1/424. Infectious crystalline keratopathy and endophthalmitis secondary to Mycobacterium abscessus in a monocular patient with stevens-johnson syndrome.

    PURPOSE: To describe the clinical and laboratory features of infectious crystalline keratopathy and endophthalmitis secondary to Mycobacterium abscessus in a patient with stevens-johnson syndrome. METHOD: Case report. A 19-year-old man with a history of stevens-johnson syndrome and multiple corneal transplants developed white crystalline corneal infiltrates. RESULTS: anterior chamber aspirate disclosed acid-fast bacilli. A repeat corneal transplant was performed and antibiotic therapy begun. Histopathology showed focal acute inflammation surrounding collections of acid-fast bacilli, which were speciated as M. abscessus. CONCLUSIONS: M. abscessus is a cause of infectious crystalline keratopathy and endophthalmitis. risk factors include ocular surface disease, corneal transplantation, and immunosuppressive therapy.

2/424. Peripheral lamellar keratoplasty for corneoscleral cyst: three case reports.

    PURPOSE: To examine whether peripheral lamellar keratoplasty (LKP) using preserved cornea was effective for the treatment of corneoscleral cysts. methods: Three patients with corneoscleral cysts underwent peripheral lamellar keratoplasty. Two patients had no history of trauma or ocular surgery and were considered to have congenital cysts. The other patient had a history of strabismus surgery that had been performed 7 years previously. The anterior wall of the cysts was removed by trephination, and the epithelial membrane lining the posterior wall was peeled off. Lamellar corneal buttons obtained from preserved corneas then were put in place and secured with 8-10 interrupted sutures. In one case, because the cyst was large and extended to the pupillary axis, peripheral LKP was performed for removal of the scleral and peripheral corneal cyst, and the inner wall of the central corneal cyst was removed with vigorous irrigation and a spatula. RESULTS: Histologic examination showed that all of the cysts were lined with nonkeratinizing epithelial cells. In all three cases, cysts have not reformed after a 1-5-year follow-up. CONCLUSIONS: The cysts were lined in epithelial cells, and removal of these epithelial cells was considered to be important for the prevention of recurrence. Peripheral LKP is effective for the treatment of corneoscleral cysts, since this procedure removes displaced epithelial cells and reconstructs the thin part of the cornea and sclera.

3/424. Long-term outcome of aqueous shunt surgery in ten patients with iridocorneal endothelial syndrome.

    PURPOSE: To report the long-term outcome of ten patients with iridocorneal endothelial (ice) syndrome who underwent aqueous shunt surgery for uncontrolled glaucoma. DESIGN: Noncomparative, retrospective case series. PARTICIPANTS: The authors reviewed charts of ten patients with ice syndrome-related glaucoma who underwent aqueous shunt surgery at one institution between 1987 and 1996. MAIN OUTCOME MEASURES: intraocular pressure (IOP), number of glaucoma medications, and further surgical interventions were measured. RESULTS: With a median follow-up of 55 months, four eyes had adequate IOP control (IOP <21 mm Hg) with one or two medications after the initial aqueous shunt surgery. An additional three eyes achieved adequate IOP control after one or more tube repositionings or revisions of the initial aqueous shunt. In this series, the aqueous shunt surgery most often failed because of blocking of the tube ostium by iris, ice membrane, or membrane-induced tube migration. CONCLUSION: Aqueous shunt surgery appears to be an effective method for IOP lowering in some eyes with ice syndrome-related glaucoma when medical treatment or conventional filtration surgeries fail, but additional glaucoma procedures and/or aqueous shunt revisions and tube repositionings are not uncommon.

4/424. Pseudo-Fleischer ring after hyperopic laser in situ keratomileusis.

    A 37-year-old woman had bilateral hyperopic laser in situ keratomileusis (LASIK). Six months postoperatively, an epithelial pigmentation ring pattern was identified on both corneas. The appearance of the ring pattern was similar to the iron deposits of the Fleischer ring of keratoconus. While corneal iron deposits in various patterns have been reported after other forms of ocular therapeutic and refractive surgery, this is the first report of the association between hyperopic LASIK and a corneal iron line, which we have called the "pseudo-Fleischer ring."

5/424. Combined phacoemulsification and penetrating keratoplasty.

    To highlight indications, technique, and advantages of closed-chamber phacoemulsification and intraocular lens (IOL) implantation during penetrating keratoplasty for corneal opacities. case reports of 2 patients who underwent combined phacoemulsification, IOL implantation and penetrating keratoplasty. The technique described allowed controlled capsulorrhexis, cataract removal and in-the-bag IOL implantation. Complications due to increased posterior pressure during open-sky extracapsular cataract were not encountered. The surgical technique described in this report can only be used in selected patients undergoing combined corneal transplant and cataract surgery. In this group of patients, however, the technique offers many intra- and postoperative advantages.

6/424. Pseudo-endothelial dystrophy associated with emulsified silicone oil.

    PURPOSE: To describe endothelial changes associated with emulsified silicone oil. methods: Report of a case. RESULTS: A 77-year-old man had multiple and diffuse clear vesicles on the endothelium of his left eye (LE). The cornea was clear and thin. He had undergone pars plana vitrectomy and intraocular silicone oil injection 5 years before presentation. Specular microscopy revealed numerous small bubbles of emulsified silicone oil and a mild degree of endothelial damage. CONCLUSION: Emulsified silicone oil can adhere to the endothelium and induce an apparent droplet-like endotheliopathy.

7/424. Characterization of epithelial downgrowth by confocal microscopy.

    A 43-year-old white woman with a history of multiple ocular surgeries, including 4 penetrating keratoplasties, developed a concentric retrocorneal membrane at the graft periphery in the right eye. A white-light, tandem, scanning confocal microscope using a 24x/0.60 contact objective was used to examine the right eye in vivo. At the endothelial layer, confocal microscopic images similar to corneal epithelial cells were detected at the graft periphery. Unlike normal endothelial cells, the imaged cells demonstrated easily recognizable nuclei.

8/424. An unreported side effect of topical clarithromycin when used successfully to treat mycobacterium avium-intracellulare keratitis.

    PURPOSE: To report a case of mycobacterium avium-intracellulare (MAI) keratitis successfully treated with topical clarithromycin. An unreported side effect of the topical medication is described. methods: A regular follow-up in the corneal clinic was arranged, and a pertinent literature search performed. RESULTS: The use of topical clarithromycin was successful in treating the keratitis. The patient did not complain of any ocular discomfort. Corneal subepithelial deposits that appeared during treatment with clarithromycin resolved shortly after the therapy was discontinued. CONCLUSION: This case report demonstrates that a rare infection like MAI keratitis can be successfully treated with topical clarithromycin. It also highlights the possible corneal deposition of this drug, which resolved after cessation of therapy.

9/424. Surgical management of coexisting pseudophakic bullous keratopathy and glaucoma.

    This technique is a 1-step surgical management approach for patients with pseudophakic bullous keratopathy, glaucoma, and an unsatisfactory intraocular lens (IOL). The outcome of 4 consecutive patients who had penetrating keratoplasty, IOL removal, vitrectomy, transscleral sutured IOL implantation, and trabeculectomy by the same surgeon were reviewed. All patients had had intracapsular cataract extraction with anterior chamber IOL implantation and were on antiglaucoma therapy. Main outcome measures were Snellen visual acuity and intraocular pressure. A 1-stage quintuple procedure can achieve relatively rapid visual rehabilitation in these high-risk eyes. The role of filtration surgery is controversial, but it was partially successful in controlling postoperative intraocular pressure.

10/424. Peters' anomaly: dominant inheritance in one pedigree and dextrocardia in another.

    Two case reports are described to illustrate the unusual occurrence of dominant inheritance of Peters' anomaly and the concomitant occurrence of Peters' anomaly with colobomatous microphthalmos and dextrocardia. Studies of additional families are necessary to determine conclusively the pathogenesis, genetic mode of inheritance, ocular and systemic associated malformations, and proper management of this complex entity.
