1/24. lip cancer: important management issues.

    lip cancer should be diagnosed and treated at an early stage. It most often presents on the lower lip of middle-aged to elderly males. The aetiology is similar to non-melanoma skin cancer, that is, chronic sun exposure. Despite treatment, a proportion of patients will experience delayed relapse and one in 10 will die from lip cancer. Clinicians need to be aware of the natural history of this disease. Two cases of squamous cell carcinoma of the lip treated by radiotherapy are presented and discussed to highlight important aspects in the management of lip cancer.

2/24. Malignant melanoma of the lip spreading in a pagetoid manner into the minor salivary glands.

    We describe a malignant melanoma of the lower lip that upon recurrence extended in a pagetoid manner into the ducts of the oral minor salivary glands. immunohistochemistry with antibodies to MART-1 and HMB-45 confirmed that the atypical cells in the ducts of the salivary glands were indeed melanoma cells. Pathologists involved in staging of oral melanoma are urged to look out for this previously unreported mode of spread of labial malignant melanoma.

3/24. Desmoplastic malignant melanoma of the lip: a report of 6 cases and review of the literature.

    Desmoplastic malignant melanoma is a rare neoplasm consisting primarily of spindle-shaped melanoma cells embedded in a fibrous stroma, with approximately 6% of cases occurring in the lip. A literature search revealed that most reported cases occur in sun-exposed areas in older men. Few cases of desmoplastic malignant melanoma of the lip in young people (in their mid-twenties) have been described. We report 6 previously undocumented cases of labial desmoplastic malignant melanoma occurring in young individuals and review 20 cases from the literature. physicians should consider the possibility of desmoplastic malignant melanoma in young people who present with atypical lip lesions, which on histopathologic analysis demonstrate spindle-cell hyperplasia. Although this diagnosis is rare and there have not been reports of such patients in the literature thus far, our findings should alert clinicians to the possibility of desmoplastic malignant melanoma as a diagnosis for lip lesions in young people.

4/24. Neurotropic melanoma. A case report and review of the literature.

    Neurotropic melanoma is a rare tumor with a biphasic growth pattern associated with a change in morphology from melanocytic features toward Schwann cell features. The tumor thereby develops a capacity for infiltrating nerves which may result in clinically evident cranial neuropathies, most commonly of the fifth and seventh cranial nerves. The histology of this lesion is difficult to interpret; it often erroneously appears fibrous in origin and may be considered to be benign. Despite this benign appearance histologically, the tumor behaves aggressively with multiple local recurrences and possible CNS invasion by either direct perineural growth or distant metastases. We review the literature of neurotropic melanomas and report a 46th case which describes a typical course with neural invasion. Although a rare cause of cranial neuropathies, the neurologist should consider this entity in the differential diagnosis and the history of a recurrent skin lesion of the face should be sought whenever examining such a patient.

5/24. Total reconstruction of the upper lip after resection of a malignant melanoma.

    The upper lip was totally reconstructed with a radial forearm sensory flap and vermilionplasty using medical tattooing after resection of a malignant melanoma. Three courses of chemotherapy (dacarbazine, nimustine, and vincristine) were given postoperatively. The reconstructed lip had good contour, colour, and sensory recovery.

6/24. Comparison of dermoscopic and histopathological findings in a mucous melanoma of the lip.

    Mucous melanomas on the lip are very rare. None of the few available reports has provided a comparison of dermoscopic and histopathological findings. We describe a 33-year-old Japanese man with a mucous melanoma on the lower lip and present a comparison of our dermoscopic and histopathological findings.

7/24. Neurotropic melanoma invading the inferior alveolar nerve.

    BACKGROUND: Desmoplastic neurotropic melanoma (DNM) and neurotropic melanoma (NM), rare lesions of the head and neck, often present as a benign-appearing nodule which later progresses to cranial nerve involvement. OBJECTIVE: To discuss treatment and outcome of 3 cases of DNM/NM of the lower lip. methods: Three case reports with 12- to 54-month follow-up and literature review. RESULTS: The first case had an initial excision of melanoma of his lower lip and presented 16 years later with severe bilateral atrophy of his muscles of mastication with NM and underwent intensity modulated radiation therapy but no further resection. We also report two other cases of DNM of the lip, one of which received gamma knife radiosurgery for intracranial extension. LIMITATIONS: Small retrospective case series. CONCLUSIONS: Locoregional recurrences from DNM are more common than distant metastasis warranting vigilant surveillance following resection of the initial lesion. radiation therapy and gamma knife radiosurgery may be used for cases of unresectable recurrences.

8/24. Superficial melanomas of oral mucous membranes.

    In accordance with microscopic and clinical criteria established for superficial melanomas of the skin (superficial spreading melanoma, lentigo maligna melanoma, acral-lentiginous melanoma), three oral lesions have been evaluated. The literature on oral melanomas has also been reviewed, with special attention given to those cases that had pre-existing melanosis. One patient with a diagnosis of superficial spreading melanoma eventually died of his untreated lesion 11 years after its first appearance. Two patients had lesions diagnosed as acral-lentiginous melanoma (a group which also includes volar and subungual melanomas) that exhibited aggressive, recurrent behavior. These lesions had microsocpic features similar to lentigo maligna melanoma but did not behave in a manner consistent with that diagnosis. Electron microscopic study of one acral-lentiginous melanoma demonstrated malenosomes and premelanosomes that were like those seen in normal melanocytes and nevus cells. The superficial or radial growth phase of many oral melanomas has apparently gone unrecognized. melanosis has been reported to be a common feature of invasive oral melanomas but has not generally been related to the natural history of these lesions. Oral lesions with a prolonged intra-epithelial or radial growth phase would be expected to have a better prognosis than nodular melanomas, but meaningful survival data are not available because of the infrequency with which oral melanomas have been subclassified.

9/24. A new technique for total reconstruction of the lower lip in a patient with malignant melanoma.

    A very rare case of malignant melanoma of the lower lip was presented. While there are many concepts and techniques for reconstruction of the lower lip including advancement flaps, switching flaps, rotation flaps, and regional flaps from the cheek, upper lip, tongue, forehead, neck, and chest, no single flap is ideal in every instance. Lower lip reconstruction utilizing a modification of bilateral nasolabial flaps has been described. The distinct advantages of utilizing myocutaneous flaps in a Z augmentation manner, including the maintenance of good functional competence of the lower lip, maintenance of adequate oral commissure diameter, and a good cosmetic result, have been demonstrated. While no single technique for reconstruction of the lower lip is ideal for every case, we feel that the method presented here offers a very good option when faced with the challenge of complete reconstruction of the lower lip.

10/24. Neurotropic melanoma of the lower lip.

    The neurotropic melanoma is a rare and highly malignant variant of melanoma that possesses a biphasic neoplastic component. A lentigo maligna melanoma commonly precedes a downgrowth of spindle cells that exhibit fascicle formation and striking neurotropism. A strong predilection for occurrence on the lip has been indicated by published reports. Difficulties exist in the initial diagnosis of the tumor because of its biphasic growth pattern, while the biologic behavior of the tumor is that of multiple recurrences, perineural metastases with cranial neuropathies and intracranial disease, and regional and distant organ metastases. The clinical and histopathologic features of a neurotropic melanoma of the lip are shown, and a brief review of the histogenesis, treatment, and prognosis are presented.
