1/6. Giant gas filled cysts of the sigmoid colon: a report of two cases.

    Two case reports of giant gas filled cysts of the sigmoid colon are presented. It is considered that radiology provides the only useful and conclusive diagnostic investigation. In one of the cases, operative confirmation was obtained. In the other, the radiological appearances are considered to be diagnostic.

2/6. Ulcerative disease of the colon proximal to partially obstructive lesions: report of two cases and review of the literature.

    carcinoma complicating idiopathic ulcerative colitis is well known. Conversely, acute colitis complicating obstructing carcinomas and other partially obstructing lesions of the colon has not been recognized until recently. The present study reports two cases of colitis secondary to obstruction: 1) a giant ulcer with colitis proximal to partially obstruction diverticulitis of the sigmoid colon, and 2) colitis proximal to obstructing carcinoma of the sigmoid colon. The purpose of this report is to document these cases and review the literature on this variety of colitis to facilitate its recognition and subsequent correct treatment. An unawareness of this entity prejudices the anastomosis and results in anastomotic complications (approximately 25 per cent), with significant morbidity and mortality.

4/6. Giant diverticular of the sigmoid colon: a review of differential diagnosis and operative management.

    Giant sigmoid diverticula ("giant gas cysts," "giant sigmoid cysts") are an unusual complication of diverticulitis. We have recently seen two patients with such lesions, bringing the total number of reported cases to 48. Most patients are elderly and complain of abdominal pain or the presence of an abdominal mass. air trapping by a ball-valve mechanism may be responsible for formation of the fibrous cysts. The gas-filled structures can usually be differentiated from other conditions by contrast studies. Excision of the cyst and adjacent colon with anastomosis can usually be accomplished safely under elective circumstances.

5/6. Occult bleeding from giant small-bowel diverticula.

    Two cases of occult intestinal blood loss due to giant ileal diverticula are described. In one of them, ectopic gastric mucosa was found at microscopic dissection. In the other that was probably originating from a blind loop formation at a previous operation, multiple mucosal erosions, probably due to retained bowel contents, were considered responsible for the blood loss. Though giant diverticula are rare throughout the gastrointestinal tract, and usually cause symptoms other than bleeding, the importance of a careful search for this malformation in children with occult faecal blood loss is stressed.

6/6. Giant sigmoid diverticulitis. A cause of partial small bowel obstruction.

    Partial small bowel obstruction developed in a patient with associated giant sigmoid diverticulitis. This complication resulted from the wall of the diverticulum adhering to a loop of terminal ileum. Small bowel obstruction had not been observed in the 15 cases of giant sigmoid diverticula reported in the literature.
