1/62. Aneurysm of the pulmonary trunk with patent arterial duct.

    We present a 14-year-old female with a massive aneurysm of the pulmonary trunk occurring secondary to bacterial endocarditis of the patent arterial duct. Echo Doppler imaging, magnetic resonance and angiography confirmed the diagnosis. On intermediate follow-up after surgical correction, the echo studies and cardiac catheterization disclosed no residual lesions. The patient is now asymptomatic.

2/62. pulmonary artery aneurysm.

    Main pulmonary artery aneurysm is an exceedingly rare entity. We present a case of main pulmonary artery aneurysm with patent ductus arteriosus in a sixty-year-old woman. The aneurysm was successfully treated with aneurysmectomy and primary anastomosis of the defect of the main pulmonary artery, and the patent ductus arteriosus was divided. The etiology, operative indication and surgical intervention of main pulmonary artery aneurysm are discussed along with a review of the literature.

3/62. Subclinical aortic perforation with the infant double-button patent ductus arteriosus occluder.

    Modification of the double-button (Sideris) patent ductus arteriosus (PDA) occluder has resulted in a single-strut aortic component rather than the conventional cross-strut design. We report the use of this infant PDA occluder for transcatheter closure in three patients with PDA measuring 2 mm, 3.7 mm, and 4 mm. Subclinical aortic perforation with a small aortic aneurysm developed in two patients 1 year after occluder implantation. The third patient had developed a small aortic aneurysm without perforation at 3-month follow-up. All three patients had a residual shunt and underwent successful PDA surgical closure with aortic aneurysmal repair. Single-strut umbrella designs are not recommended for PDA transcatheter closure.

4/62. Spontaneous closure and thrombosis of a ductal aneurysm in a neonate.

    The natural history of a ductal aneurysm detected prenatally is presented. Neurologic concerns in the neonate and absence of cardio-respiratory compromise permitted serial echo demonstration of constriction, thickening and subsequent closure with formation of thrombus in the aneurysm.

5/62. Successful resection of ductus arteriosus aneurysm in infancy.

    We report a case of thrombosed patent ductus arteriosus aneurysm in an infant. The aneurysm was detected accidentally on chest roentgenogram and presented as globular soft tissue density mass in left posterosuperior mediastinum. Resection of the aneurysm was performed without cardiopulmonary bypass. Two years after operation the patient is well and growing normally.

6/62. Giant aneurysm following coil occlusion of patent ductus arteriosus.

    A case is described in which a giant aneurysm developed following successful PDA coil occlusion in an infant with marfan syndrome. This rare and severe complication brings into question the need for careful evaluation of these children before and after transcatheter occlusion.

7/62. Aortic stent-graft for patent ductus arteriosus in adults: the aortic exclusion technique.

    The risk of closure of a patent ductus arteriosus in the elderly is high because of the fragility of the aorta and aneurysmal change in the ductus. Stent-grafting has emerged as a method of treatment of pathology of the aorta. We describe a case where this new endovascular technique has been successfully applied for closure of a patent ductus arteriosus in a high-risk patient. It may become part of the armamentarium for this pathology in adults.

8/62. A spontaneous ductal aneurysm presenting with left recurrent laryngeal nerve palsy.

    A 76-year-old man was found to have a left recurrent laryngeal nerve palsy, due to a spontaneous patent ductus arteriosus aneurysm. Ortner's syndrome (left recurrent laryngeal nerve palsy caused by cardiovascular pathology) secondary to patent ductus arteriosus aneurysm has not previously been described in the literature. We present a case, illustrated with surface rendering computer tomogram reconstructions.

9/62. Aneurysmal dilatation of the ductus arteriosus in a neonate.

    Fetal ultrasound provides the capacity for early detection of a variety of congenital heart diseases. We report a case of aneurysmal dilatation of a patent ductus arteriosus detected in utero and subsequently confirmed by transthoracic echocardiography in the neonatal period. Prompt recognition of the ductus aneurysm with resultant surgical ligation may have averted potentially serious complications from this condition.

10/62. False aneurysm and mediastinal hematoma: complications of simultaneous transcatheter therapy for coarctation of the aorta and patent ductus arteriosus in an infant.

    We report an infant with intractable congestive heart failure caused by coarctation of the aorta, patent ductus arteriosus, atrial septal defect, and multiple muscular ventricular septal defects. He had received balloon angioplasty for dilating the aortic coarctation and a Gianturco coil for closing the ductus at a single therapeutic catheterization session at 4 months old. After balloon angioplasty, his heart failure had dramatically resolved. A false aneurysm and mediastinal hematoma were noted on the following day. The hematoma revealed total resolution 10 weeks later. He was asymptomatic throughout 12 months of follow-up.
