1/16. A continuous murmur after surgery for dissecting ascending aortic aneurysm.

    We report a case of a subcutaneous arteriovenous fistula that developed after aortic surgery. A careful physical examination and the selective use of imaging tests can differentiate this relatively benign complication from the more serious causes of a continuous murmur in this setting.

2/16. Ruptured aneurysm of the sinus of valsalva into the right atrium. An uncommon congenital heart defect.

    rupture of an aneurysm of the sinus of valsalva is an uncommon heart defect. A continuous murmur may be the first clinical sign of this rupture. Additional imaging techniques, and in the first place echocardiography, can be used to confirm the diagnosis. We present a case where, at the age 53 years, the diagnosis was made of a ruptured sinus of valsalva into the right atrium, with a typical windsock image on echocardiogram. Although several cases of a ruptured sinus of valsalva are reported in the literature, it is still an underdiagnosed condition because it is frequently missed during physical examination.

3/16. Echophonocardiographic diagnosis of left ventricular pseudoaneurysm.

    We report the presence of an unusual systolic murmur associated with a traumatic left ventricular pseudoaneurysm. Echophonocardiographic studies showed the murmur to begin at the first heart sound, but end well before the second heart sound. It seems likely that the murmur is caused by the systolic flow of blood from the left ventricle into the relatively noncompliant pseudoaneurysm. The echocardiographic scan of the left ventricle demonstrated a relatively echo-free space posterior to the left ventricular wall, supporting the diagnosis of pseudoaneurysm, which has confirmed with angiographic studies and at surgery. These findings indicate that a combination of noninvasive techniques is useful in establishing this diagnosis.

4/16. Apparent obstruction of the superior vena cava and a continuous murmur: signs of a fistula between a vein graft aneurysm and the right atrium.

    A previously undescribed complication of a saphenous vein aortocoronary bypass graft, namely formation of a fistula between a vein graft aneurysm and the right atrium is reported. A patient presented with a continuous murmur and a combination of signs suggesting superior vena cava obstruction. This pathology was shown by both echocardiography and angiography. Surgical treatment was attempted.

5/16. Continuous machinery murmur in an octogenarian.

    BACKGROUND: sinus of valsalva aneurysm is a rare congenital anomaly and usually presents in adolescence to early adulthood. Manifestations are varied, ranging from asymptomatic murmur to sudden death. CASE REPORT: An elderly male presented with chest pain and machinery murmur, which was confirmed as ruptured sinus of valsalva aneurysm on echocardiogram. Emergency surgical repair led to a successful outcome. DISCUSSION: review of the literature revealed that this patient is the oldest reported case. Urgent echocardiography and surgical advice are essential. This condition is potentially treatable even in the older age group and the prognosis is good after surgical repair.

6/16. Septal aneurysm and right ventricular obstruction: a case report.

    A 32-year-old man with a history of ventricular septal defect presented with systolic and diastolic murmurs and vectocardiographic evidence of right ventricular hypertrophy. echocardiography demonstrated a prominent systolic bulging of the interventricular septum into the right ventricle. Left ventricular angiograms indicated that the ultrasonic abnormality was based on an aneurysm of the membranous septum which produced a 35 mm Hg gradient within the right ventricle. After plication of this aneurysm, murmur and abnormal septal protrusion were not recorded.

7/16. Congenital aneurysm of the muscular interventricular septum in a child.

    Congenital aneurysms of the muscular interventricular septum without ventricularseptal defect are extremely rare. We describe a five-month-old girl patient with congenital aneurysm, pathological ST changes and minimal right ventricular outflow tract obstruction. The electrocardiogram showed Wolf-Parkinson-White syndrome, pathological ST changes and combined ventricular hypertrophy criteria. The transthoracic echocardiography and cardiac catheterization demonstrated a septal aneurysm which was bulging into the right ventricle and systolic pressure gradients between the main pulmonary artery and right ventricle outflow tract with normal coronary arteries. The patient has shown no symptoms, but some rhythm disturbances may be expected because of her electrocardiographic anomalies. Therefore, these patients should be followed carefully because of possible complications.

8/16. A continuous murmur.

    A 61-year-old male with a history of a mechanical Bjork-Shiley aortic valve replacement in 1977 and repair of an ascending aortic root dissection in 1993, presented with a 2-week history of fatigue and exertional dyspnea. echocardiography confirmed the presence of an aortic pseudoaneurysm with a fistulous connection between the aorta and right atrium, which subsequently underwent successful repair. Although there have been several cases describing the development of an aortic pseudoaneurysm after aortic dissection repair, our case is the first to describe an aorta-atrial fistula as a long-term complication of a previous aortic dissection repair.

9/16. An acute posterior myocardial infarction complicated with late pseudoaneurysm of the posterior wall.

    Diagnostic image: An 80-year-old man is referred because of an acute posterior infarction. Because of persisting pain and apparition of a new murmur, an echocardiography is performed showing inferoposterolateral akinesis complicated by ventricular wall pseudoaneurysm. After discussing with patient and family, conservative therapy was continued. A clue for diagnosis of pseudoaneurysm is the presence of a systolic murmur.

10/16. Intermittent to-and-fro murmur in cardiac pseudoaneurysm: Doppler echocardiographic findings.

    A case is presented in which a to-and-fro murmur was heard at auscultation. Cross-sectional echocardiography, combined with Doppler examination, showed that the murmur was produced by flow across the neck of a left ventricular pseudoaneurysm.
