1/27. Genital cutaneous crohn disease: two cases with unusual clinical and histopathologic features in young men.

    Cutaneous crohn disease, sometimes called metastatic crohn disease or crohn disease with cutaneous involvement, is a rare complication of crohn disease in which granulomatous lesions involve skin separated from gastrointestinal lesions by normal tissue. We report two cases of cutaneous crohn disease presenting in young males with erythematous, nontender swelling of the scrotum. One of the young males presented erythematous, nontender swelling of the penis as well. In one case, cutaneous crohn disease represented the primary presentation. The original biopsy in this case showed unusual areas of degeneration of dermal connective tissue forming cystic cavities. The diagnostic biopsies in both cases showed sarcoidal granulomas with an associated superficial and deep perivascular mixed infiltrate including eosinophils. On endoscopy, both patients showed lesions of active crohn disease in the colon. Because changes that would suggest cutaneous crohn disease may not be present on the initial biopsy, unusual presentations and negative cultures may warrant a second biopsy. A high index of suspicion and open communication with the clinician are essential to diagnose this disease.

2/27. Cutaneous sarcoidosis presenting as a testicular mass.

    Urologic involvement in sarcoidosis is rare. We report a patient who presented with a testicular mass and, shortly thereafter, manifested nasal lesions of the papular variant of cutaneous sarcoidosis. Histologic examination from both organ systems yielded sarcoidal granulomas. The patient was successfully treated using oral corticosteroids. sarcoidosis continues to replace syphilis as "the great imitator of the 1990s," and clinicians must be increasingly aware of its variable presentations.

3/27. Testicular sarcoid.

    sarcoidosis occurs rarely in Caucasian males and seldom involves the testis. We report an unusual case of asymptomatic testicular sarcoid in a Caucasian man.

4/27. sarcoidosis with bilateral epididymal and testicular lesions.

    We report a case of genital sarcoidosis that presented characteristic features on MRI. A 25-year-old man sarcoidosis patient with ocular and lung lesions presented a painful mass in the left scrotum together with systemic symptoms of fever, appetite loss, headache, and stomach-ache during the tapering of steroids. The patient was hypercalcemic, and this was thought to be the cause of his systemic symptoms. MRI showed multiple nodules of bilateral testes and enlargement of bilateral epididymis; the patient was diagnosed with testicular and epididymal lesions of sarcoidosis. An increased steroid dosage improved his hypercalcemia and genital lesions.

5/27. The association of germ cell tumours of the testis with sarcoid-like processes.

    Sarcoid-type pulmonary lymphadenopathy associated with testicular cancer is a rare condition which has been previously reported in only 14 cases. Earlier case reports have failed to distinguish between generalized sarcoidosis as opposed to a local granulomatous reaction to tumour. We describe a further 8 cases of the association and provide strong supportive evidence for systemic sarcoidosis in 5 of our patients. In 3 of our patients with systemic sarcoidosis there was coexisting testicular cancer requiring additional treatment. We therefore advise caution in the interpretation of the clinical and histological findings in these patients, and recommend thorough investigation of all such cases.

6/27. Bilateral epididymal sarcoidosis. Case report.

    A 59-year-old man presented with a four-month history of almost painless bilateral scrotal swellings. Histological examination of a biopsy specimen confirmed sarcoidosis. He was well two months later, the mass had not increased, and he was receiving no treatment. This is the nineteenth reported case to our knowledge, and the eighth with bilateral disease.

7/27. azoospermia in testicular sarcoidosis is an indication for corticosteroid therapy.

    OBJECTIVE: To report improvement of azoospermia and hypogonadism after high-dose corticosteroid therapy in a patient with testicular sarcoidosis. DESIGN: Case report. SETTING: University hospital. PATIENT(S): A 27-year-old man with testicular sarcoidosis and azoospermia. INTERVENTION(S): High-dose corticosteroid therapy was commenced in an attempt to improve sperm count and restore gonadal function. MAIN OUTCOME MEASURE(S): Analysis of sperm count, T, and gonadotropin response to steroid therapy. RESULT(S): FSH and LH concentrations decreased and T levels increased in parallel with control of disease activity with steroid therapy. Repeat semen analysis demonstrated a significant increase in sperm count, allowing sperm banking to take place. CONCLUSION(S): High-dose corticosteroid therapy may be indicated in testicular sarcoidosis, not only for control of systemic disease activity but also for recovery of gonadal function and spermatogenesis.

8/27. Hypoechoic testicular mass: a case of testicular and epididymal sarcoidosis.

    We present a case of testicular and epididymal sarcoidosis in a man with a hypoechoic testicular mass. Radical orchiectomy was averted by use of intraoperative frozen section analysis.

9/27. Testicular presentation of sarcoidosis and organ preservation: case report and review of management strategies.

    We report the case of a 24-year-old Cuban-American man initially presenting with a 1-year history of bilateral testicular pain, subsequently diagnosed as sarcoidosis. The diagnosis was histologically confirmed intraoperatively with excisional biopsy of the right tunica. Genitourinary complaints are a rare initial presentation of this disease. Although therapeutic approaches have varied in published reports, no definitive protocol for treatment has been established. We suggest confirmation of the diagnosis, followed by symptomatic treatment, reserving orchiectomy and steroid therapy for severe, refractory, or disseminated cases.

10/27. sarcoidosis presenting as intrascrotal mass: case report and review.

    sarcoidosis is a multisystemic disease that usually involves the lungs and lymph nodes, but almost any organ can be involved. Genitourinary involvement with sarcoidosis is extremely rare. We report the case of a 30-year-old African-American male who presented with a right-sided intrascrotal mass and diffuse lymphadenopathy. On further workup, he was found to have sarcoidosis. Two months of corticosteroid treatment resulted in the disappearance of his intrascrotal mass.
