1/37. Plantar fat pad atrophy after corticosteroid injection for an interdigital neuroma: a case report.

A case of bilateral interdigital (Morton's) neuroma treated with steroid injection therapy developed plantar fat pad atrophy, shown on magnetic resonance imaging. Some pathologic changes at the site of injection (such as subcutaneous fat atrophy, depigmentation of the skin, and telangiectasias) are well known disadvantages of local steroid injection for the treatment of the Morton's neuroma. Scientific literature reports these problems (mainly as an aesthetic problem) in the dorsal aspect of the foot. In this work, the authors describe a case in which the steroid injection therapy has caused some changes in the plantar aspect of the feet, with serious functional problems. Fat pad atrophy is a serious problem in the foot and can cause a painful metatarsal syndrome with some important effects on the gait.

2/37. Plexiform schwannoma of the foot.

The present report describes a plexiform schwannoma involving the subcutis of the foot in an 8-year-old boy. Gross findings revealed thin fibrous septa in a multilobulated tumor that was partly separated into free body-like nodules in the subcutis. Preoperative CT and MRI failed to delineate this multinodular architecture or free bodies. This is a case presentation including the CT and MR findings associated with plexiform schwannoma.

3/37. Giant foot schwannoma.

Schwannoma of the foot is rare; only 12 cases have been reported. A schwannoma is a benign neurogenic tumour derived from schwann cells. The diagnosis is often delayed because the symptoms are mainly those of compression disorders. We describe a 7cm schwannoma of the heel in a 30-year-old man. Ten years earlier a schwannoma was removed from the same site. The recurrent lesion was widely excised and a medial plantar flap was used to repair the heel.

4/37. Cutaneous schwannoma of the foot.

Schwannomas usually present as solitary subcutaneous tumors adherent to a peripheral nerve. A solitary cutaneous schwannoma presenting as a solitary cutaneous nodule on the foot of a 19-year-old male is described. This is an unusual presentation of schwannoma. Saucerized excision produced an excellent result with no adverse effect on function or cutaneous sensation.

5/37. A Pacinian hyperplasia of the foot.

A case of Pacinian hyperplasia of the right great toe is reported. Pacinian hyperplasia is a rare benign and recently recognized painful lesion composed of an excess of well formed or hyperplastic Pacinian corpuscules, normaly involved in sensory innervation. This lesion that is usually observed in the hand, must be distinguished from nerve tumors harboring onion-bulb structure which are not true well formed Pacinian corpuscules and from Morton neuroma. Pacinian hyperplasia is considered a reactive lesion and not a true neoplasm. To our knowledge, this case is the first described in the foot.

6/37. Malignant schwannoma of the planta.

Schwannomas (neurilemmomas) are common tumors originating in the sheaths of peripheral nerves. The tumor may have the appearance of a reddish-brown or pale papule that is painful or itching. The surface is usually smooth but may be ulcerated or verrucous. Malignant reversal is rare. The authors present the case of a twenty-year-old patient with a malignant schwannoma of the plantar portion of the heel. They describe the surgical approach, the results of histopathological examination and subsequent follow-up.

7/37. Multiple schwannomas of the foot. Case report and strategy for treatment.

Determining the appropriate treatment of a benign tumor of a peripheral nerve in the foot and ankle region presents a clinical dilemma, as resection of the tumor will cause loss of nerve function and create the possibility of a painful neuroma. Several surgical solutions to this problem were used in the care of a patient who presented with painful bilateral Morton's neuromas and was found to have bilateral schwannomas on pathologic examination of the resected nerves. Subsequent evaluation for recurrent bilateral foot pain demonstrated multiple tumors along the tibial nerve in one foot. The patient also became aware of a painful mass on the dorsolateral aspect of one foot. review of the treatment options for this patient with multiple schwannomas provides a framework for decision making in the care of the patient with benign neural tumors of the foot.

8/37. Treatment of Morton's neuroma with the carbon dioxide laser.

Morton's neuroma is one of the most common causes of nerve pain in the foot. This article describes the treatment of this condition with carbon dioxide laser surgery, presenting the disadvantages and advantages of this method. This method has proven to be effective; postoperative pain and healing time are decreased and patients are able to resume normal ambulation faster than with conventional scalpel surgery.

9/37. Clear cell sarcoma presenting as an interdigital neuroma.

The incidence of malignant soft-tissue sarcomas in the general population is approximately 1.4 per 100,000. Approximately 2% of all cancer deaths are attributable to these tumors. Presented is a case history of a soft-tissue malignant neoplasm that was originally thought to be an interdigital neuroma and that eventually required a modified Chopart's amputation. A review of the literature is presented on other soft-tissue malignant tumors that have an affinity for the foot and ankle. The intention of presenting this case is to caution physicians that malignant lesions can simulate a benign process and should always be considered in the differential diagnosis of any foot mass.

10/37. Unusually large cellular schwannoma of the foot.

This article presents a case of an unusually large cellular schwannoma of the foot. This uncommon lesion of the lower extremity is described with regard to clinical appearance, radiologic evaluation (including magnetic resonance imaging), histologic characteristics, and intraoperative findings.
