1/90. Echo-enhancing sonography of a large-vessel hemangioma of the neck.

    Because of the slow flow in the venous spaces of large-vessel hemangiomas, demonstration of color flow signals with conventional color Doppler or power Doppler sonography may be difficult. We report the case of a 22-year-old female patient with a soft tissue tumor containing multiple fluid-filled spaces in the right supraclavicular region. Gray-scale, color, and power Doppler sonography could not differentiate between cystic lymphangioma and large-vessel hemangioma. The intravenous echo-enhancing contrast agent Levovist was administered, and a significant echo-enhancing effect on color and power Doppler imaging was demonstrated in the fluid-filled spaces and lasted for about 3 minutes. Histopathologic study of the excised tumor confirmed the sonographic diagnosis of hemangioma. Levovist appears useful in depicting slow flow in a large-vessel hemangioma.

2/90. Ultrasonographic detection of fetal cranio-facial hemangioma: case report and review of the literature.

    A case of an isolated cranio-facial vascular anomaly, extending from the left parietal bone to the lateral margin of the omolateral orbit is presented. Detection and differential diagnosis of fetal hemangioma is important for a variety of reasons. First, it allows the prenatal growth of the mass to be evaluated. Second, it enables appropriate arrangements for delivery to be made including its timing and selection of the appropriate clinical team necessary to support the neonate. After birth these cranio-facial anomalies can regress spontaneously, but plastic surgery is often necessary.

3/90. Prenatal MRI in a fetus with a giant neck hemangioma: a case report.

    We report a fetus with a giant neck hemangioma which was examined by MRI in utero. The initial diagnosis was made by ultrasonography. The sonolucent aspect of the mass, together with the presence of pulsating Doppler flow signals, was highly suggestive of a fetal hemangioma. In late pregnancy, fetal MRI revealed the location, size and characteristics of the neck tumor. Following prenatal corticosteroid treatment and premature delivery of the pregnancy due to fetal cardiac failure, the newborn received angiography and coil embolization of the tumor vessels. Despite vigorous treatments, the newborn died 12 h after birth. Evaluation of a fetal neck hemangioma by MRI is recommended late in pregnancy for precise information on the tumor and adjacent organs since the image is valuable for planning optimal perinatal treatment.

4/90. Intramuscular hemangioma of posterior neck muscles.

    Intramuscular hemangiomas of the head and neck are rare tumors, sparsely reported. They usually present themselves as a mass which enlarges suddenly. A case of intramuscular hemangioma involving the posterior neck muscles is presented. Computed tomography scanning, magnetic resonance imaging and angiography revealed the vascular nature of this lesion. Surgery consisted of a wide excision. The patient is free of disease after a 4-year follow-up.

5/90. radiation-induced malignant fibrous histiocytoma of the neck in a patient with laryngeal carcinoma.

    Fibrohistiocytomas are soft tissue tumors of histiocytic origin that have a variety of histological patterns. Although cases of malignant fibrous histiocytoma (MFH) of the head and neck have been reported with increasing frequency in recent years, they are considered rare. We report a case of the giant cell variant of MFH of the neck in which the patient had been given radiotherapy for T1 glottic cancer. prognosis of MFH, the use of radiation as primary treatment, and its role in the development of secondary primary tumors in the head and neck region are reviewed. [editorial comment: The authors stress the important relationship between prior radiation therapy and the induction of new tumors.]

6/90. Malignant fibrous histiocytoma arising in the upper posterior triangle of the neck.

    We describe the case of a 59-year-old man who came to us with a right neck mass of 4 to 5 months' duration. A histologic diagnosis of malignant fibrous histiocytoma was made, and the patient underwent wide local excision and neck dissection followed by postoperative radiation therapy. We briefly discuss the characteristics and treatment of this rare entity.

7/90. A case of head and neck kaposiform hemangioendothelioma simulating a malignancy on imaging.

    Kaposiform hemangioendothelioma (KH) is an endothelial-derived spindle cell neoplasm often associated with kasabach-merritt syndrome. Most cases arise in infancy and childhood and are soft-tissue tumors. The tumor displays an appearance between capillary hemangioma and Kaposi's sarcoma. We report a case of KH in a 1-year-old girl involving a mass that showed abnormal enhancement of soft tissue superficial to the right temporal bone with partial destruction of the temporal bone, the temporomandibular joint, mandibular condyle, and occipital bone. The physical finding of a discolored mass led clinicians to consider a hemangiomatous lesion, whereas the radiological picture suggested a more aggressive diagnosis of rhabdomyosarcoma and aggressive fibromatosis.

8/90. Diffuse neonatal hemangiomatosis without cutaneous lesions in an adult--a case report.

    Diffuse neonatal hemangiomatosis (DNH) is a rare disorder that first presents with multiple cutaneous and visceral hemangiomas during the neonatal period and has a high mortality rate. The authors report a long-term survivor of DNH who presented with multiple visceral hemangiomas without cutaneous lesions. Vascular endothelial growth factor (VEGF) may play an important role in tumor progression.

9/90. Follicular dendritic cell tumor of the cervical lymph node.

    Follicular dendritic cell (FDC) tumor is an uncommon sarcoma, which occurs in lymph nodes or in extranodal sites. A 61-year-old female was referred to our hospital because of a nontender mass on her neck. The mass was surgically removed and initially diagnosed as a metastasis of malignant fibrous histiocytoma (MFH). Because the subsequent progress was inconsistent with the common prognosis of a patient with MFH, the possibility of FDC tumor was taken into consideration. Immunohistological studies for FDC antigens, such as CD21 and CD35, led to final diagnosis. It is suggested that FDC tumor should be included in the diagnostic considerations when a spindle cell tumor is present in a lymph node.

10/90. Malignant fibrous histiocytoma of scalp with intracranial invasion 20 years after postoperative irradiation.

    We report a case of malignant fibrous histiocytoma with intracranial invasion occurring 20 years after radiation therapy for astrocytoma. magnetic resonance imaging suggested that a dumbbell-shaped tumor arising from soft tissue was pressing the brain. Histopathological findings revealed a storiform pattern with spindle cells such as fibroblasts, histiocytes, epithelioid cells with irregularly shaped nuclei, and bizarre giant cells. Three operations involving craniectomy were necessary because of recurrence.
