1/6. 15-HETE is the main eicosanoid present in mucus of ulcerative proctocolitis.

    prostaglandins, leukotrienes and mono-hydroxy acid products of arachidonic acid were measured in mucus of freshly recovered morning stools of a patient with an exacerbation of ulcerative proctocolitis. eicosanoids in ether extracts were separated by high performance liquid chromatography and amounts determined by radioimmunoassay. Four hydroxy-eicosatetraenoic acids were detected, of which the most important one was identified as 15-hydroxy eicosatetraenoic acid (530 ng/g mucus). leukotriene b4 was also present (21 ng/g mucus) and small amounts of immunoreactive leukotriene c4 (less than 0.8 ng/g mucus). The prostaglandins 6-keto-PGF1 alpha and PGE2 and thromboxane b2 were found in amounts of 3.7, 2.0 and 9.2 ng/g mucus, respectively.

2/6. Eosinophilic proctitis with giant cells: a manifestation of cow's milk protein intolerance.

    Two infants, subsequently shown to have cow's milk protein intolerance, presented with rectal bleeding in the first week of life. Rectal biopsies showed eosinophilic infiltration of the mucosa with several multinucleated giant cells. IgG, IgM, and IgA containing plasma cells were present but IgE could not be detected in rectal biopsies. While cow's milk protein intolerance is a well-recognized cause of proctocolitis in infancy, eosinophilic granulomatous-like lesions with giant cells may be a prominent feature and do not exclude the diagnosis.

4/6. Cancer of the colon as a late sequel of pelvic irradiation.

    A patient, who developed carcinoma of the rectum nine years following therapeutic radiation for pelvic malignancy is presented. The relationship of radiation to carcinoma of the colon is reviewed. This relationship makes rigorous follow-up of patients with radiation induced chronic proctocolitis essential.

5/6. Simultaneous onset of ulcerative colitis in the rectum and in a segment of colon used for colpopoiesis: report of a case.

    A patient is described in whom a segment of colon used to construct an artificial vagina was therefore removed from intestinal continuity and was affected by typical idiopathic ulcerative colitis. The onset of disease was simultaneous in this loop of colon and in the rectum. There was no evidence of local infection. An increased ratio of IgE-plasma cells was found in both the mucosa of the grafted colon and that of the rectum, but anticolonic antibodies were not detected in the serum. After removal of the loop of colon used for colpopoiesis, the patient continued to suffer from distal proctocolitis for almost a year, but has since then maintained a stable remission without treatment. The etiology of ulcerative colitis is discussed, and attention is drawn to the unusual psychologic aspects of this case.

6/6. Transanal anastomosis in the treatment of low rectal strictures due to tribal enemas.

    enema administration is common in Black tribal communities. In the urban environment, toxic and corrosive ingredients may be used which can lead to severe complications. The diagnosis and principles of treatment of complicated proctocolitis of enema origin are discussed. The management of 2 patients who developed long stricutres extending low into the rectum is described. After subtotal rectal and sigmoid resection, continuity was successfully restored using a transanal sleeve anastomosis. Subsequent continence was normal in both patients.
