1/35. Occlusal rehabilitation using implants for orthodontic anchorage.

    osseointegration is defined as a direct interaction of bone to an implant surface. As a result, the implant fixture is immobilized in the bone and lends itself to function as an anchor for orthodontic tooth movements. When properly treatment-planned, these implants can also be used as prosthodontic abutments for single crowns, or removable or fixed partial dentures. This article describes how implant fixtures were surgically placed within the maxillary and mandibular arches of a partially edentulous patient, and used for orthodontic anchorage to reposition the remaining teeth into a more favorable arch position, creating increased posterior interocclusal space. The fixtures were then restored with fixed partial dentures to rehabilitate the patient into a mutually protected occlusion.

2/35. Short and sticky options in the treatment of the partially dentate patient.

    As we move into the twenty-first century, patterns of dental disease in adults are changing. Surveys of adult dental health indicate that more people are keeping their teeth for longer in life. In many cases, the ravages of dental disease and the cumulative effect of a lifetime of restorative dentistry lead to gradual tooth loss. For many of these patients, restoration of a complete dentition may not be feasible nor desirable. In recent years, functionally oriented treatment planning has become acceptable in light of recent research findings. Using this approach, treatment efforts and resources are directed principally at retaining the 'strategic' part of the dentition in the long term, ie, the anterior and premolar teeth. This paper describes, with the aid of treated cases, a means of combining a shortened dental arch strategy with resin bonded bridgework. With the aid of recent research in this area of clinical practice, some suggestions as to the use of the technique are also described.

3/35. The Royal london Space Planning: an integration of space analysis and treatment planning: Part II: The effect of other treatment procedures on space.

    The Royal london Space Planning process is carried out in 2 stages. The first stage, assessing the space required to attain the treatment objectives, was described in Part I of this report, published earlier. In Part II, the process of integrating space analysis with treatment planning continues with consideration of the effects other treatment procedures have on space. These procedures include tooth enlargement or reduction, tooth extraction, the creation of space for prosthetic replacement, and mesial and distal molar movement. The effects of favorable and unfavorable growth are also considered. A brief case report is presented to demonstrate use of the Royal london Space Planning.

4/35. tooth wear and loss: symptomatological and rehabilitating treatments.

    The authors report a clinical case that presented tooth wear and absence, with painful muscular and articular symptomatology, and also alteration in deglutition, mastication and speech. The clinical procedures used were re-establishment of vertical dimension of occlusion, mandibular centric relations, and occlusal contacts through therapeutic removable partial dentures. The condyle position was analyzed in habitual occlusion and in occlusion with dentures, through transcranial radiographs of the temporomandibular joints. Oral rehabilitation was achieved with dental restoration and removable partial dentures.

5/35. A conservative aesthetic solution for a single anterior edentulous space: case report and one-year follow-up.

    Conservative solutions for the restoration of a single edentulous space in the anterior maxilla present an aesthetic challenge to the clinician. Treatment options for tooth replacement include implant placement, a classical fixed partial denture, or a resin-bonded appliance with a metal substructure or fiber-reinforced resin. The resin-bonded Encore Bridge (daVinci Studios, Woodland Hills, CA) offers a restorative solution that is conservative and aesthetic when compared to the required full-tooth preparation in a conventional three-unit fixed partial denture. This case report addresses the indications, preparation guidelines, and restorative procedures for an Encore Bridge.

6/35. Bone dynamics of osseointegration, ankylosis, and tooth movement.

    Masticatory function challenges the strength and adaptive capability of supporting bone. When osseous tissue is loaded, it accumulates fatigue damage which must be repaired by bone modeling and remodeling. The three principal masticatory abutments (normal teeth, ankylosed teeth and osseointegrated implants) are a dynamic physiologic continuum relative to bone biomechanics. Implants are rigidly integrated units that can only be moved by fracturing the interface. Normal teeth and some ankylosed teeth can be moved using implants for orthodontic and orthopedic anchorage. Because orthodontic translation generates new bone and attached gingiva, it is a form of tissue engineering. Modern interdisciplinary practice requires a thorough knowledge of the principles of bone physiology and biomechanics.

7/35. Maxillary and mandibular overlay removable partial dentures for the treatment of posterior open-occlusal relationship: a clinical report.

    This clinical report describes the use of maxillary and mandibular overlay removable partial dentures to treat a patient with class III skeletal malocclusion and a posterior open-occlusal relationship. Overlay removable partial denture therapy was used as an alternative to other options such as orthodontics and combined orthodontic/oral surgery procedures; it satisfied the esthetic and functional requirements of the patient and provided a stable occlusion. Overlay removable partial dentures are a reversible and relatively inexpensive treatment for patients with congenital or acquired anomalies, but the potential disadvantages of these prostheses include compromised esthetics when the dentures are removed; caries and periodontal disease as a result of poor oral hygiene; and veneer material fracture, debonding, discoloration, and wear.

8/35. Immediate implant placement and provisionalization in edentulous, extraction, and sinus grafted sites.

    The incorporation of restorative procedures during implant placement, as well as during the creation of natural emergence profiles and lifelike ceramic restorations, has become the focus of implantology over the last few years. Recent publications have provided guidelines for success with the immediate restoration procedure and have presented basic surgical protocols for the implant team. Enhancement of the healing phase through the local delivery of growth factors to the surgical site, as well as through advancements in bone grafting materials, has allowed the implant surgeon to accomplish multiple surgical procedures during the initial surgical visit. In addition, advancements in surgical stent designs have allowed the restorative dentist to adequately communicate to the surgeon during surgery the parameters required in the final restoration to replace the natural tooth system with form, function, and esthetics. This article presents the results of more than 400 immediate restored implants placed in edentulous sites, fresh extraction sockets, and sinus grafted sites. Also highlighted are guidelines for surgical success, as well as a description of a surgical stent design that communicates requirements for restorative success to the surgeon, while also serving as an esthetic provisional restoration.

9/35. Augmentation of an anterior edentulous ridge for fixed prosthodontics with combined use of orthodontics and surgery: a clinical report.

    Untreated periodontitis may lead to tooth and tissue loss that can result in diminished masticatory function and esthetic deformities. Combined periodontal/prosthodontic treatment for patients with advanced periodontal disease has been well documented, and treatment results can often be improved with the use of adjunctive orthodontic treatment such as forced eruption. This clinical report describes a method of preprosthetic preparation of an edentulous ridge for a fixed partial denture that incorporates the combined use of orthodontics and surgical ridge augmentation.

10/35. Provisionalizing periodontally involved dentition with implants.

    Many patients who are partially or fully edentulous have practiced poor oral hygiene. However, implants can benefit even those patients who are not ideal candidates. In cases of severe periodontal compromise, implants may enhance the survival of the natural teeth, minimize the difficulties of transition if additional tooth loss occurs, and also offer significant psychological benefit to the patient.
