1/8. Osteoid osteoma of the metacarpal bone presenting after an injury.

    We present a case of osteoid osteoma of the right third metacarpal bone in a 23-year-old man. He had an apparent history of initial injury, followed by pain and swelling. He was initially treated for post-traumatic periostosteitis, so that the diagnosis of osteoid osteoma was delayed. Computed tomography and histology confirmed the latter diagnosis. En bloc resection of the nidus was followed by complete resolution of his symptoms. This case report emphasises the difficulties in diagnosis, particularly after an injury, and illustrates its effective treatment.

2/8. Dislocation of the pisiform bone after severe crush injury to the hand.

    The pisiform bone dislocated in a 56-year-old worker who had a crush injury of his wrist. Open reduction and reconstruction of the ligaments resulted in proximal subluxation of the pisiform bone and post-traumatic arthritic changes. Other authors recommend that excision of the pisiform is considered to be a more appropriate method of treatment.

3/8. Intravascular papillary endothelial hyperplasia in the hand of a fencer.

    A case of intravascular papillary endothelial hyperplasia in the hypothenar eminence of a 23-year-old female fencer is described. Differential diagnosis with other similar benign and malignant lesions was made using histopathology and immunohistochemistry. The possible post-traumatic proliferative histogenesis is discussed. The non-malignant nature of the mass was confirmed after a three year follow-up.

4/8. Successful transfer of two reverse forearm flaps despite disruption of both palmar arches.

    The advantages of the ipsilateral forearm flap in emergency post-traumatic hand reconstruction are now well-established. A prerequisite for such a transfer has always been the presence of at least one intact palmar arch. We report the successful transfer of two reverse forearm flaps despite disruption of both palmar arches. In both cases the flap was raised on the vascular axis which had ceased to contribute significantly to the hand circulation. The anatomical basis of this transfer is discussed.

5/8. Flexor digitorum profundus common to thumb and index finger, associated with a post-traumatic distal adherence of both tendons.

    The existence of connections, in the form of tendon slips, between the tendons of the flexor pollicis longus and the flexor digitorum profundus indicis has been described by different authors as being a relatively frequent anomaly. Complete fusion between the two muscles, however, must be considered a very rare anomaly. A bilateral case is described, in which the right hand also had post-traumatic adhesions between the two tendons.

6/8. Infections due to actinobacillus actinomycetemcomitans. A report of 3 cases.

    Three cases are described which show the typical range of infections which may be caused by actinobacillus actinomycetemcomitans, namely an actinomycosis-like infection, a prosthetic cardiac valve endocarditis and a post-traumatic soft-tissue infection. Cultural studies are detailed, and a short review of the disease-producing potential of this organism and its treatment is presented.

7/8. Post-traumatic triggering of extensor pollicis longus at the dorsal radial tubercle.

    A case of post-traumatic triggering of the tendon of extensor pollicis longus around Lister's tubercle is described. This condition was successfully treated by surgical release.

8/8. True aneurysms of the palmar arch of the hand: a report of two cases.

    We report on two cases of true aneurysms of the palmar arch, both of which were post-traumatic. One was due to repetitive blunt trauma and the other to a glass cut wound. Both aneurysms were detected clinically as pulsating masses without functional disorders. The presence of elastic fibres and smooth muscle in the aneurysm wall proved it to be a true aneurysm. Both aneurysms were excised and replaced by a vein graft from the foot.
