1/22. Aseptic herpetic meningitis: an uncommon genital herpes sequelae.

    Aseptic herpetic meningitis is a clinical syndrome characterized by fever, headaches, confusion, and a combination of meningeal signs. The spinal fluid findings consist of an increase in mononuclear cells (mononuclear pleocytosis), increased protein concentration, and normal glucose concentrations. Aseptic herpetic meningitis is thought to be caused by a viral infection, although the specific virus is usually not demonstrated. The condition is self-limited and requires no treatment.

2/22. Maternal intrauterine herpes simplex virus infection leading to persistent fetal vasculature.

    herpes simplex virus can cause serious ocular and systemic disease in the neonate. The mode of transmission to the neonate is usually from the maternal birth canal to the fetus intrapartum; but much more rarely, hematogenous transplacental infection can affect the developing fetus months prior to birth. Persistent fetal vasculature occurs when there is persistence of the fetal ocular vasculature, which normally regresses prior to birth. To our knowledge, we report the first case of serologically proven intrauterine herpes simplex virus infection associated with bilateral persistent fetal vasculature in a surviving term infant. 3/22. Findings in a patient with herpes simplex viral meningitis associated with acute retinal necrosis syndrome.

    We report the case of a 41-year-old man with clinical findings of viral meningitis associated with acute retinal necrosis syndrome in his right eye. MR images showed right optic nerve enlargement and high-intensity signal abnormalities in the region of the left lateral geniculate body and the left occipital lobe.

4/22. Recurrent herpes simplex virus type 2 meningitis: a case report of Mollaret's meningitis.

    It is well known that herpes simplex virus (HSV) type 2 produces acute meningitis, while HSV type 2 rarely causes recurrent meningitis (Mollaret's meningitis). We report the history of a 40-year-old patient with recurrent HSV type 2 meningitis (Mollaret's meningitis). The patient had seven episodes of meningeal symptoms within a 7-year period. In the seventh episode, HSV type 2 dna was confirmed by nested polymerase chain reaction (PCR) with the cerebrospinal fluid (CSF). A real-time quantitative PCR study of the first CSF sample detected 2,000 copies of the HSV genome, which rapidly disappeared following treatment with acyclovir. The present case may be the first case of HSV type 2 Mollaret's meningitis to be documented in japan. In our case, HSV serum antibody titers were at low levels during the whole course of the disease. The possible pathophysiology of this case is discussed.

5/22. Low-dose acyclovir for HSV-2 meningitis in a child.

    A 7-y-old girl with genital herpes following sexual abuse presented with dysuria, fever and meningeal signs. acyclovir (15 mg/kg/d for 10 d) was administered for severe genitourinary symptoms. The CSF culture was positive for HSV type 2. Complete resolution of all symptoms demonstrates that, as in adults, HSV-2 meningitis does not require high-dose or prolonged acyclovir therapy.

    A 34-year-old woman presented to hospital with symptoms of meningitis, later confirmed to be due to herpes simplex virus type 2. She developed hydrocephalus on day 2 of her admission. We describe the first case of hydrocephalus associated with herpes simplex type 2 meningitis in an adult.

7/22. meningitis caused by human herpesvirus-6.

    Since the discovery of human herpesvirus-6 (HHV-6) the illnesses associated with it have increased steadily. Two infants with meningitis are reported: both suffered a mild meningitis and serological studies confirmed an acute HHV-6 infection. This report supports a role of HHV-6 in nervous system disease.

8/22. Multiphasic encephalomyelitis in a patient with recurrent herpes simplex type 2 meningitis.

    We here describe a patient with a history of recurrent HSV-2 meningitis who had been free from symptoms for almost 20 y when he developed an acute encephalomyelitis. The clinical course laboratory and radiological findings support an acute multiphasic disseminated encephalomyelitis induced by a recurrent HSV-2 infection.

9/22. Acute urinary retention secondary to herpes simplex meningitis.

    We report a case of acute urinary retention in a 24-year-old man with herpes simplex meningitis without genital lesions. Since the differential diagnosis in young patients who present with acute urinary retention also includes multiple sclerosis, lumbosacral disk herniation, rheumatological disorders and drug intoxication, a thorough history and careful neurological examination are of paramount importance in distinguishing these syndromes. As part of a directed neurological evaluation prompt performance of lumbar puncture is indicated; a lymphocytic pleocytosis is suggestive of herpetic meningitis. culture of herpes simplex virus from the cerebrospinal fluid should be attempted. We recommend conservative management only, typically with intermittent catheterization, since bladder function usually normalizes within 10 to 14 days.

10/22. herpes simplex virus type 2 meningitis.

    To date only 9 culture-proven cases of herpes simplex virus Type 2 meningitis have been recorded. Presented here is the 10th case with a review of the literature. Although generally a benign disease, its recognition in association with herpes progenitalis must be appreciated to avoid confusion in the treatment of this common veneral disease.
