1/3. Late presentation of a gastrocolic fistula after percutaneous fluoroscopic gastrostomy.

    Gastrocolic fistula (GCF) has previously been described with percutaneous endoscopic gastrostomy procedures. However the present case of GCF is to our knowledge the first reported in the literature. It developed as a complication of the fluoroscopic technique three months after percutaneous fluoroscopicically guided gastrostomy-tube insertion. GCF was symptomless and was discovered during button replacement in a 74-year-old head and neck cancer patient. In patients with complex upper abdominal anatomy, the use of US or CT should be recommended to avoid such problem.

2/3. Gastrobronchial fistula in untreated lymphoma.

    A 48-year-old man with B-cell lymphoma of the stomach was seen with recurrent pneumonia, weight loss, and anorexia. A barium study revealed a large malignant gastric ulcer with a gastrobronchial fistula. A computed tomography (CT) scan of the abdomen showed a gastric neoplasm invading the left lower lobe of the lung and the spleen.

3/3. Benign ulcer in a hiatus hernia complicated with a pericardial fistula.

    Penetration of the pericardium by upper gastrointestinal lesions is uncommon. It usually results from trauma, perforation of foreign objects, perforation from gastric or thoracic neoplasm or spontaneous perforation. Unfrequently it is a complication of a benign peptic ulcer of the esophagus or the stomach. survival from peptic ulcers penetrating to the heart is very rare. From a review of the English literature only 11 survivors have been described. We report the successful outcome of a 76 year old lady with a benign ulcus in a hiatus hernia perforating into the pericardium. diagnosis of the fistula was established after methylene blue swallow. Treatment consisted of early pericardiotomy and drainage followed by surgical correction of the ulcer and fundoplication of the hiatus hernia.
