1/6. Pre-operative imaging can diagnose torsion of the gallbladder: report of a case.

Torsion of the gallbladder is a rare disease and pre-operative diagnosis of the disease is uncommon. About 400 cases have been reported, but only 4 were diagnosed by pre-operative imaging. We report on a case of gallbladder volvulus diagnosed pre-operatively using pre-operative imaging with ultrasound and computed tomography.

2/6. Three cases of malacoplakia of the gallbladder.

malacoplakia is a granulomatous disease with a histiocytic infiltrate containing calcified bodies called Michaelis-Gutmann bodies considered to represent an abnormal response to infection involving defective lysosomes and abnormal microtubular assembly. The disease most frequently involves urinary and genital tracts, but has also been described from most organs. Reports from the gallbladder are extremely rare and as it might simulate specific infection, parasitic infestation as well as malignancy it is of importance for the surgeon and pathologist to be aware of the entity. In this article we present three cases of malacoplakia of the gallbladder, a rare disease in this location.

3/6. Gallbladder tuberculosis: false-positive PET diagnosis of gallbladder cancer.

Gallbladder tuberculosis (GT) is an extremely rare disease, and very few cases have been reported in the literature. The first case of GT was described in 1870 by Gaucher. A correct preoperative diagnosis of GT is unusual, and it is frequently confused with various gallbladder diseases. We present a new case of a patient who underwent surgery with the preoperative diagnosis of gallbladder cancer after a false positive positron emission tomography scan in the diagnostic work-up.

4/6. Gallbladder volvulus: a case report.

Gallbladder volvulus, a rare disease generally affecting older aged women, is generally due to an abnormal anatomical fixation of the gallbladder. As clinical and radiological signs are poor the diagnosis is generally made at laparotomy. The prognosis is excellent when early surgical treatment is performed.

5/6. Calcified ballbladder (porcelain gallbladder).

Calcification of the gallbladder wall, otherwise known as porcelain gallbladder, is a relatively rare disease and is frequently asymptomatic. Symptoms suggestive of biliary disease are often absent in patients with this manifestation. Since the condition is uncommon, it is important to recognize the clinical as well as the roentgenographic characteristics of the disease because of the high frequency (22%) of adenocarcinoma in the porcelain gallbladder. To our knowledge, no pathognomic sign of symptoms of the disease have been reported so far. Of five cases of porcelain gallbladder reported, carcinoma of the gallbladder was discovered in one patient. This disease seems to appear mostly in the sixth decade of life and is more prevalent in women. Diagnosis is made in most cases by a simple flat plate of the abdomen, and workup should include other roentgenographic examinations to exclude calcification of other viscera or the thoracic wall. Surgery should not be delayed even if the patient is asymptomatic since the occurence of carcinoma in porcelain gallbladder is remarkably high.

6/6. Gallbladder volvulus: a case report. Could the ultrasound be the key of the early diagnosis?

Gallbladder volvulus is a rare disease which may affect the elderly. It mimics clinically an acute cholecystitis, but the ultrasound signs may provide an early diagnosis: a very large, anteriorly floating gallbladder, with very thickened, multi layered wall, could orientate the physician to this rare disease. We present one case of gallbladder volvulus with its radiological signs. The symptoms, the etiology and the radiological examinations are discussed.
