1/12. Tubercular involvement of the thyroid gland: a report of two cases.

    Thyroid tuberculosis is rare. In the last decade, however, the incidence of extrapulmonary forms of tuberculosis has increased. We report on 2 cases of thyroid tuberculosis. In case 1, a tubercular abscess mimicking acute thyroiditis was found which was correctly diagnosed by fine-needle aspiration biopsy (FNAb). No evidence of active disease was noticed. Pleural thickening on chest X-ray was the only sign compatible with a previous infection. In case 2, tubercular thyroiditis with lymph node enlargement was also diagnosed by FNAb in a reevaluation setting. In both cases treatment with antitubercular drugs resulted in complete recovery. Thyroid tuberculosis should be kept in mind in the differential diagnosis of thyroid nodules, notably in patients with a history of tuberculous disease. FNAb represents the main approach to making the diagnosis.

2/12. US and CT findings of tuberculosis of the thyroid: three case reports.

    With two ultrasonographic and two CT films of three cases of thyroid tuberculosis, we evaluate the ultrasonographic and CT findings and correlate them with the pathologic findings. They are demonstrated as heterogeneous hypoechoic mass on ultrasonogram and peripheral-enhancing low-density abscess on CT scan with regional lymphadenopathy. ultrasonography (US) and CT can help the diagnosis of thyroid tuberculosis.

3/12. Acute tuberculous abscess of the thyroid gland.

    Isolated tuberculous thyroid abscess is an extremely rare form of infection of the thyroid gland. We present a case of a female patient who presented with an acute abscess of thyroid gland associated with pain and fever and without cervical lymphadenopathy. We describe the pathology of the disease and the imaging findings with their differential diagnosis.

4/12. Solitary pancreatic tuberculous abscess mimicking prancreatic cystadenocarcinoma: a case report.

    BACKGROUND: The incidence of pancreatic tuberculosis is extremely rare, and it frequently misdiagnosed as pancreatic neoplasms. The nonsurgical diagnosis of this entity continues to be a challenge. CASE PRESENTATION: A 33 year old male with six-month history of intermittent right epigastric vague pain and weight lost had found a solitary pancreatic cystic mass and diagnosed as pancreatic cystadenocarcinoma. The chest x-ray film and physical examination revealed no abnormalities. Abdominal ultrasound (US) examination showed an irregular hypoechoic lesion of 6.6 cm x 4.4 cm in the head of pancreas, and color Doppler flow imaging did not demonstrate blood stream in the mass. The attempts to obtain pathological evidence of the lesion by US-guided percutaneous fine needle aspiration failed, an exploratory laparotomy and incisional biopsy revealed a caseous abscess of the head of pancreas without typical changes of tuberculous granuloma, but acid-fast stain was positive. CONCLUSIONS: Pancreatic tuberculosis should be considered in the differential diagnosis of focal pancreatic lesions, especially for young people in developing countries.

5/12. A thyroid tubercular abscess and bilateral symmetrical hilar lymphadenopathy: a rare association.

    An 18-year-old boy presented with a rare association of a thyroid tubercular abscess and bilateral symmetrical hilar lymphadenopathy. He was put on a Category I regimen with standard short course daily chemotherapy of four anti-tubercular drugs under the National tuberculosis Programme. After a six-month of anti-tubercular treatment (ATT), the boy showed clinical and bacteriological improvement. The thyroid scan with technetium 99 (Tc 99) and the chest skiagram also became normal.

    tuberculosis of the thyroid gland is very rare, with an acute abscess formation being the least common form of presentation. We report the sono-graphic features of two cases of tuberculous thyroid abscess that were confirmed via ultrasound-guided fine needle aspiration biopsy.

7/12. tuberculosis of the thyroid bed presenting as recurrent medullary thyroid carcinoma.

    A 69-year-old lady with previous medullary thyroid carcinoma treated by total thyroidectomy alone developed a swelling in the neck. This was thought to be a recurrence of her carcinoma but was found on investigation to be a tuberculous abscess of the thyroid bed. She was treated successfully by surgical drainage and antituberculous chemotherapy.

8/12. Tuberculous abscess of the retrosternal thyroid gland displacing the oesophagus.

    Infections of the thyroid gland have been reported for various organisms, bacterial, parasite, fungus and virus, acute or chronic, suppurative or non-suppurative. Two rare conditions when combined are even rarer, the tuberculous abscess of the retrosternal thyroid gland, causing extrinsic pressure effect and producing dysphagia. Rapid relief of symptoms occurred after surgery and followed after antituberculous therapy.

9/12. Extrapulmonary tuberculosis with paravertebral abscess formation and thyroid involvement.

    Extrapulmonary tuberculosis has a broad spectrum of clinical manifestations. Involvement of the thyroid gland has rarely been reported. We report a case in a patient with a non-tender swelling of the thyroid, whose symptoms, sonographic and scintigraphic features mimicked carcinoma. Initially the patient presented with back pain due to a paravertebral mass. Fine needle aspiration and culture of the aspirate both from the paravertebral mass and the thyroid ensured the diagnosis. Although seldom observed, tuberculosis should be kept in mind in the differential diagnosis of nodular lesions of the thyroid. The preponderant absence of thyroid dysfunction in mycobacterial thyroiditis is confirmed by this case.

10/12. Tuberculous pancreatic abscess: an unusual manifestation of hiv infection.

    Pancreatic tuberculosis is unusual occurring in only 2.7% of autopsy studies of persons with miliary disease. Reports of focal pancreatic tuberculosis are rare, even in patients with the acquired immunodeficiency syndrome (AIDS) in whom abdominal tuberculosis is a frequent feature. We describe two patients infected with the human immunodeficiency virus (hiv) who developed tuberculous pancreatic abscesses. In both this was their AIDS-defining illness.
