1/10. life-threatening digestive haemorrhage from duodenal recurrence of gastric cancer.

    Invasion of the duodenum by gastric carcinoma is not uncommon. The duodenal invasion by transpiloric infiltration through the submucosal layer or lymphatic spread frequently being microscopic and in minimal number of cases involving the mucosa, is generally asymptomatic and detected only in postmortem examinations. We report a case of life-threatening gastrointestinal bleeding from cancer recurrence at duodenal stump after subtotal gastrectomy for gastric carcinoma. In such cases it can be very hard to find the haemorrhagic source because of the difficulties encountered in endoscopic and radiological evaluation of the duodenal stump.

2/10. Gastrointestinal stromal tumour of the duodenum in a 7-year-old boy.

    We report a 7-year-old boy presenting with an acute upper gastrointestinal (GI) haemorrhage subsequently diagnosed to have a very rare duodenal gastrointestinal stromal tumour (GIST). endoscopy, pertechnetate and red cell scans were negative. Abdominal US detected a 17-mm mass lesion of the third part of the duodenum. This was confirmed on CT and shown to be hypervascular on selective angiography. At laparotomy, a 20-mm submucosal duodenal lesion was found associated with mucosal ulceration. Immunohistochemical analysis revealed it to be positive for CD117 (c-KIT protein) consistent with a GIST. We emphasize the importance of a thorough abdominal US examination in children with GI haemorrhage and the consideration of GIST in the diagnosis after the common causes have been excluded.

3/10. Emergency catheter probe endoscopic sonography (CP-EUS): an effective procedure in the assessment of a bleeding gangliocytic paraganglioma.

    Catheter probe endoscopic sonography is a relatively rapid and safe procedure, carried out during standard endoscopy, capable of distinguishing solid from cystic lesions and vascular from avascular masses. Herein we discuss the role of catheter probe endoscopic sonography in the emergency assessment of a patient with recent and severe bleeding from an ulcerated polyp, arising from the papilla. During the endoscopy, catheter probe endoscopic sonography showed the solid and submucosal nature of the lesion, suggesting its localised and benign nature and, most importantly, demonstrating the high risk of rebleeding from vascular structures communicating with the ulcer. Because of this finding three metallic endoclips were positioned. The lesion was not removed endoscopically because of its difficult position and the high risk of haemorrhage. The mass, removed through a duodenotomy, was shown to be a gangliocytic paraganglioma, an uncommon tumour, frequently resulting in surgery because of abdominal pain and gastrointestinal bleeding.

4/10. Large Brunner's gland adenoma: case report and literature review.

    Brunner's gland adenoma (BGA) is a very rare benign tumour of the duodenum, which is usually asymptomatic and discovered incidentally at endoscopy. Occasionally, this lesion may be large, causing upper gastrointestinal haemorrhage or intestinal obstruction. The case had a large Brunner's gland adenoma, presenting melena that was managed by endoscopic excision.

5/10. Haemangiomas of the gastrointestinal tract in children.

    Haemangiomas of the gastrointestinal tract are unusual vascular anomalies either in children or in adults. There is a significant difficulty in their diagnosis because of this rarity. However, when they are present, haemangiomas of the gastrointestinal tract may be a source of acute or chronic blood loss and anaemia. The case histories of two children with severe gastrointestinal bleeding due to cavernous haemangioma of the duodenum and caecum are presented in this report. Progressive haemorrhage from the gastrointestinal tract was the indication for operative intervention in both children. Each child underwent complete resections of the haemangiomas and is well and symptom-free one year after the surgical treatment. The purpose of this paper is to draw attention to this uncommon benign condition which has a relatively high mortality due to the delays in diagnosis and inadequate treatment.

    hamartoma of Brunner's gland is rare. They have previously been described as a cause of gastrointestinal haemorrhage and obstruction. This is the first report of a hamartoma causing pyloric obstruction and a biliary fistula.

7/10. leiomyoma of the duodenum presenting as a pyogenic liver abscess.

    leiomyoma of the duodenum is a rare condition which is most often diagnosed only as an incidental finding at post-mortem. It may present clinically with pain or haemorrhage. A case is described of degeneration of a leiomyoma of the fourth part of the duodenum giving rise to pyogenic liver abscess. There is no previous report of this association in the literature.

9/10. Case report: therapeutic transcatheter embolotherapy in the control of recurrent haemorrhage from lymphoma of the small bowel.

    lymphoma is one of the commonest primary malignancies of the small bowel. gastrointestinal tract haemorrhage is a relatively infrequent presenting feature or complication. We describe the use of repeated transcatheter embolization as the primary treatment in the control of recurrent haemorrhage from the small bowel in a patient with B-cell lymphoma. Few cases of transcatheter embolotherapy have been described in such situations. This form of treatment is a therapeutic option in those unfit for transcatheter vasoconstrictor infusion or surgery.

10/10. Duodenal haemorrhage resulting from renal cell carcinoma metastases.

    A case of duodenal haemorrhage resulting from renal cell carcinoma metastases is presented. This is an uncommon method of presentation and is often difficult to diagnose requiring multiple imaging regimes. This case demonstrates the problems that may be encountered in identifying the source of bleeding.
