1/8. Palatal radicular multigrooves associated with severe periodontal defects in maxillary central incisors.

    BACKGROUND: This case report describes a rare condition of palatal radicular multigrooves on teeth 11 and 21 with severe periodontal defects and the findings at 3-year follow-up. METHOD: Radiculoplasty using hand curettes and rotary burs were used to remove the multigrooves on the root surfaces and change the wrinkled root form to the relatively flat and smooth normal root morphology. Minor tooth movement and frenotomy were performed for a diastema between teeth 11 and 21. Supportive periodontal therapy started immediately after completion of the active treatment. RESULTS: Improved healthy periodontal tissues and adequate plaque control have been maintained.

2/8. Pathologic migration--spontaneous correction following periodontal therapy: a case report.

    Periodontal disease is often associated with pathologic migration, which becomes an esthetic concern. A 17-year-old girl developed increasing gaps among her maxillary incisors. She had gingival enlargement in the palatal maxillary anterior region. The central incisors had pathologically migrated, resulting in a 2-mm diastema. Periodontal treatment was planned and completed. Following periodontal treatment, there was "spontaneous" repositioning of the central incisors. The 6-month follow-up revealed no change or deterioration of the periodontal condition. The patient was referred for orthodontic closure of the remaining diastema between the central and lateral incisors.

3/8. The use of orthodontic intrusive movement to reduce infrabony pockets in adult periodontal patients: a case report.

    Clinicians often encounter osseous defects that are best treated by conventional surgical techniques, including bone grafting and guided tissue regeneration, with a goal of establishing a new connective tissue attachment. On occasion, the recognition of an infrabony defect proximal to a tooth with a large diastema may present an opportunity to consider resolution by orthodontic tooth movement. Ideally, the tooth could be moved in the proximal direction until there was no further radiographic or clinical evidence of the predisposing defect. The authors decided to treat an advanced case of adult periodontitis, with extrusion and migration of a maxillary central incisor, using a multidisciplinary approach. Radiologically, a large infrabony defect was present on the mesial aspect of the incisor, with an initial probing depth of 9 mm. After the surgical periodontal therapy, the orthodontic movement started and the incisor was repositioned using an intrusive mechanism, also leading to the closure of the diastema. At the end of the treatment, there was a significant clinical decrease in the probing depth values, and radiographs showed a remarkable reduction of the infrabony defect volume.

4/8. An interdisciplinary approach for improved functional and esthetic results in a periodontally compromised adult patient.

    In contemporary dental care, an increasing number of adult patients are seeking orthodontic treatment. In such adult patients, a combined orthodontic and other specialized therapy often offers the best option for achieving a predictable outcome to solve complex clinical problems. This case report demonstrates a combined therapy with orthodontic, periodontic, and implant-prosthodontic treatments in a 56-year six-month-old female patient with mild diastemata in the maxillary anterior region and a missing left maxillary second premolar caused by a periodontal disease with medium bone loss. The patient had improved her oral hygiene condition through periodontal treatment before orthodontic treatment. The patient was orthodontically treated with a maxillary lingual arch and a maxillary edgewise orthodontic appliance. Active orthodontic treatment was completed in 18 months, and an implant-supported prosthesis was placed with a single crown in the region of the left maxillary second premolar. The treatment outcomes, including the periodontal condition and the dental implant treatment, were stable at two years after the active orthodontic treatment. We demonstrate that combined orthodontic-periodontic-implant-prosthodontic treatment can achieve an improved masticatory function, esthetics, occlusion, and periodontal condition.

5/8. Combined periodontal, orthodontic, and restorative treatment of pathologic migration of anterior teeth: a case report.

    Pathologic tooth migration related to periodontal disease is a common chief complaint of periodontal patients. This paper describes the treatment of a case of severe periodontal disease and disfiguring pathologic migration of maxillary central incisors, which required a multidisciplinary approach. After conventional periodontal treatment was performed, the anterior diastema was closed using a combination of orthodontic therapy and restorative treatment. A 6-month follow-up examination of this case revealed resolution of the anterior pathologic migration, with gains in clinical attachment levels and a successful esthetic and functional final result.

6/8. Severe localised periodontitis in an adult: initiation by orthodontic rubber bands.

    An adult patient presented with advanced periodontitis around teeth 11 and 21. This was found to have been caused by the patient placing orthodontic bands 1 year previously around these teeth to close a mid-line diastema.

7/8. Spontaneous correction of pathologic migration following periodontal therapy.

    Two case reports illustrate spontaneous correction of pathologic migration following periodontal therapy. In one case, the displaced teeth were repositioned without orthodontic appliances after nonsurgical and surgical periodontal treatment. In the other case, closure of a diastema occurred following only nonsurgical therapy. The predictability of these findings needs further investigation. A discussion of possible reasons for this type of tooth movement is included.

8/8. Severe bone loss caused by orthodontic rubber bands; management and nine-year follow-up: report of case.

    This case reports the early diagnosis and treatment of an iatrogenic acute localized periodontitis arising from the incorrect use of orthodontic elastic rubber bands to close a maxillary midline diastema in an eight-year-old girl. The case illustrates the misdiagnosis and mismanagement of a transitional diastema in the "ugly duckling" stage of the mixed dentition. The nine-year follow-up shows that thorough periodontal and orthodontic management has allowed successful retention of both maxillary permanent central incisors, which had initially pathological grade 3 vertical mobility, little remaining alveolar bone, and a hopeless prognosis.
