1/199. A case of fibrosing mediastinitis with obstruction of superior vena cava and downhill esophageal varices: a rare cause of upper gastrointestinal hemorrhage.

    Fibrosing mediastinitis (FM) is an excessive fibrotic reaction that occurs in the mediastinum and may lead to compression of mediastinal structures (especially vascular or bronchial). In the present study we describe the first case report of FM, in a patient who developed downhill esophageal varices and bleeding, which was secondary to superior vena cava obstruction.

2/199. Pacemaker-induced superior vena cava obstruction: bypass using the intact azygous vein.

    Superior vena cava thrombosis due to pacemaker leads is an uncommon but well-recognized complication. Its pathogenesis remains unclear and it is usually a benign condition. Superior vena cava occlusion can be successfully treated by thrombolysis and anticoagulation if the occlusion is recent, balloon venoplasty and stenting, and surgery. We describe a case of superior vena cava obstruction successfully bypassed using the intact native azygous vein, a technique that has not been described before, with excellent long-term results.

3/199. Use of a metallic stent for relief of symptoms caused by superior vena caval obstruction in a patient with advanced cancer: a case report.

    This report describes the insertion of a metallic stent in the superior vena cava to relieve the symptoms of malignant superior vena caval obstruction in a 75-year-old woman with far-advanced lung cancer in whom other methods of symptom control had been ineffective. Her symptoms were quickly relieved by insertion of the stent. She died 1 month following the procedure, without recurrence of the symptoms. The technical aspects of the procedure and the issues affecting the clinical decision-making process in this case are discussed.

4/199. The ectopic ACTH syndrome and superior vena cava obstruction due to a malignant carcinoid tumour of the thymus.

    We report the case of a 28-year-old African Caribbean woman with Cushing's syndrome and superior vena cava obstruction secondary to an ACTH-secreting carcinoid tumour of the thymus. The case highlights the problems which may be encountered in performing the 2-day high dose dexamethasone suppression test but clinicians are reminded that this test or any other dynamic test is absolutely essential for elucidating the cause of ACTH-dependent Cushing's Syndrome.

5/199. Surgical treatment of an endovascular metastatic melanoma of the superior vena cava.

    A 42-year-old woman, who had undergone excision of a melanoma of her right forearm 3 years before (Breslow's index 4.4 mm), was admitted to hospital for the evaluation of an superior vena cava syndrome. The thoracic CT scan and the phlebography showed obstruction of the superior vena cava by an endovascular tumor. Abdominal, pelvis and cranial CT scans did not reveal any other metastatic localization. Surgery with extracorporeal circulation was performed. The mass was resected and histopathologic examination confirmed the endovascular metastatic melanoma. There was no heart metastasis. The patient was then given a polychemotherapy. She was still alive after 18 months of follow-up after the initial metastasis. To our knowledge, no similar case has previously been reported and surgical treatment, as for isolated heart metastatic melanoma, may be considered for vascular metastasis, as in our case.

6/199. Superior vena cava obstruction in prostate cancer.

    This report describes the case history of a man with adenocarcinoma of the prostate. After an initial response to maximal androgen blockade, he developed massive mediastinal and cervical lymphadenopathy, causing left recurrent laryngeal nerve palsy and superior vena cava obstruction. biopsy confirmed metastatic prostate cancer and he responded well to local radiotherapy. The hormonal treatment of advanced prostate cancer is discussed.

7/199. Systemic to pulmonary venous communication (right-to-left shunt) in superior vena cava obstruction demonstrated by spiral CT.

    An unusual case of systemic vein to pulmonary vein communication in superior vena cava obstruction is reported. This was a right-to-left shunt, demonstrated by spiral CT and aided by three-dimensional reconstruction. The pulmonary venous shunts were mainly seen in fibro-atelectatic lung where prominent bridging veins were concentrated.

8/199. coronary artery bypass and superior vena cava syndrome.

    superior vena cava syndrome is the obstruction of the superior vena cava or its main tributaries by benign or malignant lesions. The syndrome causes edema and engorgement of the vessels on the face, neck, and arms, nonproductive cough, and dyspnea. We discuss the case of a 48-year-old obese diabetic woman who was admitted with unstable angina. She had previously been diagnosed with superior vena cava syndrome. Urgent coronary artery bypass grafting was necessary Although thousands of coronary artery bypasses are performed every year, there are not many reports on patients with superior vena cava syndrome who successfully undergo cardiopulmonary bypass and coronary artery grafting with an internal mammary artery as the conduit. The results of the case and alternative recommended methods are discussed.

9/199. Surgical management of extensive lipomatous hypertrophy of the right atrium.

    Lipomatous hypertrophy of the right atrium is a rare lesion that is usually limited to the interatrial septum. The authors report on a patient that suffered from an extensive lipomatous hypertrophy, which protruded into the right and left atrium as well as the superior vena cava and caused inflow obstruction. Therapeutic management is discussed.

10/199. Superior vena cava obstruction caused by radiation induced venous fibrosis.

    superior vena cava syndrome is most often caused by lung carcinoma. Two cases are described in whom venous obstruction in the superior mediastinum was caused by local vascular fibrosis due to radiotherapy five and seven years earlier. The development of radiation injury to greater vessels is discussed, together with the possibilities for treatment of superior vena cava syndrome.
