated with scleral melting and staphyloma formation: diagnosis and management.

    PURPOSE: Although conjunctival rhinosporidiosis is endemic in india, associated scleral melting and staphyloma formation are quite rare. We report clinical features and management in three patients with this unusual presentation. methods: Retrospective review of case records of three patients with conjunctival rhinosporidiosis. RESULTS: infection occurred in young, healthy adults and was localized to the forniceal conjunctiva in all patients. The affected conjunctiva had numerous grey-white spherules, but a polyp-like lesion was not present in any patient. diagnosis was based on clinical features and examination of scrapings from the involved conjunctiva. Treatment was surgical, with excision of infected conjunctival tissues and staphyloma repair with homologous sclera or autogenous periosteum. Failure to recognize the conjunctival pathology in one patient resulted in recurrence of the staphyloma. CONCLUSION: Conjunctival rhinosporidiosis can be associated with scleral staphyloma in young, healthy, adults. Differentiating this entity from idiopathic scleral ectasia requires knowledge of the clinical features of conjunctival rhinosporidiosis and a high index of clinical suspicion.

2/6. Conjunctival rhinosporidiosis. Report of three cases from Zaire.

    The authors report three cases of conjunctival rhinosporidiosis from Zaire, two of which are believed to be the first documented in the northeastern part of the country. All patients were males. Two patients were very young children, respectively 5 and 6 years old. The third patient was a 35-year-old man. The diagnosis was based on histopathology which revealed the characteristic features of rhinosporidium seeberi. Total excision was the elective treatment. The disease recurred eleven months later in one case, probably from local dissemination of the parasite during surgery.

3/6. Conjunctival rhinosporidiosis.

    Conjunctival rhinosporidiosis is usually a surprise diagnosis in histological section of an excised conjunctival mass. The condition is rarely encountered outside the endemic coastal areas of South india. Accurate diagnosis of this rare condition is infrequent in clinical practice; tumour, neoplasm, papilloma being the common misdiagnoses. Herein, a report of a case of an 18-year-old otherwise healthy male who attended outpatient department of Regional Institute of ophthalmology, Medical College, Kolkata with a red fleshy papillomatous growth about 7 mm x 4 mm in size, in the palpebral conjunctiva just behind the intermarginal strip of his right upper lid. His routine blood examination was within normal limits. The growth was excised under local anaesthesia and histopathological examination revealed rhinosporidiosis. There was no recurrence of the growth within one month of follow-up.

4/6. Histopathologically confirmed ocular rhinosporidiosis in two Canadians.

    CASE REPORT: In india and Southeast asia, rhinosporidiosis is a common infectious disease, but it has rarely been reported in western countries. Infrequently, isolated ocular rhinosporidial infections have been reported, but to our knowledge, there are no reported cases in canada. Two cases of rhinosporidiosis have been recently diagnosed and managed at our university-based hospital. COMMENTS: rhinosporidiosis presents with certain characteristic clinical features; however, the diagnosis is confirmed histopathologically. The presence of typical sporangia and spores in a fibrovascular stroma infiltrated by acute and chronic inflammatory cells including granulomas is diagnostic. Surgical excision is the treatment of choice, and recurrence is possible but rare.

5/6. The histological and the histopathological pattern of conjunctival rhinosporidiosis associated with papillomavirus infection.

    The present study describes for the first time, the clinical, light and electron microscopic findings of two cases of conjunctival rhinosporidiosis. One was with concurrent infection of papillomavirus. Investigations at the ultrastructural level have provided additional information on the development of rhinosporidium seeberi and would suggest that the formation of the wall of this organism is a continuous morphological and biochemical spectrum throughout its cytological maturation. The current observation on the wall formation is probably a modification of the classical pattern as an environmental protection carried out by the fungus against the virus. In contradistinction to the usual histopathological picture of rhinosporidiosis, the case with the viral infection lacked the characteristic marked inflammatory reaction. This finding, together with the relatively short interval of the frequent recurrences of this lesion, have led us to postulate the presence of a localised acquired immune deficiency state. It is possible that this local immune deficiency may be caused by an immunosuppression mechanism. This is probably mediated by papillomavirus and/or due to the weak antigenicity of the host virus-infected cells which contain only copies of viral dna in an unintegrated form.

6/6. Conjunctival rhinosporidiosis.

    rhinosporidium seeberi is the fungus that causes rhinosporidiosis, which affects the mucocutaneous tissue of humans and domestic animals. Ten cases of conjunctival rhinosporidosis have been reported in the united states. This is a case report of conjunctival rhinosporidosis that is believed to be the 11th reported from the united states and the only case reported from arkansas. A clinical and pathologic description of the disease is given.
