1/4. Secondary epispadias repair with rectus abdominis musculocutaneous flap.

    Genital defects in patients with epispadias cause functional, aesthetic, and psychological problems. Successful reconstruction of epispadias should improve the body image and enable the patient to perform sexually. Although various operative techniques have been defined for reconstruction of epispadias--predominantly using local tissue flaps--classic reconstructive methods are sometimes not sufficient in severe cases. Secondary reconstruction is inevitable after failed reconstructive attempts with classic techniques. The authors used the rectus abdominis musculocutaneous flap for severe secondary epispadias repair. The urethra, dorsal chordee, osteo-escutcheon contour defect, and the tissue defect created after dissection of the dorsal surface of the penis were reconstructed successfully using the rectus abdominis musculocutaneous flap. The neourethra was constructed using the skin island of the flap, and the muscle segment was used to reconstruct the dorsal defects. A straightened and lengthened penis and a new urethra without stricture were created during one operative procedure using this method.

2/4. Use of a vertical island flap for correction of epispadias.

    An 18-year-old man presented with epispadias. His main complaints were dorsal curvature of the penis and difficulties during erection and micturition. Primary urethroplasty at another hospital had been only minimally successful. The public bones were normal and he was continent. He was treated by complete excision of the chordee and urethroplasty. Local flaps were used to reconstruct the glandular urethra, and a vertical penile island flap was used to replace the missing portion of the penile urethra. Good straightening of the penis and a new urethra with a well-closed meatus at the tip of the glans were achieved in a single-stage operation. No complication occurred, and the result was satisfactory from both the aesthetic and functional points of view.

3/4. Ventral preputial island flap technique for the repair of epispadias with or without exstrophy.

    The preputial island flap technique introduced by Duckett for repair of severe hypospadias has met with much clinical popularity. We report the use of a modification of this technique for repair of epispadias in 3 boys with exstrophy of the bladder and in 1 with penile epispadias. The results have been excellent in 3 cases and good in 1. Minimum followup has been 18 months.

4/4. Ventral preputial onlay island flap and proximal flip-flap technique for the repair of epispadias: a case report.

    Use of the ventral preputial island flap technique for repair of epispadias has yielded satisfying functional and cosmetic results. We report a modification of this technique in a patient with proximal penile epispadias without exstrophy. We used the Duckett modification of the onlay island flap using ventral preputial skin and coupled it with a proximal flip-flap of perimeatal skin without division of the urethral plate. This achieved an excellent result.
