1/30. Hypernephroma arising in wall of simple renal cyst.

An unusual case of hypernephroma arising from the wall of a simple renal cyst is presented. Despite prior controversy as to the existence of this entity continuity between normal and neoplastic cells lining the cyst wall was demonstrated thus confirming this lesion as a distinct entity. Furthermore, we wish to direct attention to the limitations of biochemical and cytologic analysis of renal cyst fluid in distinguishing benign and neoplastic lesions.

2/30. The coexistence of cystic nephroma and renal cortical adenoma.

Cystic nephroma is rare, but its coexistence with renal cortical adenoma is unique and has never been reported previously. We present a unique case of simultaneous occurrence of cystic nephroma and renal cortical adenoma in 47-year-old man. Both tumours were situated in the lower pole of the right kidney and were clinically asymptomatic.

3/30. Spontaneously ruptured multilocular cystic nephroma.

Multilocular cystic nephroma is a relatively rare benign tumor of the kidney. An adult case of spontaneously ruptured multilocular cystic nephroma was reported. A 33-year-old woman who presented with a right flank colic attack. The preoperative diagnosis was spontaneously ruptured renal tumor. A right nephrectomy was performed. A multilocular cystic lesion, 10 cm in diameter, was found in the removed specimen, which had a hematoma inside and direct tumor extension into the renal pelvis. On microscopic examination, the surface of loculi was covered by a layer of hobnail epithelium, and septa were composed of fibrous tissue with dilated vessels, lacking in normal renal components. There were no signs of malignancy. The lesion extending into the renal pelvis showed the same microscopic findings. The final diagnosis was multilocular cystic nephroma. Two respects, spontaneous rupture and tumor extension into the renal pelvis, were unique to the present case. There has been no report of spontaneously ruptured mutilocular cystic nephroma. The tumor extension into the renal pelvis, however, has been found in several reports. This growth pattern might be one of the characteristics of this benign renal tumor.

4/30. Local recurrence of a unilateral cystic nephroma.

Cystic nephroma is an uncommon, benign renal lesion. We report the first case of local recurrence of a unilateral cystic nephroma in an adult. Only three cases of recurrence in bilateral cystic nephroma have been described in the literature before. Other renal lesions may not be differentiated preoperatively from cystic nephroma and thus require surgical exploration. Long-term follow-up is recommended to rule out local recurrence.

5/30. Multilocular cystic nephroma in adults.

Predominantly cystic neoplasms of the kidney may cause diagnostic difficulties. We describe a case of cystic nephroma, a rare renal tumour characterized by a mixture of epithelial and stromal elements, in an elderly man, uncommonly located close to the renal pelvis.

6/30. Small lymphocytic lymphoma involving an enlarging complex renal cyst.

A 47-year-old white man presented for evaluation of a complex right renal mass. He had a history of human immunodeficiency virus. Cervical lymph node biopsy had revealed small lymphocytic lymphoma. Computed tomographic scan disclosed diffuse mesenteric and retroperitoneal adenopathy consistent with chronic lymphocytic leukemia, as well as a 4.5-cm complex cystic right renal mass, which 17 months later enlarged to 6.2 cm. The mass resembled multiloculated cystic nephroma. Partial nephrectomy revealed infiltration of the cyst wall by small lymphocytic lymphoma. To our knowledge, this is the first reported case of lymphoma arising in or colonizing a renal cyst.

7/30. Case report: bilateral multilocular cystic nephromas: a rare occurrence.

We present a rare case of bilateral renal cystic tumours in an 18-month-old female child, which proved to be cystic nephromas on imaging and histopathology. Although unilateral cystic nephromas are known in children; the occurrence of bilateral tumours is rare.

8/30. A complicated case of bilateral cystic nephroma in a 16-month-old boy.

We describe a case of a progressive bilateral cystic nephroma (BCN) in a child undergoing a multistaged surgical procedure. After partial resection of the left sided tumor, a progress occurred on that side and the left kidney had to be removed 10 weeks later. After 35 months a tumor progression was observed on the right side together with an ureteral obstruction leading to a decreased renal function. In a third operation a complete tumor resection on the right side was achieved through longitudinal partial nephrectomy, reconstruction of the renal pelvis, and reanastomosis of the reconstructed pelvis and ureter. The patient showed no evidence of the disease at 28 months of follow-up. The presented case provides an evidence that in BCN a tumor progress may occur after multistaged surgical approaches. A single-staged complete tumor resection with renal salvage techniques seems indicated.

9/30. Cystic renal cell carcinoma arising from multilocular cystic nephroma of the same kidney.

Multilocular cystic nephroma is an uncommon benign entity grouped among the cystic non-genetic diseases. It is characterized by variable-sized, non-communicating cysts separated by irregular thin walled septa. Though multilocular cystic nephroma is usually considered a benign lesion, malignant changes in the cysts should not be overlooked.

10/30. Cystic partially differentiated nephroblastoma.

Cystic partially differentiated nephroblastoma is a relatively rare tumour of the kidney usually affecting infants. Cystic Wilms' tumour and multilocular cystic nephroma should be distinguished from cystic partially differentiated nephroblastoma. Multilocular cystic nephroma is a benign tumour whereas cystic Wilms' tumour is at the malignant end of the range of classification of such tumours. Cystic partially differentiated nephroblastoma may undergo local recurrence but there is no report of metastasis.
