1/44. Unusual manifestations of soft tissue metastasis.

    We present a patient with pulmonary adenocarcinoma metastatic to the left forearm. The magnetic resonance imaging (MRI) revealed no discrete tumor, but an infiltrative signal change of the subcutaneous tissue and muscle. The lymphatic invasion by cancer cells explains this unusual manifestation of image findings.

2/44. Prostatic carcinosarcoma: a case report and review of literature.

    A unique case of carcinosarcoma of the prostate detected in a 71-year-old man is presented. pelvic exenteration was performed, and the resected prostatic mass was found to consist of two histologically distinct elements; adenocarcinoma and sarcoma with focal osteosarcomatous element. The patient is still alive with neither metastasis nor recurrence. This is the 42nd case of carcinosarcoma of the prostate to be reported in the literature.

3/44. An unusual malignancy in a young female; urachal adenocarcinoma.

    Urachal adenocarcinoma is a rare tumour that predominantly affects men between the ages of 50-60; it presents late and is therefore associated with a poor prognosis. We discuss the presentation, investigation, treatment and prognosis of this uncommon tumour.

4/44. A rare case of metastatic ovarian carcinoma originating from primary intrahepatic cholangiocarcinoma. Case report.

    BACKGROUND: A rare case of metastatic ovarian carcinoma arising from intrahepatic cholangiocarcinoma is reported and the literature reviewed. CASE: A 49-year-old woman presented with ascites and a left pelvic mass. Optimal debulking surgery was carried out including a segmental resection of segment 5/6 of the liver. Histopathology confirmed an intrahepatic cholangiocarcinoma metastatic to the ovaries and omentum. CONCLUSION: Distinguishing a metastatic tumor from a primary ovarian tumoris critical for appropriate management. A high index of suspicion intraoperatively and subsequent expert pathological review are essential in making the correct diagnosis.

5/44. Extension of adrenocortical carcinoma into the right atrium--echocardiographic diagnosis: a case report.

    BACKGROUND: adrenocortical carcinoma is a rare, highly malignant tumor. Cardiac involvement of the tumor is very rare. echocardiography facilitates the evaluation of the cardiac involvement of the tumor. CASE PRESENTATION: We describe a patient with an adrenal tumor. Transthoracic echo showed its extension into the right atrium. Accordingly, a combined abdominal and cardiac operation was performed, monitored by transesophageal echocardiography. CONCLUSION: This case highlights the importance of echocardiography in revealing the cardiac involvement by this tumor and in planning the operative procedure.

6/44. Salivary duct carcinoma of the larynx: report of a rare case.

    Salivary duct carcinomas are primarily high-grade, aggressive malignancies that affect men in the fifth and sixth decades of life. These tumors are usually found in the major salivary glands; rarely do they originate in the minor salivary glands. The distinctive feature of these neoplasms is their remarkable histologic resemblance to infiltrating ductal carcinomas of the mammary gland; both types of tumor feature epithelial and myoepithelial cells arrayed in solid, papillary, and cribriform patterns. To the best of our knowledge, only one case of a primary salivary duct carcinoma of the larynx has been previously reported. In this article, we describe a new case, and we review the literature on salivary duct carcinomas.

7/44. Basaloid carcinoma of the lung presenting concurrently with cutaneous metastasis.

    Basaloid carcinoma of the lung is a rarely occurring form of lung cancer morphologically resembling the basal cell carcinoma of the skin. Although various histologic types of lung cancers have metastasized to the skin, basaloid carcinoma of the lung that shows cutaneous metastasis has yet to be reported. A 48-year-old man was admitted to our pulmonology department with mild dyspnea and a solitary cutaneous mass on the left portion of his chin. Chest radiography and computed tomography of the thorax revealed an irregular infiltrative mass confined to the central portion of the left lower lobe bronchus, which is a compatible finding of primary lung cancer. Microscopically, both a cutaneous mass and fine-needle aspirate material in the lung revealed the identical findings of basaloid morphologic patterns.

8/44. Paraneoplastic pemphigus in association with hepatocellular carcinoma.

    Paraneoplastic pemphigus (PNP) is an autoimmune mucocutaneous blistering disease associated with neoplasms, most frequently of the lymphoproliferative type. Rare PNP cases related to nonhematological solid tumors have been reported. The patient in this report presented with severe mucocutaneous involvement of PNP associated with hepatocellular carcinoma. Histopathology showed vacuolar interface dermatitis with keratinocyte necrosis and intraepidermal acantholysis. Direct immunofluorescence exhibited deposition of intercellular IgG and complement and granular complement at the dermoepidermal junction. Indirect immunofluorescence testing showed a typical intercellular staining on monkey esophagus and rat bladder epithelium. immunoprecipitation showed characteristic target antigens of 250, 210, and 190 kDa molecular weights. This patient met all diagnostic criteria for paraneoplastic pemphigus and is, to our knowledge, the first report of a case associated with hepatocellular carcinoma.

9/44. Massive zosteriform cutaneous metastasis from rectal carcinoma.

    A 44-year-old man presented with a large and rapidly growing skin lesion approximately six months after resection of a rectal carcinoma. The lesion measured 40 cm in size, extended from the suprapubic area to the proximal half of the left groin, and showed a particular zosteriform aspect. biopsy confirmed a metastatic skin adenocarcinoma. Cutaneous metastases from rectal cancer are very uncommon. Their gross appearance is not distinctive, although the skin tumors are usually solid, small (less than 5 cm) and painless nodules or papules. Early biopsies for suspicious skin lesions are needed in patients with a history of colorectal cancer.

10/44. Situs ambiguous.

    The case of a female child is described who presented with recurrent respiratory infections and "recurrent right subphrenic abscess." Detailed radiological work-up identified situs ambiguous abdominis with bronchiectasis, duodenal malrotation, umbilical hernia and spina bifida.
