1/4. Colouterine fistula complicating diverticulitis: a case report and review of the literature.

    Colouterine fistula, secondary to diverticulitis, is an extremely rare complication, and only few cases have been reported in the literature. We report the case of 76-year-old woman, who presented with vaginal discharge over a three-month period. Pelvic examination and laboratory investigations suggested a colouterine fistula, that was confirmed by non-invasive imaging. Surgical treatment was a one-stage, en bloc resection of the uterus and sigmoid colon. In the presence of severe inflammatory reaction or paracolic abscess, a two-stage procedure should be safer. Otherwise, a one-stage procedure, en bloc resection of the uterus and sigmoid colon is preferred, as we cannot rule out a malignancy prior to surgery.

2/4. mucocele formation 20 years after an appendiceal uterine transplantation for infertility mistaken for hydrops tubae profluens.

    A 56 year old woman was admitted to our hospital with a 9-year history of recurrent, lower abdominal pain and mucoid vaginal discharge 20 years after an appendiceal uterine transplantation. The removal of the uterus and the attached appendix resulted in the disappearance of the symptoms. A mechanism linking the appendiceal mucoid discharge with abdominal pain in this menopausal patient is suggested.

3/4. Primary chylous vaginal discharge in a 9-year-old girl: CT-lymphangiogram and MR appearance.

    Chylous reflux is a manifestation of primary or secondary lymphatic obstruction. Primary lymphatic obstruction is defined as lymphangiectasia and incompetency of lymphatic valves without an underlying cause. Lymphangiectasia resulting from trauma, neoplasm, irradiation, or inflammation characterizes secondary lymphatic obstruction. Leakage of chyle into the uterus, vagina, bladder, or rectum can occur with either primary or secondary lymphatic obstruction. We report a patient with chylous vaginal discharge, a rare presentation of primary chylous reflux syndrome. CT-lymphangiography and magnetic resonance imaging clearly depicted this disorder. To our knowledge, only 20 cases of chylous vaginal discharge have been reported previously; chylous vaginal drainage occurred in the absence of chylous uterine reflux in only three. Although this is a rare anomaly, chylous reflux should be considered in a child with chronic vaginal discharge and lower extremity swelling.

4/4. Rare urogenital anomaly causing discharge and pain. A case report.

    BACKGROUND: patients with mullerian anomalies usually seek help because of poor reproductive performance. CASE: A 16-year-old, white woman presented with a persistent, brown vaginal discharge and right lower quadrant pain. Because of voluntary guarding, the uterus could not be palpated on bimanual examination. However, transvaginal sonography showed a right cystic mass and a slightly binodular contour of the uterus. Laparoscopic evaluation revealed the presence of a bicornuate uterus. A hysterosalpingogram showed a double uterine cavity and cervical canal below the right uterine cavity, leading to a cystic, paravaginal mass. Incision of the mass and drainage of its chocolate-brown fluid content revealed a small, blind vagina leading to a second cervix. Using a wire probe, an isthmic communication was demonstrated between the two uterine cavities, leading to the final diagnosis of bicornuate uterus, laterally communicating, with a double cervix and vagina, unilaterally blind. CONCLUSION: awareness of the possibility of this clinically puzzling anomaly will avoid delayed or unnecessary surgical treatment.
