1/5. Removal of a retained Herbert-Whipple screw with use of the Osteochondral Autograft Transfer System (OATS) core harvester: a case report.

    After a revision procedure is performed to address a full-thickness chondral defect of the knee, hardware removal can be challenging. We describe using a core harvester, the Osteochondral Autograft Transfer System (OATS; Arthrex, Naples, Fla), to remove a Herbert-Whipple screw placed earlier in a failed attempt to fix an osteochondritis dissecans lesion on the lateral aspect of the medial femoral condyle. This hardware removal technique can be performed either arthroscopically or in an open fashion and may also be used with other revision procedures (ie, revision anterior cruciate ligament reconstruction) in which an embedded, poorly visualized, or stripped screw must be removed.

2/5. Aplasia of the anterior cruciate ligament with a compensating posterior cruciate ligament.

    Aplasia of the anterior cruciate ligament is a rare condition and is usually associated with other abnormalities of the lower extremities. We report aplasia of the anterior cruciate ligament with a compensating posterior cruciate ligament in a 15-year-old boy.

3/5. Talar dome fracture repaired using bioabsorbable fixation.

    We describe a man with an acute osteochondral defect in the lateral talar dome associated with a supination-adduction-type ankle fracture. The osteochondral defect was readily visible on plain film radiographs, and magnetic resonance imaging was ordered to determine the full extent of soft-tissue and articular injury. It was discovered that the patient had a stage IV lesion of the talar dome, with complete inversion of the fragment, and rupture of the anterior talofibular and calcaneofibular lateral ankle ligaments. Furthermore, the patient experienced an oblique fracture of the medial malleolus with comminution. The talar dome lesion was surgically reduced and fixated using bioabsorbable pins. Nine months after surgery, the patient was fully recovered from his injury and had no functional limitations.

4/5. osteochondritis dissecans of the anterior tibial spine.

    A 12-year-old boy had a history of pain in the right knee for 2 months, mainly pain in extension. He could not recall any trauma to the knee. The radiograph suggested osteochondritis dissecans in the anterior spine. An arthroscopy was performed and a prominence in the tibial insertion of the anterior cruciate ligament in the anterointernal area was observed, and the fragment resected. The diagnosis of osteochondritis dissecans was confirmed by pathological study. As far as we are aware, this is the first case describing this new location.

    We report a patient with osteochondritis dissecans of the medial femoral condyle associated with congenital hypoplasia of the lateral meniscus and anterior cruciate ligament. This is the first report of such a case.
