1/30. Occupational neutrophilic asthma.

    Occupational asthma is typically associated with an eosinophilic bronchitis. The case of a 41-year-old woman who developed symptoms of asthma after occupational exposure to metal working fluids is reported. The diagnosis of asthma was confirmed by an forced expiratory volume in 1 s (FEV1) of 1.7 (59% predicted), with 11% reversibility after inhaled bronchodilator and a provocation concentration of methacholine to cause a fall in FEV1 of 20% (PC20) of 0.4 mg/mL. Induced sputum examination showed a marked neutrophilia. Over the next six months, serial sputum analyses confirmed the presence of a marked sterile neutrophilic bronchitis during periods of occupational exposure to metal working fluids, which resolved when the patient was away from work and recurred when she returned to work. The sputum findings were mirrored by corresponding changes in spirometry and PC20 methacholine. The findings indicate the occurrence of occupational asthma associated with an intense, sterile neutrophilic bronchitis after exposure to metal working fluids.

2/30. The clinical spectrum of Sweet's syndrome (acute febrile neutrophilic dermatosis)-a report of eighteen cases.

    Eighteen cases of Sweet's syndrome are described. This is the largest series so far reported and the first from the tropics. The essential features are the characteristic morphology, the histological appearances, the dramatic response to corticosteroids and the absence of scarring. attention is drawn to the frequent involvement of the eyes and joints during the course of the illness. fever and neutrophilia were found less frequently than a raised ESR or a preceding upper respiratory tract or skin infection. The term Sweet's syndrome is preferred to acute febrile neutrophilic dermatosis. The frequency of occurrence of different clinical manifestations is discussed.

3/30. Acute febrile neutrophilic dermatosis (sweet syndrome).

    We describe the third reported case of acute febrile neutrophilic dermatosis (sweet syndrome) in the united states. In the majority of reported cases, the patients were female. However, in our case and in the two cases reported previously in the united states, the patients were male. In two of these three male patients, a malignant neoplasm has been found, possibly as a coincidence.

4/30. Neutrophilic pleocytosis in cerebrospinal fluid: adult-onset Still's disease.

    We describe a unique patient whose clinical and laboratory findings fulfill diagnostic criteria of adult onset Still's disease and at the same time, this case was complicated by aseptic meningitis with neutrophilic pleocytosis in cerebrospinal fluid, as well as sensorineural hearing loss. The symptoms of the patient improved greatly with prednisolone therapy. Some studies in the literature suggest that this disease may lead to aseptic meningitis with neutrophilic pleocytosis.

5/30. cerebrospinal fluid neutrophilic pleocytosis in hospitalized west nile virus patients.

    A description of 7 consecutive patients admitted for various forms of neurological impairment. All these patients were found to have cerebrospinal fluid positive for west nile virus IgM, and routine bacterial cultures were negative. Six of these patients did not present with the typical lymphocytic pleocytosis often quoted when discussing a viral meningitis/encephalitis; rather most presented with a cerebrospinal fluid neutrophilia.

6/30. Sweet's syndrome associated with sideroblastic anaemia.

    Previous reports have noted an association between Sweet's syndrome (acute febrile neutrophilic dermatosis) and leukaemia, and less commonly other haematological abnormalities. We report a previously unrecognised association between Sweet's syndrome (SS) and sideroblastic anaemia (myelodysplastic syndrome--refractory anaemia with ring sideroblasts). Both patients were males and one had prominent extracutaneous features of SS. bone marrow cytogenetic studies were normal in this latter patient and neither patient showed progression to leukaemia.

7/30. Acute febrile neutrophilic dermatosis (Sweet's syndrome) following BCG vaccination.

    Several factors may trigger or be associated with acute febrile neutrophilic dermatosis (AFND). We report a case of AFND following BCG vaccination as an interesting association although no direct interrelation can be certified.

8/30. Febrile neutrophilic dermatosis in acute myelogenous leukemia.

    A 50-year-old woman with acute myelogenous leukemia on two occasions had fever and exquisitely tender, warm, erythematous, indurated to frankly bullous plaques on the face and upper extremities. Histologically, the lesions showed rather dense dermal infiltrates of mature polymorphonuclear leukocytes; special stains for bacteria and fungi were negative, as were all cultures for infectious agents. The lesions did not respond to antibiotics but cleared rapidly with systemic steroid treatment. Febrile neutrophilic dermatosis of acute myelogenous leukemia is believed to be a nonspecific reaction to the underlying malignant disease process. By prompt recognition of this entity, prolonged expensive courses of antibiotics may be avoided.

9/30. Acute febrile neutrophilic dermatosis: Sweet's syndrome.

    A case of acute febrile neutrophilic dermatosis (Sweet's syndrome) is described in a patient with chronic myelogenous leukemia, with a review of all published reports in the English literature. The disease is characterized by an acute febrile illness with painful plaques involving the extremities, face, and neck, and responds dramatically to corticosteroids with occasional recurrences. Histologically, the skin biopsy shows a dense dermal infiltrate of polymorphonuclear leukocytes without evidence of vasculitis.
