1/20. Gangrenous sigmoid volvulus in a pregnant woman.

    A rare case of gangrenous sigmoid volvulus in a pregnant woman causing intestinal obstruction is reported. The patient had intrauterine foetal death. Laparatomy for resection of sigmoid colon and hysterotomy for removal of dead foetus was carried out. Terminal iliac colostomy with closure of rectal stump was done in the first surgery. The patient underwent colorectal anastomosis 2 months after the first operation and recovered uneventfully.

2/20. Volvulus of the splenic flexure of colon: a case report and review.

    We report a case of splenic flexure volvulus in a patient who had previously been operated upon for a sigmoid volvulus. The clinical features and management of splenic flexure volvulus are discussed.

3/20. Volvulus and gangrene in intra-abdominal colon after colonoplasty for esophageal stricture.

    Ten years after colonoplasty using the left colon for impassable corrosive stricture of the esophagus, a 27-year-old woman developed volvulus and gangrene of the remaining intra-abdominal colon. It was resected and colostomy was done. In the second stage, after 3 months, the ascending colon and cecum were mobilized and anastomosed to the rectal stump. Eight years later, the patient is asymptomatic.

4/20. Midgut volvulus following cesarean section.

    Total midgut volvulus and gangrene due to a congenitally elongated mesentery in a 27-year-old primipara occurred 3 days after cesarean section. All of the small bowel distal to the duodenum except for 12 inches of jejunum was resected, and the patient survived. This case and the literature concerning volvulus in pregnancy are reviewed here, and suggestions for prevention and treatment are made.

5/20. Massive gastrointestinal bleeding in infants with ascariasis.

    Although infestation with ascaris lumbricoides causes various intestinal complications, massive gastrointestinal bleeding is rare. The authors report on 2 infants who presented with massive hematemesis. In the first patient, a gastroduodenoscopy was performed. Roundworms were found adhering to an oozing duodenal ulcer. Duodenal perforation occurred after extraction of the parasites. The second case was hematemesis together with peritonitis. Exploration found volvulus and a gangrenous segment of the distal ileum. Both infants previously passed roundworms with their stool. The authors conclude that the intestinal ascariasis can be a cause of massive gastrointestinal bleeding, especially in temperate and tropical countries.

6/20. Volvulus of the small intestine caused by right paraduodenal hernia: a case report.

    This report describes a very rare case of right paraduodenal hernia presenting as volvulus of nonherniated small intestine. A 12-year-old boy presented with sudden onset of lower abdominal pain, and emergency laparotomy was performed on a diagnosis of small intestinal obstruction. laparotomy confirmed right paraduodenal hernia and volvulus of the small intestine out of the hernia sac.

7/20. Small bowel volvulus leading to gangrene and short bowel syndrome.

    A young man was operated for acute abdomen. laparotomy revealed small bowel diverticulosis with very short mesentery leading to volvulus of near total small bowel. Resection and end to end anastomosis was performed. Patient ended up with short bowel syndrome.

8/20. Ileosigmoid knotting: a case report and review.

    Ileosigmoid knotting, or compound volvulus, has not been reported previously in australia. A 41 year old woman of Anglo-Saxon origin presented with a short history of extreme abdominal pain that was out of proportion to her physical findings. At operation it was found that her entire ileum was tightly knotted around the redundant, twisted sigmoid colon. Both closed loops were gangrenous and it was impossible to unravel the bowel. She recovered uneventfully following resection and primary anastomosis of both portions of the intestine. Most patients with this uncommon condition have been reported from finland and eastern africa. An arrangement of the small bowel and sigmoid colon on long, narrow mesenteries would appear to be a prerequisite. The brevity of the history and the severity of the abdominal pain call for early laparotomy. Prolonged attempts to untie the knot are dangerous. It is safer to divide the ileum at the knot and resect it in order to release the sigmoid colon. Primary anastomosis is feasible where the history is short and the uninvolved intestine is clean and collapsed.

9/20. Mesenteric cystic lymphangioma: unusual cause of intra-abdominal catastrophe in an adult.

    Mesenteric cystic lymphangiomas (MCLs) are rare benign cystic tumours of unknown aetiology, most often seen in paediatric patients. The clinical presentation is diverse, ranging from an incidentally discovered abdominal cyst to symptoms of acute abdomen. A 20-year-old male presented with generalised abdominal pain, nausea and vomiting of several hours duration following heavy lifting. Emergency laparotomy revealed a 15 x 10 x 8-cm pedicled cystic mass of the mid-ileal mesentery, causing a volvulus. The cyst and a 20-cm gangrenous intestinal segment were resected with anastomosis. The postoperative course was uncomplicated. MCLs should be included in the differential diagnosis of cystic intra-abdominal lesions. Even when asymptomatic and discovered incidentally, they must be treated surgically because of the potential to grow, invade vital structures and develop life-threatening complications.

10/20. Ileal resection for gangrenous ileal volvulus in a term pregnancy: a case report.

    Mechanical bowel obstruction is a rare complication of pregnancy, usually seen in small bowel, and most commonly due to an adhesive band or hernia. Surgical evaluation is the method of treatment in cases that do not answer to conservative support procedures. A very rare case of small-bowel strangulation caused by herniation of a loop of ileum through another ileoperitoneal adhesion is presented.
