1/3. obesity as a cause of "false-positive" alcohol misuse laboratory investigations.

    Three patients are reported with a prior history of alcohol misuse accompanied by abnormal liver toxicity tests and other indices of alcohol misuse. A decreased but persistently raised serum gamma-glutamyl transferase activity during subsequent abstinence or controlled drinking was interpreted incorrectly as due to continued alcohol misuse whereas obesity-related fatty liver was the probable cause. The value of serum carbohydrate deficient transferrin assays in the differential diagnosis of abnormal liver toxicity tests is emphasized.

2/3. Raised hepatic free fatty acids in a patient with acute fatty liver after gastric surgery for morbid obesity.

    A patient died after gastric surgery for morbid obesity. Necropsy showed severe fatty liver, and biochemical analysis of hepatic lipids showed unusually high free fatty acid concentrations, which may have contributed to the hepatic failure.

3/3. Nonalcoholic steatohepatitis in obesity: a reversible condition.

    Nonalcoholic steatohepatitis is a rare complication of obesity with laboratory and histological features indistinguishable from alcoholic hepatitis. Three patients with 50-60% overweight and steatohepatitis are reported. All responded with normalization of the biochemical and/or histological changes after modest weight reduction.
