1/12. myositis associated with a Baerveldt glaucoma implant.

    PURPOSE: To describe a case of myositis in the presence of a Baerveldt glaucoma implant. METHOD: Case report. RESULTS: A 41-year-old black woman developed myositis after placement of a Baerveldt glaucoma implant. Echography demonstrated migration of the seton plate against the medial rectus muscle insertion. myositis resolved after removal of the Baerveldt glaucoma implant. CONCLUSION: The Baerveldt glaucoma implant may have precipitated myositis in this patient.

2/12. 'Black cornea' after long-term epinephrine use.

    Fifteen years after a partial maxillectomy and radiation therapy for left antral carcinoma, a 53-year-old woman presented to the eye plastics and orbit Service of the massachusetts eye and ear Infirmary, boston, with phthisis and a large, black corneal lesion in the left eye. She had been treated for unilateral glaucoma in the left eye for more than 10 years with topically administered epinephrine borate, timolol maleate, and pilocarpine hydrochloride. Clinically, the lesion was smooth, black, and homogeneous, and was thought to represent uveal prolapse covered by a thin layer of epithelium. An eyelid-sparing anterior exenteration was performed. Histopathologic examination revealed an acellular, homogeneous substance that stained positively with the Fontana Masson stain for melanin and bleached with potassium permanganate, findings consistent with corneal adrenochrome deposition. Since adrenochrome can be easily dissected free from the cornea, this case illustrates that misdiagnosing adrenochrome deposition may lead to unnecessary surgery.

3/12. glaucoma and Peters' anomaly. A clinicopathologic case report.

    A 26-year-old black woman with Peters' anomaly who had been followed since early childhood developed an open-angle form of glaucoma, which required filtering surgery. light and electron microscope studies of the trabeculectomy specimen, which was obtained by en bloc resection of the trabecular meshwork and iridectomy specimen, revealed that the structures of the anterior-chamber angle were grossly normal aside from evidence of premature aging on the basis of abundant broad-banded collagen fibers in the trabecular lamellae.

4/12. nevus of ota with glaucoma: report of three cases.

    We report three cases of unilateral open-angle glaucoma associated with ipsilateral nevus of ota. All three occurred in black patients and were associated with ipsilateral melanocytic infiltration of the outflow tracts. glaucoma probably is associated with nevus of ota in blacks more frequently than has been reported. All patients with nevus of ota should be examined regularly to screen for glaucoma.

5/12. Transconjunctival Nd:YAG laser revision of failing trabeculectomy.

    We used a Q-switched Nd:YAG laser delivered transconjunctivally to restore filtration to a failing trabeculectomy of a 51-year-old black woman with primary open-angle glaucoma and uncontrolled intraocular pressure (IOP) on maximally tolerated anti-glaucoma medical therapy. Initially, the Nd:YAG laser was directed via a goniolens at the interface of the trabeculectomy flap and scleral bed. This attempt to restore filtration was unsuccessful. In two separate treatment sessions the Nd:YAG laser was directed transconjunctivally onto the visible trabeculectomy flap margins, followed by focusing the laser onto the gray-white episcleral fibrous tissue overlying the scleral flap. During the last transconjunctival treatment, the bleb elevated immediately and the IOP decreased from 30 mm Hg to 18 mm Hg. These effects have been sustained over 3 months of follow-up.

6/12. Temporary visual loss with ciliary body detachment and hypotony after attempted YAG laser repair of failed filtering surgery.

    ciliary body detachment, hypotony, and visual loss developed within one month after an attempt to reopen a closed filtering bleb with YAG laser treatment. The patient was a 68-year-old black woman who had a previously functioning thermal sclerostomy. YAG laser energy delivered to the sclerosed filtering site ab interno was unsuccessful in restoring patency to the filtering site as judged by clinical examination. Because of hypotony, all glaucoma medications were discontinued. Resolution of the ciliary body detachment with return of vision and stable intraocular pressures occurred after three weeks of intensive topical and systemic corticosteroid treatment.

7/12. Pigmentary open-angle glaucoma secondary to posterior chamber intraocular lens implantation and erosion of the iris pigment epithelium.

    This report describes two patients who developed a secondary form of pigmentary open-angle glaucoma after implantation of a posterior chamber intraocular lens. The glaucoma was associated with considerable erosion of the iris pigment epithelium and a charcoal-black pigmentation of the trabecular meshwork. One case responded to argon laser trabeculoplasty and the other case did not. Possible causes of this syndrome are discussed.

8/12. Open-angle glaucoma associated with posterior polymorphous dystrophy. A clinicopathologic study.

    Clinical and histopathologic features of a 62-year-old black man with posterior polymorphous dystrophy and open-angle glaucoma requiring trabeculectomy are reported. The surgical procedure was modified to allow en bloc resection of the deep limbal tissue and peripheral iris. light and electron microscopy of the specimen revealed a high insertion of the iris into the posterior portion of the trabecular meshwork with compression of the intertrabecular spaces. These findings are similar to changes that have been reported in primary congenital glaucoma and suggest a developmental anomaly of the anterior chamber angle in this form of glaucoma associated with posterior polymorphous dystrophy.

9/12. Pigmentary glaucoma in a black albino.

    Pigmentary glaucoma has been only rarely reported in the black population. There have been no reports of this disorder in an albino. We report a case of pigmentary glaucoma in a 55-year-old black albino. A dramatic increase in iris transillumination was observed after institution of topical epinephrine. A possible explanation for this progressive iris transillumination is discussed.

10/12. Major early complications of laser trabeculoplasty.

    A complication frequency of 3.1% visual loss and 1.8% emergency glaucoma surgery occurred in 224 and 159 eyes respectively after laser trabeculoplasty. The latter was associated with blacks and advanced cases of glaucoma. Both were highly associated with a 24-hour rise in pressure after laser.
