1/7. Three-dimensional CT scan analysis for implant-supported fixed prostheses.

    Rapid technological advances in dental implantology, computer imaging, and dental radiography in combination with traditional prosthetic concepts offer today's clinicians unprecedented possibilities in diagnosis and treatment planning. This article presents information on the use of three-dimensional computed tomography scan analysis to determine the quality (density), quantity, and location of bone to establish the most effective implant placement. In addition, this article analyzes the correlation of prosthodontic, periodontic, and technological concepts necessary for completion of a maxillary and mandibular fixed, full-mouth rehabilitation.

2/7. Implant-supported denture in a patient with Huntington's disease: interdisciplinary aspects.

    patients with extrapyramidal diseases often cannot maintain independent, efficient oral hygiene due to restricted motor ability of the upper extremities and lack of coordination. The hermetic closure of the mouth and lips, and the associated ability to keep liquid and toothpaste in the mouth, can become so weak that effective oral hygiene cannot be maintained. Over a period of many years, this illness leads to loss of teeth and the need for complete prosthodontic care. Dyskinesia and hyperkinesia of the tongue and the peri-oral musculature, combined with xerostomia and pooling of saliva, make it impossible for the patient to wear a conventional complete denture, despite an anatomically-adequate bearing area. In such cases, an implant-supported prosthesis is a better therapeutic measure, although some aspects of oral hygiene must initially be disregarded. Two ITI implants were inserted into the anterior mandibular region of a patient with Huntington's chorea, because a complete denture could not be retained on the alveolar ridge, despite adequate vestibule depth, due to tongue dyskinesia. A bar joint was used to anchor this mucosal-borne denture. This implant-supported complete denture led to a clear improvement in the patient's chewing function when observed over a period of a year.

3/7. Rehabilitation of dental implants for the post-irradiated and marginally resected mandible in an oral cancer patient.

    We describe a case with severely compromised edentulous ridge in the mandible, which previously received marginal resection and radiotherapy due to oral cancer at the mouth floor. Through careful evaluation, the patient had 30 dives of hyperbaric oxygen therapy (HBO) before implant surgery. The edentulous ridge was rehabilitated with 4 endosteal implant fixtures, and palatal mucosa grafting vestibulosulcoplasty. The case had been followed for 4 years with stability of bone and a satisfactory result of rehabilitation.

4/7. Spontaneous fracture of hypertrophied genial tubercles.

    A case of spontaneous fracture of hypertrophied genial tubercles is reported. This is an uncommon event with only six previously reported cases. This entity is usually associated with severe edentulous bone resorption, a condition related to the pathogenesis of this fracture. Clinical complaints include painful swelling of the floor of the mouth and impaired tongue function. If left untreated, mucosal inflammatory changes may appear and a biopsy should be performed. Total recovery is obtained after removal of the fractured fragments.

5/7. The fabrication and use of an extraoral surgical guide for implant placement coincident with soft tissue matrix expansion grafts: a clinical report.

    During soft tissue matrix expansion (tent pole) graft (STMEG) surgery, the traditional intraoral surgical guide for dental implant placement cannot be used because the surgery is initiated and maintained outside the oral cavity. The essential records established from an intraoral surgical guide must be transferred to an extraoral surgical guide. This clinical report describes the fabrication and use of an extraoral surgical guide employed during STMEG surgery.

6/7. hemorrhage in the floor of the mouth during implant placement in the edentulous mandible: a case report.

    This report describes a life-threatening hemorrhage in the floor of a patient's mouth during routine implant placement in the anterior mandible. airway obstruction caused by hematoma development resulted in acute nasotracheal intubation and subsequent surgical intervention. Surgical, radiographic, and anatomic considerations to prevent severe bleeding are discussed. An extraoral submental approach in cases with large sublingual hematomas is recommended. An outpatient should be treated in or close to a hospital where these complications can be dealt with promptly and effectively.

7/7. The use of precision attachments in a lower full arch rehabilitation with osseointegrated implants--a clinical report.

    A technique for restoration of a case of Kennedy Class V edentulous span was described. This situation is a long edentulous span bounded anteriorly by a weak support and considered as one of the most difficult situations to restore. Two osseointegrated screw implants were inserted in the edentulous span. Together with remaining natural teeth supported a full arch restorations. Three precision attachments were included in the restoration to allow movement of the remaining teeth and to allow broken stresses at where the prosthesis extended along the corners of the mouth.
