1/4. Mycotic pseudoaneurysm of common carotid artery mimicking parapharyngeal abscess.

    In this case a secondarily infected pseudoaneurysm of the common carotid artery presented with clinical features suggestive of a parapharyngeal abscess. The causative organism was identified as community-acquired methicillin-resistant staphylococcus aureus. To the authors' knowledge this condition not previously been reported.

2/4. Right internal carotid pseudoaneurysm mimicking a retropharyngeal abscess in a child.

    A 7-year-old girl with tonsillar infection with antibiotics. Two weeks later, there was a right sided neck lump. Computed tomography scans demonstrated a predominantly hypodense right retropharyngeal area with peripheral enhancement and mass effect. There was intense enhancement within the postero-superior aspect of the lesion which was continuous with the right internal carotid artery. Ultrasound demonstrated tapering of the right internal carotid artery. magnetic resonance imaging and magnetic resonance arteriography showed a right internal carotid artery pseudoaneurysm. Surgical exploration confirmed the finding. This case highlights an unusual presentation of an internal carotid pseudoaneurysm and how imaging provided the diagnosis.

3/4. Pseudoaneurysm of the internal carotid artery in an infant aged 8 months.

    Pseudoaneurysm of the carotid artery is a potentially life-threatening complication of deep neck space infection. It clinically manifests by pulsatile neck swelling, lower cranial nerve palsy, and Horner's syndrome. There may be sentinel bleed from nose or oral cavity before the massive haemorrhage, from carotid artery rupture. We present a case of pseudoaneurysm of the internal carotid artery (ICA) following parapharyngeal and retropharyngeal space infection in an 8-month-old infant. Emergency carotid artery ligation was performed and the infant recovered well without any neurological sequelae.

4/4. ligation of the common carotid artery for the management of a mycotic pseudoaneurysm of an extracranial internal carotid artery. A case report and review of the literature.

    Although vascular involvement by deep neck space infections occurs very rarely today with the widespread use of antibiotics, they often pose a significant challenge to the modern otolaryngologist, who most likely will have had no previous experience with either the diagnosis or treatment of these potentially life-threatening infections. We describe the case of a young female presenting with fevers, dysphagia, and blood-tinged sputum, who was diagnosed by contrast-enhanced computerized tomography, to possess a mycotic pseudoaneurysm of her right extracranial internal carotid artery, for which ligation of her common carotid artery was required. We also discuss both the clinical findings which should lead one to suspect that a neck infection may be involving the extracranial carotid arteries, and the English literature, on the morbidity and mortality of ligating a common carotid artery.
