1/9. autoimmune diseases in a Nigerian woman--a case report.

    autoimmune diseases (AD) are conditions in which there is the development of antibodies against self cells/ organs. AD could either be organ-specific or non-organ specific (systemic) in clinical presentation. Commonly reported ADs includes: myasthenia gravis, Hashimoto thyroiditis, Guillian-Barre syndrome, vitiligo, type 1 diabetes mellitus, Graves diseases, Goodpastures syndrome, pemphigus, rheumatoid arthritis, systemic lupus erythematosis, Addisons disease, multiple sclerosis, pernicious anaemia, autoimmune haemolytic anaemia, chronic active hepatitis, idiopathic thrombocytopenic purpura. There is paucity of locally documented information on the occurrence of AD in same patient in our environment. We therefore report the case of a 66 year old woman who presented at the University College Hospital (UCH), Ibadan, with a spectrum of the AD, vitiligo, rheumatoid arthritis, myasthenia gravis, impaired glucose tolerance.

2/9. Concomitant vitiligo and psoriasis in a patient treated with interferon alfa-2a for chronic hepatitis b infection.

    Well-known cutaneous side effects of interferon (IFN)-alfa therapy include dry skin, pruritus, hair loss, and psoriasis. Presumably because of its potent immunomodulatory activity, the use of IFN-alfa has also led to the development of autoimmune diseases in susceptible individuals. vitiligo, an autoimmune cutaneous disease, has been reported to occur during IFN-alfa therapy. We report a 10-year-old girl with chronic hepatitis b infection in whom IFN-alfa treatment induced de novo development of vitiligo and psoriasis. Neither skin condition improved after withdrawal of interferon treatment. Concomitant occurrence of vitiligo and psoriasis due to IFN-alfa has not been reported previously.

3/9. vitiligo associated with pegylated interferon and ribavirin treatment of patients with chronic hepatitis c: a case report.

    The use of pegylated interferon (PEG-IFN) and ribavirin is considered standard therapy for patients with chronic hepatitis c. Many adverse effects of IFN appear to be of autoimmune origin. vitiligo is a hypomelanotic disease, whose cause, despite many investigations, remains unknown, although some evidence points to an autoimmune pathogenesis. This report describes a case of vitiligo that occurred during the third month of treatment with PEG-IFN and ribavirin. The sustained virologic response was the result of a 52-week regimen; however, hypomelanotic cutaneous patches persisted. Autoimmune adverse effects of IFN therapy, which may include vitiligo, should be carefully monitored. The decision about whether to discontinue treatment should be discussed with the patient, who must be informed that the lesions may persist.

4/9. Chronic active hepatitis associated with vitiligo, nail dystrophy, alopecia and a new variant of LKM antibodies.

    In this report we describe the case of a 7-year-old boy, suffering from autoimmune-type chronic active hepatitis (AI-CAH) associated with vitiligo, nail dystrophy, alopecia areata and a variant of liver kidney microsomal (LKM) autoantibodies. This patient's antibodies are different from LKM-1 which are directed against cytochrome P450 db1. They react predominantly with perivenous hepatocytes in contrast to LKM-1 antibodies which homogeneously stain the whole liver lobule in immunofluorescence. In Western blot analysis this LKM variant reacts with a liver microsomal protein of approx. 50 kDa, but not with recombinant LKM-1 (cytochrome P450 db1) antigen. Immunosuppressive treatment led to a normalization of liver histology after 1 year and a significant improvement of vitiligo and alopecia areata. In summary, a case of autoimmune-type chronic active hepatitis is presented which is associated with a new variant of LKM antibodies reacting with a 50 kDa microsomal protein different from cytochrome P450 db1. Furthermore, this patient suffers from extrahepatic syndromes (alopecia, nail dystrophy) that have not been described previously in LKM antibody-positive chronic active hepatitis.

5/9. Human immunodeficiency virus-associated vitiligo: expression of autoimmunity with immunodeficiency?

    Persistent viral infections have been postulated to be trigger factors for the development of autoimmune disease. We report the development of vitiligo in four patients with human immunodeficiency virus (hiv)-related conditions and in one patient with hepatitis who later developed both psoriasis and acquired immunodeficiency syndrome (AIDS). Other common features were hepatitis and multiple other viral infections. ribavirin was associated with repigmentation in one patient. vitiligo may be an example of an autoimmune disease triggered by viral infection in a genetically predisposed host.

6/9. porokeratosis of Mibelli associated with active chronic hepatitis and vitiligo.

    A patient with porokeratosis Mibelli is reported who suffered from long-standing chronic active hepatitis and rapidly expanding vitiligo of more recent onset. This type of disease association has never been reported before, although it is in line with several reports of porokeratosis in association with immunoregulatory disorders, mostly after drug-induced immunosuppression. The lesions of the present patient responded well to treatment with topical 5-fluorouracil.

7/9. pityriasis rubra pilaris, lichen planus, alopecia universalis and vitiligo in a patient with chronic viral hepatitis c.

    A 43-year-old man, affected with alopecia universalis and vitiligo since his childhood, developed erythrodermic pityriasis rubra pilaris followed then by lichen planus; chronic viral hepatitis c was diagnosed. This appears to be a unique constellation of cutaneous diseases implying both autoimmunity and hepatitis c virus.

8/9. Interferon-induced vitiligo in a patient with chronic viral hepatitis c infection.

    vitiligo is an autoimmune disease characterized by depigmentation of the skin due to destruction of melanocytes. interferons have been used for the treatment of chronic hepatitis c and some malignancies. We report interferon alpha-2a-induced vitiligo in a male patient with chronic active hepatitis c. All skin lesions disappeared completely without requiring therapy after discontinuation of interferon. This case suggests that vitiligo may be developed during interferon therapy as a side effect.

9/9. vitiligo associated with alpha-interferon in a patient with chronic active hepatitis c.

    We report a case of vitiligo that occurred during the second month of interferon alpha 2a therapy for chronic active hepatitis c. The patient received the interferon for four months.
