1/20. Palpebral myiasis.

    myiasis is most prevalent in mexico, central and south america, tropical africa, and the southwestern united states. Although dermal myiasis is rare in most of the United States, it is a disorder that may be seen in international travelers. In the United States, external myiasis is usually caused by the cattle botfly. We report here a case of ophthalmomyiasis involving the left upper eyelid of a child. We examined a six-year-old boy who presented to the massachusetts eye and ear Infirmary (MEEI) in September 1998. He complained of persistent swelling of his left upper eyelid for the previous ten days. The edema and erythema were unresponsive to warm compresses and oral antibiotics. Ocular examination revealed a mild preseptal cellulitis of the left upper eyelid with a small draining fistula. On slit-lamp examination, we found one larva protruding intermittently from the fistula site. The larva was extracted with forceps, wrapped in a moist towel and sent in a jar to the parasitology laboratory. The specimen was identified as a Cuterebra larva by a parasitologist at the Harvard School of public health. One week later, the patient's eyelid edema and erythema had completely resolved.

2/20. Case report: ophthalmomyiasis externa in Dallas County, texas.

    Ophthalmomyiasis externa is an uncommon condition in north america. If not recognized and managed accordingly, it can be complicated by the potentially fatal condition ophthalmomyiasis interna. Ophthalmomyiasis externa is mainly caused by the sheep bot fly Oestrus ovis; thus, it is more common in farming communities. We report a case of ophthalmomyiasis externa in a young woman from Dallas County, texas, who had no known history of contact with farm animals.

3/20. Internal ophthalmomyiasis presenting as endophthalmitis associated with an intraocular foreign body.

    Ophthalmomyiasis interna infestation by fly larva can present in various forms. A 3-year-old girl with a 15-day history of pain, redness, and tearing of the right eye was referred to our clinic with the diagnosis of endophthalmitis associated with an intraocular foreign body, based on clinical and ultrasonographic findings. The patient underwent pars plana vitrectomy, during which an 8-mm long larva was encountered within the vitreous cavity. It was removed and identified as a cattle botfly. The patient developed a retinal detachment 1 month postoperatively, but the family refused further treatment and the patient was lost to follow-up. Ophthalmomyiasis should be included in the differential diagnosis of endophthalmitis and intraocular foreign bodies in patients from rural areas.

4/20. Intraocular infestation by the reindeer warble fly larva: an unusual indication for acute vitrectomy.

    A case of ophthalmomyiasis interna posterior (OIP) caused by the larva of the reindeer warble fly is reported. The larva was initially found intraretinally in the inferior parts of the fundus. From this position it moved into the macula lutea within two days, causing decreased visual acuity. Acute vitrectomy was performed and the larva was removed through a retinotomy. About two months afterwards a small retinal detachment appeared near the ora serrata, where the larva had entered the eye. The retina was reattached with a scleral buckling procedure. The patient regained most of his vision postoperatively. Pars plana vitrectomy is a safe procedure and we consider that in any case of OIP, with a living larva in the eye, acute vitrectomy should be considered, since the larva may damage vital parts of the eye. Furthermore, the site of entrance should be treated with photocoagulation to prevent retinal detachment.

5/20. Ophthalmomyiasis interna posterior.

    Ophthalmomyiasis interna posterior (OIP) means infection of the posterior part of the eye with the larvae of flies. Few cases have been reported in which the larvae have been removed from the eye and the visual acuity preserved. We report four cases of OIP with living larva in the vitreous cavity. The larvae produced severe uveitis and were removed alive with vitrectomy and a procedure for removing foreign bodies. They were identified as first-stage reindeer warble fly larvae (Oedemagena tarandi). We also observed dead larva in the vitreous cavity of a patient who suffered severe uveitis in 1943, and this eye survived with normal visual acuity. Another patient had a blind eye enucleated and the remnants of a larva were found inside the eye. All patients live in scandinavia.

6/20. Ophthalmomyiasis interna posterior. Subretinal tracks and intraocular larvae.

    Only seven cases of identifiable intraocular botfly larva infestation have been reported in north america--five since 1969. Two additional cases, both from the same institution, are documented here. The offending maggot or larva was visible in both instances. The findings in each case were diagnostically and morphologically striking. The characteristic funduscopic finding of subretinal tracks, along with the appearance of the larva--encysted in one patient and free-floating in the other-deserves emphasis, since this condition is not always benign and may not be as rare as previously assumed.

    With international travel increasing to more rural areas, physicians must be aware of the accidental ingestion of fly ova from contaminated food, which results in the fecal passage of these nondigestible eggs. The purpose of this report is to review this entity.

8/20. Posterior internal ophthalmomyiasis. Identification of a surgically removed Cuterebra larva by scanning electron microscopy.

    The clinical presentation of an 11-year-old boy with unilateral posterior internal ophthalmomyiasis is described. ophthalmoscopy of the affected eye in this individual disclosed characteristic subretinal depigmented linear tracks and a larva within the vitreous. The intravitreal larva was retrieved successfully by pars plana vitrectomy and identified as the first instar of Cuterebra sp. (rodent botfly) by scanning electron microscopy. This was done by examining the external features of the larva by scanning electron microscopy and comparing them to scanning electron micrographs that were prepared on other first instar larvae of flies known to be implicated in internal ophthalmomyiasis. The study demonstrates the usefulness of scanning electron microscopy for taxonomic identification of fly larvae. Since even a small fragment of these organisms may contain characteristic features of external morphology, scanning electron microscopy may be useful in cases where the specimen is damaged or incomplete.

9/20. Ophthalmomyiasis interna causing visual loss.

    Ophthalmomyiasis interna caused severe intraocular inflammation and loss of vision in two eyes. In the first eye, the organism was found in the vitreous and created a severe uveitis; a second-stage larva of Hypoderma lineatum was later removed from the anterior chamber. Phthisis bulbi ensued with loss of all vision. In a second eye, a subretinal maggot was observed to produce tracks in the pigment epithelium, with subretinal and vitreous hemorrhage. Severe uveitis and traction retinal detachment later developed. Despite successful reattachment of the retina, visual acuity remained only light perception. These cases demonstrate that ophthalmomyiasis interna is not always a benign condition.

10/20. Ophthalmomyiasis interna.

    A 15-year-old boy was seen initially with decreased vision in his left eye. Ophthalmoscopic examination showed numerous pigmented subretinal tracts with traction lines radiating to the fovea. Peripheral examination disclosed a maggot in the superior nasal quadrant of the vitreous. There was a hemorrhage emerging from the exit wound in the retina. Removal of the maggot was not indicated because there was no inflammation.
