1/13. oral manifestations as a hallmark of malignant acanthosis nigricans.

    We present a case of malignant acanthosis nigricans (AN) that initially manifested in the oral cavity. In the present report, the patient had typical clinical and histological findings of oral and esophageal AN, with subtle skin changes, associated with a gallbladder adenocarcinoma. The importance of the clinical oral examination is emphasized because the recognition of oral lesions led to the diagnosis of AN and to the following detection of the internal malignancy. Since the tumours associated with AN are highly malignant, it is of the utmost importance to recognize the skin and mucous membrane changes in adults.

2/13. Oral acanthosis nigricans as a marker of internal malignancy. A case report.

    BACKGROUND: acanthosis nigricans (AN) is a rare mucocutaneous condition that can involve the oral tissues. There are 2 clinical forms of AN: benign and malignant. Benign AN is related to systemic diseases such as diabetes and obesity or can be induced by drugs such as systemic corticosteroids, nicotinic acid, estrogens, insulin, and fusidic acid. Malignant AN appears in association with tumors such as lung, ovarian, breast, and gastric carcinoma. methods: A rare case of malignant AN that initially manifested in the oral cavity of a 73-year-old patient is reported. RESULTS: A bladder and lung carcinoma were detected following the diagnosis of AN. CONCLUSIONS: The diagnostic importance of oral AN is emphasized because, in our patient, its recognition led to the detection of 2 occult malignant tumors.

3/13. Oral psoriasis in a patient with hepatitis c virus infection.

    We report a case of 65-year-old patient with psoriasis vulgaris who developed a psoriatic manifestation on his lower lip, along with typical features of psoriasis on his trunk and extremities. Mucous membranes, palate and tongue were not affected. A biopsy specimen from lip showed acanthosis of the epidermis with parakeratosis, and mild cellular infiltrates in the upper dermis. He was also suffering from type C hepatitis, however, he had not been treated with interferons. Oral psoriasis involving the lip is extremely rare.

4/13. Oral acanthosis nigricans, tripe palms and sign of leser-trelat in a patient with gastric adenocarcinoma.

    acanthosis nigricans (AN), tripe palms (TP) and the sign of Leser-Trelat (LT) may be seen with the presence of malignancy. acanthosis nigricans may have a mucocutaneous localization involving the oral mucosa with papillomatous and verrucous lesions usually on the lips and buccal mucosa. These paraneoplastic dermatoses are generally linked with intra-abdominal malignancy, most often gastric adenocarcinoma. Improvement of the associated dermatoses after the treatment of the malignancy has been frequently observed. We report the case of a 53-year-old man suffering from advanced gastric adenocarcinoma, in which metastases seemed to sustain all three paraneoplastic dermatoses. To the best of our knowledge this is the first case of a patient showing manifestations of all three paraneoplastic dermatoses. patients presenting with this set of dermatoses should be suspected to harbor an occult malignancy, or have persistence of a known malignancy.

5/13. Considerations on the diagnosis of oral psoriasis: a case report.

    This paper discusses the difficulties in making a definitive diagnosis of oral psoriasis based upon clinical and histological evidence only. A young black male presented with multiple lesions showing erosions, fissures, and yellowish scales on the vermilion borders of both lips. He also had erythematous-erosive areas on the gingivae, a fissured tongue showing greyish areas on its ventral surface, whitish lesions and longitudinal sulci in the hard palate with lacelike lesions on the soft palate. Biopsies from the lower lip, gingiva and soft palate showed hyperkeratosis, spongiosis, acanthosis, and elongation of rete ridges. In addition, collections intraepithelial micro-abscesses of Munro were observed. These findings are consistent with oral psoriasis. Typical cutaneous lesions and a family history of psoriasis were absent.

6/13. Esophageal acanthosis nigricans in association with adenocarcinoma from an unknown primary site.

    A case of acanthosis nigricans with esophageal involvement is presented. Six months after the patient's initial examination, metastatic adenocarcinoma from an unknown primary site was discovered. This represents the ninth reported case of esophageal acanthosis nigricans; six of the eight previously described cases were in association with malignancy localized to the gastrointestinal tract. Esophageal acanthosis nigricans may be a more specific marker for associated malignancy than cutaneous or oral mucosal manifestations. If present, esophageal acanthosis nigricans may serve as an indication for an extensive search for occult malignancy.

7/13. Oral lichenoid eruption secondary to imatinib (Glivec).

    BACKGROUND: Oral lichenoid drug eruption (OLE) is a self-restricted inflammatory condition usually secondary to a variety of drugs and dental materials (mercury, gold), and rarely to immunomodulators and new antineoplastic agents such as imatinib mesylate (Glivec). OLE clinical features are reminiscent of lichen planus and include symmetric and asymptomatic, erythematous, reticulated and painful ulcerated plaques. Histological examination reveals perivascular lymphocytic infiltrate and the presence of eosinophils that obscures the dermal-epidermal junction, with acanthosis and mild hyperkeratosis. methods: A 62-year-old man was treated with Glivec for metastatic gastrointestinal stromal tumour (GIST). RESULTS: The patient presented with grey-violaceous plaques on both cheek mucosal surfaces and the lateral sides of the tongue and labial mucosa, with a reticular pattern resembling oral lichen planus. These lesions appeared approximately 1 year after therapy and partially subsided in about 10 weeks, to clear after topical corticosteroids and oral omeprazole. The diagnosis of OLE induced by imatinib was confirmed by histological biopsy evaluation; immunohistochemical analysis revealed intense immunoreactivity with only cytokeratins 5/6 and 14 in the epithelium of affected mucosa. All laboratory investigations proved asiderotic anaemia but excluded infectious causes. CONCLUSION: It is believed that this patient developed imatinib-induced OLE; this relationship with therapy rather than the underlying disease rules out a paraneoplastic reaction, and negative immunofluorescence excluded autoimmune dermatosis. In addition, we propose that these lesions are correlated with abnormal expression of some cytokeratins directly caused by this drug.

8/13. pemphigus vegetans of the oral cavity.

    BACKGROUND: pemphigus vegetans, a variant of pemphigus vulgaris, constitutes a rare form of all pemphigus cases, and oral involvement is common. Two clinical subtypes of pemphigus vegetans exist, characterized initially by flaccid bullae and erosions (Neumann) or pustules (Hallopeau). Both subtypes subsequently develop into hyperpigmented vegetative plaques with pustules and hypertrophic granulation tissue at the periphery. methods: We report three cases of pemphigus vegetans with oral manifestations exclusively. Two patients were male aged 30 and 45 years old, respectively, while one was a 51-year-old female. CONCLUSION: Oral lesions in all cases consisted of erosions and whitish, vegetating plaques. The histopathological characteristics were in all cases identical. The spinous cell layer was characterized by intense acanthosis and by the presence of vesicles between the spinous and basal cell layers. Inside the vesicles exudative elements were observed consisting mainly of eosinophils. In the upper lamina propria severe inflammatory reaction was observed. streptavidin-biotin immunoperoxidase technique showed in all cases intercellular epithelial deposition of IgG and C3.

9/13. Melanoacanthosis (melanoacanthoma) of the oral mucosa.

    A pigmented lesion of the oral mucosa that bears histologic resemblance to the cutaneous melanoacanthoma is discussed. A study of 22 cases, including 4 in the current series, shows that the intraoral lesion occurs most commonly in adult black women on the buccal mucosa and lip. The lesion is known to regress. The term melanoacanthosis is suggested for this condition.

10/13. Improvement of oral lesions associated with malignant acanthosis nigricans after treatment of lung cancer.

    A rare case of malignant acanthosis nigricans associated with lung cancer is presented. The case involved a 70-year-old man who had oral changes and various immunologic dysfunctions, including an increase in antinuclear antibodies. Oral lesions were characterized by papillary hyperplasia, formation of grooves, pigmentation of the upper lip and cheeks, and a loss of taste sensation. After the patient's lung cancer was treated with anticancer drugs, the histopathologic examination showed a marked improvement in oral lesions and significant normalization of immunologic abnormalities. The present case provides unique evidence that oral lesions associated with acanthosis nigricans have a significant correlation with lung cancer and high titers of antinuclear antibodies.
