1/13. Massive lymphatic fluid leakage during laparoscopic lymphnode biopsy for abdominal malignancy.

    This report concerns massive lymphatic fluid leakage during laparoscopic abdominal lymphnode biopsy for mesenteric tumor of non-Hodgkin's disease. Laparoscopic lymphnode biopsy was performed on a 58-year-old man who presented with a huge abdominal mass. The initial diagnosis was based on abdominal computed tomography, which revealed a large mass. This was followed by laparoscopic abdominal lymphnode biopsy for a definitive diagnosis. During the operation, massive lymphatic leakage up to about 300 mL occurred, but was stopped completely by electric coagulation. Histological examination indicated the mass to be a B-cell type non-Hodgkin's lymphoma. hospitalization was uneventful, and the patient was discharged 7 days postoperatively to the Department of internal medicine for chemotherapy. This new endoscopic approach offers a useful alternative to the traditional transabdominal excision of intra-abdominal lymphnodes, although attention must be paid to possible complications including massive intraoperative lymphatic leakage.

2/13. Large-cell "mixed-phenotype" lymphoma in AIDS. Identification of a CD5-expressing subset of B-cell non-Hodgkin's lymphoma.

    We have characterized cells from an AIDS-associated non-Hodgkin's lymphoma (NHL). We found a malignant CD5 B cell (approximately three-fourths of cells), with the balance of cells comprised of T cells having an activated phenotype. The lymphoma was unusual, in that all cells bear "T-lineage" markers, CD3 and CD5, and "B-lineage" markers, CD19 and CD20. Thus, conventional immunohistologic techniques were insufficient to type this mixture of cells; single-cell analysis was required. These "mixed-phenotype" lymphomas may occur frequently in AIDS-associated NHL. Our results show striking similarity to those obtained in the analysis of certain murine CD5 B-cell lymphomas, suggesting that the study of lymphomagenesis in the mouse may aid in the understanding of AIDS-associated NHL.

3/13. Nodular sclerosing Hodgkin's disease showing positive reaction for anti-IL 6/BSF2 antibody.

    A 53-year-old man presented with bilateral cervical lymph node swelling and low fever. Swollen lymph node was biopsied, and diagnosed as nodular sclerosing Hodgkin's disease (HD). By combined radio- and chemotherapy, he went into complete remission. After 3 years, fever of 38-39.2 degrees C and lymph node swelling appeared in the para-aortic region and later became systemic. A biopsy was diagnosed as syncytial variant of HD with aid of immunohistochemistry on frozen sections. Large atypical mono- or binucleated cells reacted with anti-interleukin 6 (IL-6) antibody. The patient's disease progressed with fever, and he died 66 months after the onset of disease. A previous biochemical study showed that IL-6 displayed a real relation to the generation of fever. Therefore it is suggested that fever in this patient is related to IL-6.

4/13. Non-Hodgkin's lymphoma in the gluteal region: a case report.

    The infiltration of the skeletal muscles on the part of lymphomatous cells is rarely reported and only histological examination can reveal it. Based on a review of the literature, it is the purpose of this study to describe a rare case of non-Hodgkin's lymphoma with muscular invasion, characterized by voluminous swelling in the left gluteal region, in a female aged 60 years who was completely asymptomatic. biopsy and immunohistochemistry alone allowed us to diagnose non-Hodgkin's lymphoma. It is the purpose of this study to document the existence of NHL with musculoskeletal localization, emphasizing the morphological features and the site, as well as hypothesizing that wide-margin surgery can constitute a valid therapeutic aid for such extralymphonodal forms, that are particularly large and with a single localization.

5/13. Nucleolar positivity for CD20: a diagnostic aid in neoplasms of T-cell lineage?

    STUDY DESIGN: During a period of 5 years (1998-2002), 125 lymphoma cases consisting of 78 non-Hodgkin's lymphoma (NHL), 38 Hodgkin's disease (HD) and 9 NHL/HD were diagnosed by fine needle aspiration (FNA) cytology. In FNA smears of NHL, the 2 most commonly used antibodies for immunocytochemical staining were CD3 (29 cases) and CD20 (33 cases). A case was considered positive if > or = 50% of cells showed a positive reaction. RESULTS: Of the 78 NHL cases, 33 stained for CD20. Thirty were successfully stained; of these, 26 were positive. Twenty-nine cases stained for CD3. Twenty-five were successfully stained; of these, 3, consisting of 2 lymphoblastic lymphomas and 1 large cell lymphoma, showed a positive reaction. CD20 staining performed on 1 of these lymphoblastic lymphomas and the large cell lymphoma showed a diffuse cytoplasmic reaction, respectively, in 2% and 0% only but positive staining of the nucleoli in 96% and 84% of cells, respectively. Another lymphoblastic lymphoma case, which did not have an adequate number of CD3-positive cells to label it as of T-cell lineage also showed CD20 positive nucleoli in 60% of cells and a diffuse cytoplasmic reaction for CD 20 in 5% of cells. CONCLUSION: If our observation is corroborated by other studies, CD20 nucleolar positivity may serve as a diagnostic aid in T-cell neoplasms.

6/13. Primary lymphoma of bone: a review of 13 cases emphasizing orthopaedic problems.

    The records of 13 patients treated in the radiotherapy department in Newcastle 1973-1988 for primary lymphoma of bone (PLB) were reviewed. Treatment was by radiotherapy in combination with surgery and/or chemotherapy. Cause-specific disease-free survival at 5 years in 75%. Eight patients had PLB in a weight-bearing bone: four of these patients suffered pathological fractures in the treated bone. The radiological differentiation of recurrent PLB and radiation osteitis remains unsatisfactory. None of the four post-treatment pathological fractures in this series was due to recurrent PLB. None of these fractures healed spontaneously. All four patients remain disease-free but all have restricted mobility and require walking aids. PLB in weight-bearing bones is associated with long-term fragility and propylactic stabilization of the affected bone should be considered.

7/13. Composite Hodgkin's and non-Hodgkin's lymphoma in a patient with acquired immune deficiency syndrome. In-situ demonstration of Epstein-Barr virus.

    A 44-year-old man infected with human immunodeficiency virus had Hodgkin's disease, mixed cellularity, and malignant non-Hodgkin's lymphoma, diffuse large cell type. Colorimetric in-situ hybridization showed the Epstein-Barr virus (EBV) genome in the cells of the large cell non-Hodgkin's lymphoma and in the reed-sternberg cells and reactive lymphocytes of the Hodgkin's lymphoma. These results suggest that EBV may play a similar causative role in both neoplasms. This colorimetric method of hybridization, yielding results within 8 hours, is applicable to archival material and will be useful in further epidemiologic work associating EBV and lymphoid proliferations and malignancies.

8/13. lymphoma in sjogren's syndrome: urinary monoclonal free light chains as a diagnostic aid and a means of tumour monitoring.

    Non Hodgkins lymphoma (NHL) is reported to be at least 40 times more common in sjogren's syndrome (SS). diagnosis may be difficult as blood and bone marrow haematology can remain normal, with no evidence of a serum paraprotein band or Bence-Jones proteinuria by routine electrophoresis. Using the technique of isoelectric focusing in agarose, followed by immunofixation, monoclonal free light chains can be found in the urine of 44% and 74% respectively of patients with NHL and B cell chronic lymphocytic leukaemia, but not in normal individuals. Three patients, two with typical severe primary SS and the third with several features of SS including abnormal sialography and reduced tear secretion, developed B cell NHL of parotid and/or lung. Using the above method on concentrated urine specimens, monoclonal free light chains of the same class as that demonstrated on the tumour cells were found to be present in each patient's urine. In one patient the level of urinary free light chains was monitored and found to correlate with disease activity. Using similar techniques no monoclonal light chains could be found in the urine from a further 10 cases of primary SS and 18 cases of SS secondary to rheumatoid arthritis, all of whom had no clinical evidence of lymphoma. Screening of SS patients' urine by the method described for monoclonal urinary free light chains may provide an early diagnostic clue to the development of lymphoma and be a means of tumour monitoring.

9/13. Computed tomography in the evaluation of lymphoma of the uterine cervix.

    Primary involvement of the uterine cervix is a rare presentation of extranodal lymphoma. The clinical and computed tomography findings are described in four patients ranging from 23 to 74 years of age. All presented with vaginal bleeding. Computed tomography findings were similar to those of other primary pelvic neoplasms, with diffuse uterine enlargement and lobular contour alteration often mimicking fibroids. Lymphadenopathy was not observed in comparison to secondary lymphoma of the female genitalia. All patients had excisional biopsies. One patient had an initial TAH and BSO. histology was varied with one case of large cell lymphoma, one of diffuse histiocytic, one of unclassifiable poorly differentiated lymphoma, and one of nodular poorly differentiated lymphocytic lymphoma that was finally classified as granulocytic sarcoma. Disease extent could be assessed by computed tomography and aided treatment planning. In conclusion, computed tomography was found to be useful in the evaluation of uterine lymphoma. Although the findings are not specific, the local tumor extent can be evaluated at the same time as other sites of potential involvement.

10/13. Primary prostatic involvement in non-Hodgkin lymphoma.

    We report 3 cases of primary extranodal lymphoma of the prostate, an unusual extranodal presentation rarely diagnosed antemortem. Symptoms of prostatism associated with an enlarged hard prostate with pyuria and hematuria in younger patients should suggest the diagnosis. urine cytologic examination should aid in the diagnosis of this condition.
