1/3. Ophthalmological findings in nephropathia epidemica in Lapland.

We report ophthalmological findings in 15 cases of nephropathia epidemica. The patients, 13 men and 2 women, were 20 to 62 (mean 30) years of age. The onset of the disease was characterized by high fever, nausea, headache, abdominal pain, backache, somnolence, red throat, proteinuria, and oliguria. The symptoms subsided rapidly during the polyuria stage. Transitory myopia occurred in 8 patients (53%). Conjunctival injection and haemorrhages were seen in 3 patients (20%). One patient had acute glaucoma with oedema in the cornea and shallowing of the anterior chamber, with subsequent anterior uveitis and haemorrhages in the ocular fundus, and another patients had acute glaucoma. Three patients had photophobia which occurred in 2 patients without any glaucoma or anterior uveitis.

2/3. Spontaneous hyphema associated with anterior uveitis.

Few reports have described hyphema in association with anterior uveitis. Five cases of anterior chamber haemorrhage are reported in patients with five different anterior uveitic entities: Reiter's syndrome, juvenile chronic arthritis, ankylosing spondylitis, idiopathic anterior uveitis, and herpes simplex. hyphema has been reported in association with idiopathic non-granulomatous anterior uveitis, but not with the other four entities. In three cases, iris rubeosis was present. In two cases the patients were taking non-steroidal anti-inflammatory agents. The hyphemas occurred at times of heightened inflammation and resolved spontaneously without complication in all but one case, a boy with idiopathic uveitis who required surgery to remove the blood. The clinical outcome of these cases provides evidence that conservative medical management is usually sufficient.

3/3. Bilateral anterior uveitis and retinal haemorrhages after administration of trimethoprim.

In this case report we present a female patient with recurrent urinary infection treated with trimethoprim/sulfamethoxazole, trimethoprim alone and ciprofloxacin. She developed adverse systemic symptoms and bilateral anterior uveitis following administration of the former two drugs, and in addition retinal haemorrhages after the use of trimethoprim. uveitis and retinal haemorrhages are rare side effects of trimethoprim/sulfamethoxazole. To our best knowledge they have not been described as a side effect of trimethoprim alone.
