    We describe a case of early human immunodeficiency virus infection characterized by transient eosinophilia without an elevated immunoglobulin e concentration, allergic symptoms, or atopic dermatitis. Possible mechanisms of the eosinophilia are discussed.

3/19. Immunologic evaluation of dental patient with history of hypersensitivity reaction to sodium hypochlorite.

    A 12-year-old girl, with a previous history of bronchial reaction and contact dermatitis to sodium hypochlorite, was referred for root canal treatment. Complete immunologic evaluation revealed a mild hypersensitivity condition, as it was assessed by the RAST investigation to different allergens and the DTH reactivity expressed though migration inhibition test. The absence of a serious immunologic disregulation in the patient's immunologic profile justified the term 'non-allergic hypersensitivity' to sodium hypochlorite to describe the condition.

4/19. Alantolactone sensitivity in chrysanthemum contact dermatitis.

    A case of occupational chrysanthemum contact dermatitis is reported. patch tests showed the patient to be sensitized to chrysanthemum morifolium (Chr M) leaves, flowers, and stems (alcoholic extracts) and to alantolactone. An attempt at desensitization appears to have been successful. Gas chromatography indicated the presence of alantolactone in all the various parts of Chr M, mostly in the flowers. The "maximization test" succeeded in sensitizing guinea pigs to alantolactone.

5/19. Contact dermatitis to alcohol swabs masquerading as vaccine allergy.

    A 14-year-old girl developed an eruption on her upper arm of pruritic, follicular papules each time she received a vaccination. This was initially thought to be a local reaction to the vaccines. After a similar rash appeared at a venepuncture site, a contact allergy to the alcohol swabs used before the procedures was postulated. A repeated open application test clarified this observation.

6/19. Chromate sensitization and elicitation from cement with iron sulfate.

    For some years, iron sulfate has been added to cement manufactured in the Scandinavian countries to prevent sensitization to and elicitation from chromate in cement. Allergic contact dermatitis from chromate is reported here in 3 workers with hand dermatitis and exposure to cement containing iron sulfate. Although iron sulfate had been added to the cement, high chromate concentrations were found in many samples of cement to which these workers were exposed.

7/19. Allergic contact dermatitis to nickel in children with atopic dermatitis.

    We report two atopic boys with allergic contact dermatitis to nickel. Both children had early onset of atopic dermatitis and subsequently presented with infraumbilical dermatitis corresponding to the site of contact with metal snaps. A positive patch test response to 2.5% nickel sulfate in petrolatum was observed in both boys. Allergic contact dermatitis in patients with atopic dermatitis is not uncommon and probably occurs more often than recognized.

8/19. Cinnamic aldehyde in toothpaste. 1. Clinical aspects and patch tests.

    Although cinnamon is known to cause dermatitis in bakers and confectioners, it has only rarely been reported as causing trouble in food or cosmetics. A newly-formulated 'spicy' toothpaste containing cinnamon as a flavouring agent was the cause of oral symptoms in eight patients referred to clinics in Buckinghamshire and in Malmo and in a further eight patients discovered subsequently. Similar symptoms and patch test results were observed independently at both centres. Positive reactions were obtained with 1% cinnamic aldehyde in 15 out of 16 patients tested. Only one patient reacted to a standard sample of balsam of peru (25% in petrolatum).

9/19. Palindromic rheumatism: an allergic manifestation?

    A case of palindromic rheumatism is presented in which the arthritic attacks usually were accompanied by an allergic vasculitis and dermatitis. Although this is not proof of the etiology, the constant association is suggestive of an allergic cause. A brief review of palindromic rheumatism in included.

10/19. plasmapheresis treatment in patients with severe atopic keratoconjunctivitis.

    A 19-year-old woman with hyperimmunoglobulinemia E (the hyper-IgE syndrome [HIE]) and a 62-year-old man with atopic dermatitis experienced dramatic improvement in their chronic ocular symptoms and signs of atopic keratoconjunctivitis after plasmapheresis was instituted. Both patients had previously received topical and oral corticosteroids as well as topical cromolyn sodium without significant beneficial effect. The authors recommend a trial of plasmapheresis in selected patients with recalcitrant and debilitating atopic keratoconjunctivitis when standard therapy proves unsuccessful.
