1/11. Staphylococcal pyomyositis in a patient with non-Hodgkin's lymphoma.

    pyomyositis is a rare disease, encountered mainly in tropical climates. The diagnosis of this entity is difficult, if not misdiagnosed, because of its rarity and its subacute presentation. We report of a 42-year-old man, in whom pyomyositis developed while he was receiving the standard chemotherapy for T-cell non-Hodgkin's lymphoma (NHL). Three months following splenectomy, multiple abscesses occurred in the muscles of both thighs while the patient was receiving the third course of the CHOP regimen. A purulent exudate was aspirated from the abscesses under computed tomographic guidance. coagulase-positive Staphylococcus aureus was cultured in the aspirate. pyomyositis was completely resolved following the surgical drainage and the antistaphylococcal antibiotic treatment. This patient has shown that immunosuppression due to splenectomy, NHL, and chemotherapy, especially when using steroids, could be risk factors for pyomyositis in nontropical or semitropical countries.

2/11. Laryngopyocele: three new clinical cases and review of the literature.

    Laryngopyocele is a fairly rare disease. It is a complication of laryngocele. Its clinical picture is often alarming; thus it needs fast differential diagnosis. Computed tomography allows early diagnosis. Surgical treatment permits a complete recovery. An association between laryngocele and carcinoma of the larynx has been reported by some authors. Three new cases of laryngopyocele are presented and discussed.

3/11. Pyogenic thyroiditis and hiv infection.

    Infective thyroiditis remain rare disease entity. Six patients were seen in Jos, nigeria in the last five years. All of them were hiv positive. The presentation and management of these patients are high-lighted. The clinical features and bacteriology were similar to those previously documented for infective thyroiditis.

4/11. Mycotic pseudoaneurysm of the ascending aorta after mediastinitis in an infant.

    Mycotic pseudoaneurysm of the aorta is a rare disease in children. We report our experience with a postoperative mycotic pseudoaneurysm of the ascending aorta secondary to mediastinitis after a modified fontan procedure. The pseudoaneurysm was successfully repaired using an autologous pericardial patch through a right thoracotomy under total circulatory arrest. During 6 months of postoperative follow-up, there were no recurrences of pseudoaneurysm formation.

6/11. Community-acquired methicillin-resistant staphylococcus aureus pyomyositis in an intravenous drug user.

    pyomyositis is a disease of abscess formation deep within large striated muscles. Outside of the tropics it is a rare disease which occurs mainly in certain patient populations such as the immunosuppressed or intravenous drug users (IDUs). A case of community-acquired methicillin-resistant staphylococcus aureus (CA-MRSA) pyomyositis in an IDU is described. The incidence of both CA-MRSA and pyomyositis is currently increasing. To the authors' knowledge this is the first reported case of CA-MRSA pyomyositis in the UK. Cases of CA-MRSA pyomyositis are likely to increase and it may be necessary to empirically treat certain patients with glycopeptides.

7/11. pyomyositis tropicans: a diagnostic dilemma.

    pyomyositis tropicans is a rare disease in non-tropical climates and thus presents diagnostic difficulties. Two children with single staphylococcal psoas muscle abscesses were recently successfully treated. Computerized axial tomography was found to be a useful diagnostic aid, allowing exact localization of the lesion. The diagnosis and therapy of these abscesses are discussed.

8/11. General peritonitis induced by rectal examination: an unusual complication of primary psoas abscess.

    Primary psoas abscess is a relatively rare disease with highest incidence in children and adolescents. It usually presents with fever, abdominal pain and limp. Limping tends to incriminate musculoskeletal problems below the pelvis, but movement of the hip involves the psoas muscle which is mostly retroperitoneal and intimately related to pelvic and intraperitoneal organs. Although the current tendency is to use abdominal sonography, rectal examination is still a valuable step in clinical examination, and may help to assess a pelvic mass or abscess. The following case report describes the elusive nature of psoas abscess and a rare occurrence of abscess rupture and peritonitis, immediately after rectal examination.

9/11. pyomyositis during induction chemotherapy for acute lymphocytic leukemia.

    PURPOSE: pyomyositis is a rare disease in temperate climate regions and frequently has a subacute presentation. Because of this, the entity is often misdiagnosed. patients AND methods: Two boys with acute lymphocytic leukemia (ALL) who presented with muscle pain, shortly after receiving induction chemotherapy, were evaluated. RESULTS: Presenting physical examination and laboratory findings were unremarkable except for extremity pain and tenderness. These symptoms were initially attributed to a neurotoxic side effect of vincristine. As the children's symptoms progressed, muscle abscess formation was finally delineated by gallium and computed tomography scans, and the diagnosis of pyomyositis was made. In both cases, the invading organism was Staphylococcus aureus. Both children responded well to incision and drainage of the abscesses and antibiotic therapy. CONCLUSION: Four cases of pyomyositis occurring in ALL patients shortly after induction chemotherapy have now been described. We feel that when children from this population present with muscle pain, pyomyositis should be part of the differential diagnosis. With early medical and surgical intervention, morbidity and mortality can be avoided.

10/11. Ruptured cervical aneurysm of the carotid artery--case report of a rare disease.

    rupture of a cervical carotid artery aneurysm is a rare but life-threatening event. The diagnostic evaluation and therapeutic management of a 55-year-old woman with such a lesion are described. In this case, the aneurysmal rupture was complicated by localized Staphylococcus aureus infection confirmed by bacteriologic culture of excised tissue, septicemia, and prevertebral and intraspinal abscess. Definitive diagnosis was a ruptured mycotic aneurysm of the extracranial carotid artery. The origin of the infection was probably the patient's previously infected vascular prosthesis for hemodialysis.
