1/46. Coronary stent deployment in a young adult with Kawasaki disease and recurrent myocardial infarction.

    A 19-year-old man developed a huge coronary aneurysm and stenosis in the right coronary artery as a sequela of Kawasaki disease (KD) that resulted in recurrent episodes of myocardial infarction. Coronary ischemic events were successfully prevented after balloon angioplasty followed by coronary stent implantation into the stenotic lesion. The stent deployment may have an advantage compared with balloon angioplasty and other new devices for the treatment for patients with KD showing stenotic lesions without dense calcification.

2/46. Surgical management of arteriosclerotic coronary artery aneurysm.

    A 60-year-old man suffered antero-septal myocardial infarction at the age of 56. coronary angiography demonstrated total occlusion of the left anterior descending artery and a large saccular aneurysm of the right coronary artery. Diffuse coronary ectasia was also shown in the right coronary artery adjacent to the aneurysm. Despite anticoagulant therapy, the aneurysm formed a thrombus and developed coronary artery stenosis distal to the aneurysm. ligation of the aneurysm and in situ gastroepiploic artery grafting were performed. Sudden heart failure was developed during skin closure. As this condition was considered to be graft hypoperfusion, supplemental saphenous vein grafting was placed. ligation is a simple, reliable technique to prevent future complications for a large saccular right coronary artery aneurysm, however, gastroepiploic artery might be an inappropriate bypass conduit for the ligated coronary artery with diffuse ectasia.

3/46. Thrombosed giant coronary artery aneurysm presenting as an intracardiac mass.

    Giant coronary artery aneurysms are rare in adults and are usually found in association with Kawasaki's disease arising in childhood. We report a case of a thrombosed giant right coronary artery aneurysm presenting as an intracardiac mass detected after inferior wall myocardial infarction. Histologic analysis indicated that fibromuscular dysplasia was the underlying cause of the aneurysm.

4/46. Surgical repair of right-atrial aneurysm.

    atrial fibrillation and embolic events are the most common clinical symptoms of congenital right- or left-atrial aneurysms. We report an a case of righ-atrial aneurysm, in a patient with typical history of atrial fibrillation and history of stroke. The aneurysm was resected, but the patient suffered from acute embolic occlusion of the left anterior descending coronary artery on the fourth postoperative day despite of systemic heparinization with 300 IU/kg bw per 24 hours.

5/46. tissue plasminogen activator in pediatric myocardial infarction.

    Children rarely present to emergency physicians during an acute myocardial infarction. However, this may occur in the setting of Kawasaki disease with thrombosed coronary aneurysms. We present the first case in which intravenous tissue plasminogen activator was used to successfully treat a 7-year-old child having an acute myocardial infarction caused by a thrombosed coronary aneurysm.

6/46. ticlopidine plus aspirin for coronary thrombosis in Kawasaki disease.

    Selective inhibitors of the adenosine 5'-diphosphate pathway of platelet activation have been used rarely in children in the united states. We report the successful use of ticlopidine, together with aspirin, in a 7-month-old infant with Kawasaki disease complicated by a thrombus in a giant coronary aneurysm that failed to resolve with thrombolytic therapy. 7/46. Successful recovery after ventricular fibrillation in a patient with Kawasaki disease.

    Kawasaki disease (KD) is an uncommon cause of sudden death in young adults in europe. Angiographically, the disease is characterized by coronary artery aneurysms which can be fully obstructed by acute thrombosis or by progression of the disease. If diagnosis of KD is made, immediate investigation should be made to determine whether ischemia is occurring and if so, to establish optimal time for revascularisation or cardiac transplantation. We describe an 18-year-old Caucasian male who was not previously known to have KD and who suffered from an acute myocardial infarction complicated by ventricular fibrillation, caused by acute thrombosis of a coronary artery aneurysm.

8/46. Aneurysmal and partially thrombosed orifice of a coronary artery fistula into the right atrium combined with patent foramen ovale.

    We report a case of a right atrial sessile tumor combined with a patent foramen ovale and the characteristics of fat containing tumor in magnetic resonance imaging. Histologic study revealed this to be an aneurysmal and partially thrombosed formation of a coronary artery fistula.

9/46. Two- and three-dimensional transesophageal echocardiographic diagnosis of intramyocardial dissecting hematoma after myocardial infarction.

    The echocardiographic findings of an intramyocardial dissecting hematoma that formed after an extensive acute myocardial infarction of the anterior wall of a 42-year- old man are described. Serial transesophageal studies were used to construct 3-dimensional images that clarified the participation of various myocardial layers that surrounded the dissecting hematoma. The patient was successfully treated with intra-aortic balloon counterpulsation and subsequently coronary artery bypass grafting. Intramyocardial dissecting hematoma is a rare complication of acute infarction; differential diagnosis must be made with pseudoaneurysm by establishing integrity of epicardium and with intracavitary thrombosis by identifying the endomyocardial layer surrounding the neoformation and associated wall movement.

10/46. Adjunctive therapies in the cath lab. Successful thrombolysis using the combination of tissue plasminogen activator and abciximab in an adult with Kawasaki's disease.

    Kawasaki's disease is an acute systemic vasculitic syndrome that primarily affects children. Coronary aneurysms are common vasculitic sequelae of Kawasaki's disease. Intracoronary thrombosis and embolization are potential consequences of coronary aneurysms. We describe our experience of successful thrombolysis using the combination of reduced-dose intravenous tissue plasminogen activator and abciximab as described in the Thrombolysis in Myocardial Infarction 14 trial (TIMI 14) to treat a patient found to have intracoronary thrombus at the site of aneurysm formation due to Kawasaki's disease.
