1/10. Synovial chondromatosis of the subcoracoid bursa.

    Synovial chondromatosis, is the chondroid metaplasia of the synovial membrane. Large joints such as the knee and hip are most commonly involved. Extraarticular involvement is rarely described. Synovial chondromatosis may be associated with impingement syndrome of the shoulder. We report a case of synovial chondromatosis of the subcoracoid bursa, which resulted in impingement symptoms.

2/10. Synovial chondromatosis of the temporomandibular joint: varying presentation in 4 cases.

    Synovial chondromatosis is a rare condition in which cartilage is formed in the synovial membrane of a joint. The manifestations of this benign neoplastic process can mimic many common temporomandibular joint and parotid diseases. Four cases of synovial chondromatosis are presented. In each case, atypical presentation, coexisting joint disease, or both caused diagnostic confusion. The histories and physical examinations were initially consistent with more common joint diseases in each case. Imaging provided some insight into diagnosis and was a definitive indication for surgical treatment. Treatment by subtotal synovectomy and by removal of chondromatous nodules were undertaken in each case. No patient in our series has had recurrence of disease or symptoms after surgical treatment.

3/10. Localized pigmented villonodular synovitis of the knee: diagnostic challenge and arthroscopic treatment: a report of three cases.

    The localized form of pigmented villonodular synovitis (LPVS) is a lesion characterized by focal involvement of the synovial membrane. The knee is the most commonly affected joint. We report three cases of LPVS of the knee which were not diagnosed upon clinical evaluation. The aim is to bring the attention of clinicians to this pathological entity, which is often regarded as extremely rare and is therefore not considered in the early differential diagnosis of various knee derangements. Diagnostic and therapeutic arthroscopy was performed. The lesions were completely resected and patohistological findings confirmed the diagnosis of LPVS. All of our three patients have remained asymptomatic at 8, 10, and 12-month follow-ups.

4/10. Irreducible dorsal dislocation of the proximal interphalangeal joint caused by an osteochondral fragment.

    We report two cases of irreducible dorsal dislocation of the proximal interphalangeal joint caused by interposition of a small osteochondral fragment that avulsed from the base of the middle phalanx with the volar plate and outstretched collateral ligament.

5/10. Biodegradable screw presents as a loose intra-articular body after anterior cruciate ligament reconstruction.

    We report a case of intra-articular movement of a broken piece of a poly-L-lactide (PLLA) bioabsorbable interference screw from the femoral tunnel in anterior cruciate ligament (ACL) reconstruction with quadrupled semi-tendinosus and gracilis tendon grafts. Eleven months after initially successful ACL surgery, the patient felt a sudden locking of the knee without associated trauma or injury. The patient experienced pain and swelling episodes after heavy lifting with knee flexion at work, but without symptoms of giving way or locking. On revision arthroscopy, a broken part of a bioabsorbable interference screw was seen in the lateral compartment, which was subsequently removed without incident. The semitendinosus-gracilis graft appeared intact without disruption. After revision surgery, the patient's recovery was uneventful, with return to activity within a few months. This case further shows the problem of biointerference screw breakage in ACL reconstructive surgery and the need to bury the femoral bioabsorbable interference screw on graft fixation.

6/10. Irreducible palmar dislocation of the proximal interphalangeal joint caused by a fracture fragment: a case report.

    We present a case of irreducible palmar dislocation of the proximal interphalangeal joint of the little finger caused by entrapment of a fracture fragment attached to the collateral ligament. The bony fragment was trapped between the radial condyle of the proximal phalanx and the volar plate. Reduction was easily accomplished by hooking out the fragment.

7/10. Arthroscopic treatment of an avulsion fracture of the ligamentum teres of the hip in an 18-year-old horse rider.

    We present the case of an 18-year-old horse rider with an avulsion fracture of the ligamentum teres causing persistent hip pain and locking for 2 years. The patient had no history of major trauma; however, repeated minor hyperabduction injuries occurred. We were able to successfully treat this patient by arthroscopic removal of the bony fragments and two loose bodies and partial resection of the ligamentum teres.

8/10. Loose intra-articular body following anterior cruciate ligament reconstruction.

    We report a case of intra-articular fracture of a bioabsorbable fixation device from the femoral tunnel in an anterior cruciate ligament reconstruction using a bone-tendon-bone graft. Thirteen months after successful reconstruction surgery, the patient experienced episodes of locking and medial joint pain. There was no history of trauma and no symptoms of instability or swelling. On revision arthroscopy, a fractured tip of a bioabsorbable RIGIDfix cross pin (Mitek, Westwood, MA) was identified in the medial compartment of the knee. There was a broad area of chondral erosion affecting the medial femoral condyle and a small defect to the medial tibial plateau where the loose body had been lodged. The bone-tendon-bone graft was intact without disruption. After arthroscopy, the patient was symptom free for 3 weeks but then developed further symptoms of locking. magnetic resonance imaging showed another loose body within the knee. A repeat arthroscopy was performed 6 weeks after the earlier procedure and another piece of the polylactic acid RIGIDfix cross pin was removed, this time from the lateral gutter. This case raises concern about the potential for breakage and resultant loose body formation that may occur after bioabsorbable cross-pin fixation and, particularly, the associated chondral damage that can occur if early intervention is not conducted.

9/10. Synovial chondromatosis of the temporomandibular joint: report of two cases.

    Two cases of synovial chondromatosis of the temporomandibular joint are reported. This condition is rare but benign, with only 36 cases reported in the literature to date. Symptoms include tenderness, swelling, and limited range of motion, with deviation to the affected side. diagnosis is made both from the clinical presentation and histologic examination. The etiology is thought to be cartilaginous foci within the synovial membrane that become detached and proliferate in the synovium as chondrocytes. Treatment includes removal of the "loose bodies" and possible resection of the synovial membrane, condyle, and disk.

10/10. Synovial osteochondromatosis: an unusual cause for subacromial impingement.

    Impingement syndrome is usually caused by encroachment on the rotator cuff of the acromion, coracoacromial ligament, acromioclavicular joint or coracoid process. Bursal causes of impingement are rare but include rheumatoid thickening, and thickening from previous and iatrogenic causes such as sutures, pins, or wires left from previous surgery. We report a case of synovial osteochondromatosis presenting as chronic impingement syndrome. Synovial osteochondromatosis may be difficult to differentiate from chronic calcific tendinitis, but we describe a radiological sign and its anatomical basis that can differentiate between the two. If densities are seen on plain radiographs distal to the greater tuberosity, then loose bodies within the subacromial bursa should be considered.
