1/347. Spontaneous bladder rupture: rare cause of peritonitis.

    Spontaneous rupture of the urinary bladder is an uncommon, but important, cause of generalized peritonitis. It is a surgical emergency which may be rapidly fatal if diagnosis and treatment are delayed. Bladder disease or obstruction, coupled with a sudden increase in intra-abdominal pressure usually accounts for the rupture. Characteristic symptoms are acute lower abdominal pain followed by generalized peritonitis. In most cases, the rupture is intra-peritoneal.

2/347. Congenital bladder diverticula in children.

    BACKGROUND/PURPOSE: The authors report their experience with the management of congenital bladder diverticula in children. methods: The authors reviewed the histories of six boys (mean age, 4.4 years) in whom congenital bladder diverticula was treated from 1980 to 1996. Diverticula were unilateral in four patients and bilateral in two patients. All patients presented recurrent urinary tract infection, and two boys had several episodes of urinary retention. Secondary kidney damage was present in two patients with ureteral obstruction and one with vesicoureteral reflux. Surgical treatment was undertaken in all patients. RESULTS: After surgical treatment, none of the patients has had recurrence of the diverticula, and all remain asymptomatic. CONCLUSIONS: Congenital bladder diverticula have a wide clinical spectrum and could lead to severe kidney damage. urinary tract infection and urinary retention are the most frequent presentation forms. Surgical treatment should be indicated in all symptomatic cases according to each anatomic and functional situation.

3/347. hamartoma of the urinary bladder.

    BACKGROUND: Hamartomas of the urinary bladder are extremely rare. We report on a case in a 58-year-old female who presented with the chief complaint of pain on urination. methods/RESULTS: cystoscopy revealed a solid tumor on the left posterior wall of the bladder. Transurethral resection of the tumor failed to provide a definitive pathological diagnosis of the tumor. Thus, we performed partial cystectomy. The pathological diagnosis was hamartoma arising from the urinary bladder. CONCLUSION: This is the ninth case diagnosed as urinary bladder hamartoma to be reported in the literature.

4/347. Spontaneous rupture of the urinary bladder in the alcoholic patient.

    A case of a young man with an acute abdominal condition and hematuria is presented. BUN and SCr levels were markedly elevated. Retrograde cystography revealed intraperitoneal extravasation of contrast material. At exploration, a large intraperitoneal bladder perforation was noted and repaired in two layers. Recovery was uneventful. The presentation, diagnosis, and treatment of spontaneous rupture of the urinary bladder are discussed.

5/347. schistosomiasis of the urinary bladder.

    Authors review a case of urinary schistosomiasis, where the process caused left-sided urinary obstruction. Because of the suspicion of a tumor transurethral resection was performed, whereafter the ureter passage became unhindered. diagnosis became clear upon the histological examination of the resected tissue. Based on the cited literature, reference is made to the mortality rate of the disease in the expanded endemic areas, as well as to the high number of patients at risk. A brief summary is given of the pathology, symptoms, diagnostics of the disease, with mention of differentiation and therapeutic possibilities as well. With regard to importation of the disease, attention is called to the importance of careful anamnesis.

6/347. endometriosis of the urinary bladder.

    Although every seventh or eighth case of endometriosis is found to have urinary involvement, this type of endometriosis is still rarely recognized. The symptoms are not specific and are frequently suggestive of a neoplastic process. On the basis of our two cases of histopathologically two types of endometriosis as well as literature review the practical significance of endometriosis in urology is described. In each patient who previously underwent a gynaecological or surgical procedure in the abdominal cavity, endometriosis of the bladder is likely to occur. This possibility should not be ruled out, otherwise it could lead to misdiagnosis and inappropriate treatment.

7/347. Inflammatory pseudotumor of the urinary bladder and sigmoid colon.

    We report an unusual case of an inflammatory pseudotumor of the bladder. A biopsy specimen prior to surgical treatment suggested leiomyosarcoma, and computerized tomography and magnetic resonance imaging strongly suggested bladder tumor with sigmoid colon invasion. Accordingly, radical surgical treatment consisting of total cystectomy, anterior resection of the sigmoid colon, and ileal conduit was performed. After operation, however, histological examination revealed an inflammatory pseudotumor of bladder and sigmoid colon. These findings were confirmed by immunohistochemical staining for smooth muscle specific actin, desmin, S-100, and vimentin. Histological examination revealed a diverticulum of the sigmoid colon in the middle of the pseudotumor, and this diverticulum may have ruptured to cause the inflammatory pseudotumor of bladder and sigmoid colon.

8/347. Cyst-like chylous coagulum in the urinary bladder of a patient with recurrent chyluria.

    We report a rare case of recurrent chyluria in a 64-year-old man, who had undergone sclerosing therapy for chyluria with complete remission 40 years previously. The chyluria recurred 4 months before the present hospital admission. The patient presented with acute urine retention and weight loss of 6 kg during the past 6 months. urine biochemistry studies showed elevated concentrations of albumin (0.7 g/dL), globulin (0.6 g/dL), cholesterol (0.59 mmol/L), and triglyceride (0.89 mmol/L), confirming the diagnosis of chyluria. Intravenous urography showed a substantial filling defect in the urinary bladder. cystoscopy revealed milky urine flowing out of the right ureteral orifice, and a large chylous coagulum presenting as a cyst-like mass in the urinary bladder. The chyluria was successfully treated with sclerosing therapy with injection of 76% Urografin (Shering AG, berlin, germany) into the right upper urinary tract, and the chylous coagulum was evacuated endoscopically. No evidence of chyluria recurrence has been noted during a year of follow-up, and the patient has regained 6 kg of body weight. This particular presentation of chylous coagulum in a chyluria patient has not been previously reported in the literature.

9/347. Paravesical granuloma after inguinal herniorrhaphy. Case report and review of the literature.

    Paravesical granuloma after herniorrhaphy is an unusual complication due to infected suture material and often mimics bladder or urachal malignancy. We present 3 cases of this disease and reviewed 21 previously reported cases. Our patients underwent hernial repair 2-7 years before examination and presented urinary symptoms. They were treated with antibiotics and underwent exploration of the inguinal wound infection but the symptoms did not resolve. En bloc excision and partial cystectomy were performed and resulted in complete resolution of the symptoms. It is important to consider paravesical granuloma in patients who had undergone herniorrhaphy in the differential diagnosis of bladder or urachal tumors.

10/347. Laparoscopic excision of endocervicosis of the urinary bladder.

    Endocervicosis is a benign lesion in which endocervical mucosa develops in anatomic locations distant from the endocervical canal. Two cases of infiltrating endocervicosis of the urinary bladder were managed by laparoscopic partial cystectomy. This avoided the morbidity of laparotomy and provided improved visualization of the extent of bladder resection required to remove the lesion completely.
