1/11. Radiographic features of eosinophilic gastroenteritis (allergic gastroenteropathy) of childhood.

    The child with eosinophilic gastroenteritis has failure to thrive, a history of allergy or asthma, iron deficiency anemia, and peripheral eosinophilia. This type of eosinophilic gastroenteritis has earned the designation "allergic gastroenteropathy." Pathologic change in the gastric antrum of six children with this disease correlated with an abnormal air-contrast examination of the antrum. The affected children had an irregular, lacy, antral surface rather than the smooth, bald surface seen in normal children. The small bowel in allergic gastroenteropathy is usually abnormal but nonspecific as to etiology. Much more specific is the combination of small bowel and gastric antral abnormalities. Findings encourage air-contrast radiography of the antrum in children with suspected allergic gastroenteropathy.

2/11. linear iga bullous dermatosis responsive to a gluten-free diet.

    dermatitis herpetiformis is associated with a gluten-sensitive enteropathy in >85% of cases. Both the skin lesions and the enteropathy respond to gluten restriction. linear iga bullous dermatosis has a much lower prevalence of histological small bowel abnormalities, and lesions are not known to respond to gluten restriction. We report a patient with linear iga bullous dermatosis and gluten-sensitive enteropathy. This report addresses the issue of whether linear iga bullous dermatosis can be associated with gluten-sensitive enteropathy. We evaluated the response to gluten restriction and normal diet by following the status of the patient's jejunal biopsies and skin lesions. The patient responded to gluten restriction, as shown by resolution of jejunal abnormalities and skin lesions and subsequently by recurrence of jejunal abnormalities and skin lesions with reinstitution of a gluten-containing diet. This report demonstrates that linear iga bullous dermatosis can respond to gluten restriction if an underlying gluten-sensitive enteropathy is present.

3/11. food allergy and IgE sensitization caused by spices: CICBAA data (based on 589 cases of food allergy).

    BACKGROUND: spices originate in various botanical families: apiaceae, lamiaceae, lauraceae, Leguminosae, liliaceae, myristicaceae, myrtaceae, piperaceae, solanaceae, zingiberaceae.... METHODOLOGY: Prick-tests to native spices have been carried out in patients suspected of food allergies to spices. The CICBAA data bank includes 589 cases of food allergies, a part of which has benefited from investigations for spices. Data about the rate of sensitization and food allergy are available. RESULTS: Frequent sensitization to apiaceae is observed: coriander, caraway, fennel, celery: 32% of prick-tests in children, 23% of prick-tests in adults. Sensitization to liliaceae: garlic, onion, chive, is observed in 4.6% of prick-tests in children, 7.7% of prick-tests in adults. Rare cases of sensitization to paprika and saffron are recorded. Prick-tests to nutmeg, ginger and clove are currently negative. 10 food allergies related to the mugwort-celery-spices syndrome are reported: coriander: 1, caraway: 2, fennel: 3, garlic: 3, onion: 1. food allergy to spices is unfrequent: 2% of the totality of food allergies. However, only adults are allergic to spices and allergy to spices accounts for 6.4% of food allergies in adults. Tiny amount of proteins are usually ingested. patients at risk of spice allergy are young adults sensitized to mugwort and birch allergens, sharing cross-sensitization with various food vegetal allergens. The clinical suspicion raises from frequent post-prandial systemic reactions. Other allergens of vegetal origin have to be cleared. diagnosis can be established by DBPCFC using powdered spices in capsules.

4/11. Oral disodium cromoglycate and ketotifen for a patient with eosinophilic gastroenteritis, food allergy and protein-losing enteropathy.

    We present a case report of a 10 years old boy with protein-losing enteropathy and eosinophilic gastroenteritis who had positive histamine release tests, increased allergen-specific IgE antibodies to some food items, and low levels of total serum protein and albumin. Upper gastrointestinal endoscopy revealed a number of polyps and diffuse gastritis. biopsy specimens of the stomach and duodenum showed widespread eosinophilia and neutrophilia. Although a restricted diet was recommended, a diet which excluded foods with positive results to both histamine release test and allergen-specific IgE antibodies was poorly tolerated, and the patient rejected systemic administration of corticosteroids. Thus, we initiated an oral disodium cromoglycate (DSCG) and ketotifen therapy. After oral DSCG and ketotifen administration, the patient's condition improved gradually. Therefore, oral DSCG and ketotifen therapy might be considered as treatment option in patients with eosinophilic gastroenteritis and protein-losing enteropathy caused by food allergy.

5/11. Case report of an allergic reaction to parenteral nutrition in a pediatric patient.

    We report a 34-month-old girl with stage IV neuroblastoma who developed hives when parenteral nutrition (PN) containing amino acids, dextrose, electrolytes, minerals, vitamins, and trace elements was infused. Administration of diphenhydramine resulted in disappearance of the rash. Infusion of the PN solution without intravenous fat emulsion produced a similar rash with itching. The pediatric multiple vitamin (PMV) preparation was removed from the PN formula and the formula was infused without incident. The patient was maintained on PN and an oral vitamin supplement with no further complaints. Inadvertent administration of a PN solution containing PMV resulted in a recurrence of hives. Absence of any adverse reactions when the PMV preparation was removed from the PN solution and an allergic reaction when the multivitamin was added to the PN solution support the possibility that the allergic reaction was related to the infusion of the multiple vitamin preparation.

6/11. milk-sensitive eosinophilic gastroenteritis in a 10-day-old boy.

    Eosinophilic gastroenteropathy in children is a rare, idiopathic disease characterized by eosinophilic infiltration of the gastrointestinal tract. We describe a 10-day-old boy with aspecific clinical manifestations (unconsolable crying). Treatment with a semi-elemental diet was successful, suggesting a "milk-sensitive enteropathy", although he had been exclusively breast fed. Recent literature is reviewed.

7/11. Protein-losing enteropathy in atopic dermatitis.

    We evaluated a child with atopic dermatitis associated with protein-losing enteropathy, which responded to a chicken-based diet. Allergic gastroenteropathy is a potentially hazardous disease that may be more frequent in children with atopic dermatitis than is currently realized.

8/11. thrombocytopenia in allergic protein-losing gastroenteropathy. Report of an infant with Down's syndrome.

    An eleven week old boy with Down's syndrome displayed the typical picture of protein-losing enteropathy, which was complicated by thrombocytopenia. The production of thrombocytes in the bone marrow specimen had increased. After withdrawal of cow's milk form the diet, the concentration of serum albumin and the number of blood thrombocytes normalised within two weeks. We take the view that hypercoagulability, due to the excessive loss of inhibitors of the coagulation system into the intestinal lumen, is the possible mechanism in the excessive consumption of the thrombocytes.

9/11. D-2-hydroxyglutaric aciduria: case report and biochemical studies.

    A patient with protein-losing gastroenteropathy and egg allergy has been shown to have a previously unrecognized organic aciduria, D-2-hydroxyglutaric aciduria. The observations made are consistent with an inherited metabolic disorder in the catabolism of 5-aminolaevulinate possibly due to deficient activity of a specific D-2-hydroxyglutarate dehydrogenase.

10/11. Enteropathy related to fish, rice, and chicken.

    Gastrointestinal symptoms in relation to the ingestion of proteins are common but only in the case of sensitisation to cows' milk protein, soy, or gluten have alterations in the function and structure of the small-intestine been reported. We describe 3 children with cows' milk protein intolerance and associated enteropathy related to fish, rice, and chicken, respectively. Repeated intestinal biopsies before and after an acute challenge with the specific food showed changes in the histological appearance of the intestinal mucosa identical with those observed after the acute administration of cows' milk. These findings support the conclusion that cows' milk-protein intolerance is not an isolated entity; it represents the first part of a broader intolerance of various dietary proteins.
